The air crash that killed R&B singer Aaliyah had more safety holes than swiss cheese. John Goglia and Todd Curtis examine at the Bahamian authority report of the 2001 event.
Eight passengers and a pilot were onboard the Cessna 402B charter flight. John and Todd discuss the details of the accident report and the multiple organizational issues of the aircraft operator.
They find that shoddy airline operations are at the center of the cause of this event. They talk about the specific shortcomings that involve everything from pilot verification, to aircraft maintenance to illegal operations.
John and Todd talk about the management oversight and systems needed to safely operate a charter airline. They also cover the responsibility of people booking these flights to check out who they are working with.

You have an overloaded plane on a hurried flight, piloted by two tired and possibly stoned pilots..what could possibly go wrong?

I was reading about this accident a few days ago, thank you for teaching me so many new things May I suggest an accident for comments? Here in Brazil the GOL 1907/ Legacy N600XL crash is still very controversial in so many aspects, and I know the CENIPA and NTSB didn’t agree on the final report. To be fair, they did agree that the pilots from GOL didn’t do anything wrong and the CVR (available to the public from a public inquiry) is so sad, you can hear them working on the engines and gear while the fuselage breaks during the free fall of the aircraft in the middle of the amazon. An american plane with brazilian pilots and a brazilian plane with american pilots. Changed and thankfully improved our industry big time.

the winglet on the corporate jet sliced off the wing of the GOL airliner..two inches one way and it would have missed, two inches the other way and both planes would have crashed.

The Jenni Rivera accident is another tragic ,avoidable event. There were so many things that the charter company did illegally that cost several people their lives. It’s too bad her “ team” put her on this plane without doing their due diligence in regards to the charter company.

While I don't have a direct first person perspective from a celebrity manager's point of view, it seems like they casually book an air charter like booking a ground transportation charter (car & driver). Very concerning since we've seen the consequences of an 'eh, that'll do' attitude.

Greg Feith is my cousin and he told me that 747 was brought down by a missile over Long Island, NY. We were at a family reunion a few days after and he was a lead investigator for NTSB at that time and he told me his colleagues told him it was a missile.

I've given you guys a thumbs-up for your effort. But, realistically speaking, what effect do you think you are having on the GAl accident rate and the number of innocent fatalities involved? Who's really absorbing your preaching and complaining about the FAA Part 135 industry? Likely no one, frankly. The real question is what are the aviation insurance companies doing to put the hammer down HARD to keep questionable/unsafe operations on the ground? The answer is - surely not enough. And offering discounts on GA aviation insurance to someone who might have listened to some old guys telling sea-stories about days gone by is counterproductive to keeping fly-by-night operations out of the sky. Aviation insurance companies - wake up!!!

Valid points, but at the end of the day these guys have more impact saying this than not saying it...for example, if my daughter was a celebrity, I would watch over her transport operators. I might not have otherwise.

@@aerotube7291 Totally agree - we need to keep pushing to make things better. The average person has no idea about the difference between part 91, part 135 or part 121, and nor should they. They are making the (wrong) assumption that people are honest and following the rules, and that if these operations were not legal they would be shut down by the authorities. I'd love to see the FAA hire a bunch of retired pilots to hang out at airports and audit these kinds of flights. It could be easily funded by the fines imposed and the regular FAA budget.
