To help patients better understand the opportunities and potential risks of exercise in the presence of valvular disorders (including aortic stenosis and mitral regurgitation), we interviewed Dr. Allan Stewart, the Chief of Cardiac Surgery at Miami Heart Institute in Miami, Florida. Dr. Stewart, who has performed over 5,000 cardiac operations, is a triathlete and marathon runner who has trained with many of his heart surgery patients.
Key questions addressed by Dr. Stewart in this video include:
1. Is it common that heart valve disease impacts people who are physically fit and eat right?

I am 79 I have watched my cardio output for years thinking going down due to aging. I am overall very fit being a runner for years and CrossFit for the last 9 years. 10 weeks ago I had open heart surgury to replace my aortic valve and correct prolapse and regurgitation of the mitral valve. Today is 10 weeks since I left the hospital. I am doing walk run for up to 9km and back at CrossFit for 3 weeks. I will build back stronger over the next 3 months.

Good show 😊in 10 weeks of open heart surgery u r doing great…I hv done TAVR proceedure on 4/09/2023…do about 5-6 Km/day…hope to recover fully after 15 weeks….n my aim is do brisk walking 10 Km/day…I am about 69 years old now living in India!!!!!

hey iam a 18 year male from greece.. i was diagnosed with bav with 22mm aorta one year ago ..i went to a cardiologist and said that i should stop weighlifting any kg above my weight..then i went to a private hospital and the cardiologist there said that i should not have restrictions on weightlifting and other exercises..iam so worried because bodybuilding is a part of life and i dont want to stop doing it ..whats your opinion?

Thank you for the video. I've had BAV associated with 44m ascending aorta dilatation, 26 years old, no symptoms at all. Of course, I will consult that with my cardiologist but still, I would like to ask you, whether from your perspective there are some restrictions regarding the bodyweights exercises and weightlifting for small muscle groups (biceps, triceps, shoulders) in a high range of reps? It's not clear for me because you can fatigue the muscles a lot in that type of exercise as well.

I would not restrict you at all on bodyweight exercises.

Thanks for the great video Dr. Stewart. I just received a phone call from my cardiologist that I have mild to moderate Mitrial Valve Regurgitation. What took me in is that when exercising I was having shortness of breathe. I am 49 years old, former professional triathlete and runner. I am getting ready for the Leadville Trail 100 MTB race (all above 10k ft) and doing a marathon in 4 weeks after to qualify for Boston. I had a stress echo that diagnosed this, I did a run today and compared to 2 years ago, my avg was 40 beats more over same distance and effort. I am currently on blood pressure medicine (just started this year). While the cardiologist is good, they do not specialize in endurance athletes and I am only 49 (they said they usually see what I have in 70+ year olds). My big question is , can I still keep racing and training hard, doing ultra endurance races and running marathons fast? I do not have to do these now, I just do them for fun, so I can do regular exercise. Anyway any guidance would help, kinda freaking out a bit. Thank you

I’m 32 years old, also an athlete and was recently diagnosed with mild to moderate mitral valve regurgitation and wondering the same thing. No one in my family has heart disease, so this was shocking 😢

Great question! As each patient situation, diagnosis and anatomy is unique, we would encourage you to discuss this directly with your medical team. Given the current status of your mitral valve prolapse (mild, moderate or severe), your doctors will be able to provide you specific medical advice specific to exercise and your mitral valve prolapse.

Question: I was just viewing this video. I know that this video is a year old, however my question is for a patient in their mid 50s not over weight is presented with 3 Moderately leaky valves (Aortic, tricuspid &Mitral) since I live on a Horse farm I’m active most of the time as the owner is not physically able to do the grunt work in taking care of them. My chore routine is daily cleaning out of 50+lbs of material times 4 and moving hay bails that weigh slightly more but just awkward to move. I can no longer ride the horses because of a pars fracture from 15 years ago. If I choose to have surgery to fix my back at some point, is it safe to have the surgery with my valve issues? Another question: re: riding my bike, what level is safe, staying on flat roads and moderate incline like hills or no hills? I know this seems silly. I would hope you can clear up the activity issue. How much is too much or should I not do anymore?

Great video, I wonder if bodyweight squats for high reps, so no added weight would be safe?

Bodyweight squats are fine. The limitation is to prevent the intense buildup of intra-abdominal pressure from not breathing, Deadlifts, heavy squats, heavy bench pressure, etc requires you to close your airway and tighten your diaphragm. This increase in pressure translates to increased pressure in your heart. All body weight exercises are fine...with the exception of press handstand

Dear Doctor, Two of the valves of my heart are damaged. One require replacement and another one repairing. I also suffer from dry cough since many years. And so I feel that the surgery may not be successful (how will the cardiologist surgeon manage if and when I want to cough) Thank you Levitt Niangtee North East India

I’m 32 years old and recently got diagnosed with mild to moderate mitral valve regurgitation. No history of heart disease in my family. I lift weights and do kickboxing, very active lifestyle (at the gym 5 days a week) and wondering if it is safe to continue doing this. Thank you

Is TAVR technique only for the aged people???

Hi Tina, Great question. TAVR availability is not really about age. It's more about the risk of the patient. TAVR was originally for inoperable and high-risk patients. @@HeartValveSurgery thanks for replying.. Actually my son who is 11years old has congenital Bicuspid valve disease. As doctor says that we only have to observe him and TAVR is not for him.. I am scared of open bypass surgery in future... Is there any treatment for him. Pls suggest.
