Hello, Dr.Tarnay, I am Maria Rodriguez you were my Doctor 23 years ago in Olive View Hospital in Sylmar, California. At the time, I delivered a premature baby and I hope you remember that I named my son in honor of you, Mark Chris, and I always pray for you and may God bless you every single day of your life. You are the best Doctor in the whole world and thank you for everything you did, for visiting my baby every day at the NICU and all you did for me at UCLA when he was transferred there. I used to have leaky bladder and I wore pads, but I came across the solution by accident. I was having other health issues and decided to change my diet to an antiinflammatory unprocessed wholefood diet. Guess what happened? Besides healing so many other issues, my leaky bladder totally went away. I was so surprised! I eat fruits, veggies, seeds and smaller lean clean meats. No dairy and no gluten. I kept a food diary to figure out my personal food intolerances and got them out. I believe that the root cause of so much of this is from inflammation and diet is huge.

I have discovered something really strange. You know when you have to pee and it's almost your brain tells you too late and you sit on a chair or couch you hold it in and you can't get up because the water would run out of you if you stand up or try to walk to the bathroom? (If that wasn't the longest question ever) What you can do is drink water right in the moment of the urge. Wait a couple of seconds and the urge is gone. And you can walk to the bathroom without an accident. That is so paradox, it doesn't make sense, but it works all the time.

Well done Another simple kegel idea is to just stop urinating 2-3 times while passing urine by squeezing those muscles You can adequately feel whether you are producing enough pinch if you can get the flow to stop If at first you don’t succeed keep trying it improves

OUTSTANDING video! I grew up in a nursing home while my mother worked there. Then went to school and spent a little over 20 years working in it myself. I wasn't medically nieve until this point😬🙄. As a child, I had a few traumas leading up to about 10 years old, and I needed surgery. I'm unsure what was done, but I had terrible spasms and was unable to urinate. I believe it had something to do with the sphincter. I'm 57 years old right now. I still have difficulty with leakage and full-on urination when I laugh too hard, cough, and the big one, making love with my husband. It's gotten SO much worse. I'm humbled by this video and grateful for the ideas to bring up to my doctor. Thank you so so much!

Do not go the artificial mesh route. It hardens/contracts overtime as it binds into your tissue, can come apart, cause pain and is almost impossible to remove the imbedded pieces it leaves. . My opinion based on an in depth documentary and women’s comments who’ve gone through this.

After intestinal surgery ( 14 inches from middle of chest down to privets) I later realized I had trouble with urine leakage. My surgeon told me they had to cut so many muscles he was not surprised. Well, here it is 4 years later, and at times I wear pull-ups, other times just a pad. I still travel, do what I want etc., but I always pack enough to cover my stay. I am so active, people have a hard time believing that I deal with this.....but I do,,,,,every day. Do not despair, if I can do this.....so can you. After my husband died, I have remained single, and I am fine with that issue. As said I am very active, animal rescue work, fossil hunting, gardening, fishing, vacations, painting, I am always doing something, staying busy.

To be honest, I like male providers better regarding my care. It's always been assumed that female providers will be more relatable but I just find them to be intrusive & dismissive. I went to a male provider once for my birth control pills and he was very respectful of my boundaries & answered my questions thoroughly like you do in this video. thank you.
@IAMGiftbearer
@IAMGiftbearer Год назад
That may be because woman have an extra tough time competing with men in medicine and are under pressure to be extra assertive to the point of becoming aggressive throughout their training and residency and they carry that into their relationship with their patients. That said, I don't think it affects all women doctors this way. I had a very compassionate female doctor who did pelvic surgery on me when I had fibroids and problems with my period. Unfortunately her practice stopped taking both of my forms of insurance and I had to stop seeing her. Then later she moved out of state. I have not been impressed with any of the replacement doctors I've tried since both male and female, so currently I don't have one for these type problems. Many doctors in general are dismissive and intrusive and I find many don't like to answer questions and take offense to it especially if you ask intelligent questions they don't know the answer to. I'm getting ready to dismiss my second Gastroenterologist because of the latter. The previous one was a male and he was just as bad about that and even put snarky comments in my chart about my asking questions that he thought were outside his specialty. When you have multiple medical conditions you can't always know what symptoms are connected to which condition because there can be alot of overlap. I'd rather ask and find out it's not related to that specialty than not ask and have a doctor miss something, thus delaying treatment. I've reached the point at my age that if their fragile egos can't take that then they can kick rocks and I'll take my business elsewhere. I think it has alot to do with a doctor's individual personality and how welk or badly they cope with stress and uncertainty. Males are actually socialized to have all the answers and to fix everything, so if anything I would think more males would be insecure about things they can't immediately solve than women, but I'm sure there are exceptions in both genders.

My mother who is 90 now, had urgency and UTIs for about 10 years. Was on all kinds of medication. We finally talk to a dietitian. Her doctor said that was the last thing she could try other than surgery. The number one thing the dietitian said was to get off sugar. IT WORKED!! After 2months of being off sugar in any form, she was able to start having a small amount of food with sugar in it. Buy sugar I mean including fruits, and some vegetables that are high in carbs and sugar. It was hard at first for her but I was able to be in charge of her diet which really helped. It’s been a year and she eat small amounts oh

I am almost 62 yrs old now and I am free of pads for good! All grains made my muscles weak and the first one to go was my bladder muscles. Who would of thought that grains can cause so much trouble?

There's one exercise that fixes this. Lie on your back but raise yourself up resting on your forearms. So head and upper back is off the floor. Now raise one leg while keeping the other an inch off the floor. Raise as high and as long as you can hold without your other leg touching the floor. Lower leg but not to floor. Repeat with other leg. At no point should your head or your upper back or your legs are to touch the floor until you're done exercising.

My mother had an op 40 years ago and did not help but made the issue worse. She wished she had never had an op. Therefore, hearing that from mother from time to time… when it happened to me, any op was out of the question for me after mother’s experience. I’m a retired Physical Therapist specialized in Pelvic Therapy for urinary & fecal incontinence as well as painful intercourse and pelvic pain in general. We treated about as many male patients as female patients at UF Health (previously Shands) outpatient PT in Gainesville,FL. I looked up the Nilah Control Plan which from what I read is a vaginal probe with electrical stimulation and biofeedback. This sounds similar to what we used however there is no substitute for professional patient education specific to each individual’s diagnosis and in the case of males (some status post prostatectomy secondary to prostate cancer) or females with fecal incontinence the use of a rectal probe. Anyone interested in finding a pelvic PT can contact the APTA (American Physical Therapy Association). Hoping this is helpful for all with these types of problems.
@53sandie
@53sandie 3 года назад
Also I’d like to say that females that undergo surgery (a bladder lift, etc) that still has incontinence after surgery, it’s not the doctors fault (or the surgery) but it may mean the pelvic muscles are still weak and in this case seeing a trained Pelvic PT is almost alway successful depending on the patients compliance. love the information this MD offers and that he says that kegels are effective but I believe conservative therapy (pelvic PT) should be the first option.

I have not been in any research studies, however what you cited about women experiencing incontinence for about 6 years before talking to someone about it was spot on. I am 50 and after about 5 years finally decided to talk to my PCM about my stress incontinence. I really do wish that i, like so many others, did not feel so embarrassed about this condition so that i would have sought care sooner. I actually have my first appointment with a Urogyn provider in two weeks. Your video played after something else i was watching by Dr. Eric Berg on urinary incontinence. The timing of seeing your video was perfect for me. I just want to thank you for advocating for bringing this medical condition "out of the bathroom" in an attempt to encourage all women to seek timely treatment! This was, I think, the most useful video I have seen in years. Aside from anything else, such a relief to know how frequent this occurence is! I began to have 'urge' issues very suddenly, a few years ago, and was embaressed and concerned. I am in my mid 70's, and for a while it was a big issue. I had not seen the muscle exercises, and will be doing that as of now. I used to be called "Miss Ironbladder" at work, because I could control my bladder all day. As a baker, you just can't nip off when you have a machine going , making merringe, for example....so it became second nature to 'hold it'. It came as a nasty shock to lose that control, I can tell you. Thank you for all the information,

I have what he is describing here as “urge incontinence”. I have not given birth. No children. Plus, I have danced all of my life (since 1956 age 4), and taught dance and performed professionally since I was 26, plus taught yoga for the last 20 years and Pilates/Garuda for the recent 5 years. I have always maintained an active lifestyle. My abdominal & core muscles are VERY STRONG. I have performed “kegels” with little or any difference. So I was amazed to hear/see this Dr. describe my exact situation: latchkey syndrome-as soon as I unlock the door to my house I am racing to get to the bathroom. Cannot get my coat/shoes off fast enough! This sometimes starts as I park my car in the garage and stand up from the car door to leave. Also, it happens perhaps 30% of the time after I wake up. I cannot get to the toilet fast enough!🥺 One comment below mentioned the possibility of spinal impingement/stenosis as a potential factor. I do have this: stenosis in L4-L5 plus much, much more. Anterolisthesis/Retrolisthesis L4/L5/S1 or commonly called: spondylolisthesis.

I have not told him about my in continence! But I’m going to after 2 bad episodes this weekend!! I’m curious if your dr has ever suggested the neuromodulator to you. If so, please send me back a note on if you did it and how it helped. Thank you so much! Kay Coleman Melton

On those occasions, ( running half asleep) I have learned to wear skirts to bed with no underwear because every second counts. I have wondered however, if the door/latch syndrome is also a visual trigger, maybe my brain sees how close I am to the toilet causing the bladder to start the evacuation process. What do you think? Dr. Tarnay presents an excellent cutting edge review of the options with OA. I am one that has suffered with it for 30 years! However, some of the side effects or lifestye adjustments with many of the "first options" are extremely inconvenient and/or are just as or more problematic than the original problem. Also, he doesn't mention that surgical solutions can be exhaustive, a time-consuming commitment and expensive, often not covered under the best PPO's. Tthis is to say nothing of the search for a copasetic physician. If you're up for spending a good portion of your dwindling time, particularly as an elder, with a urologist, you need someone local (driving and distance particularly in southern California where Dr. Tarmay is located can be a logistical issue), who has your back. I have interviewed many in my area who are book smartand experienced bar none, but lack the compassion and kindness that is typical of oncologists. The only one I found who seemed to fit was Dr. Lesser and he doesn't do female urology!Dr. Tarnay is a man. I'd like to hear the same from a female gyn/uro who has experienced OA bladder.

Thank you so much for doing this webinar. I will say I actually cried because I thought I was the only one that had the “lock & key” syndrome. It feels so embarrassing to have urge incontinence, especially when I am a very independent person. It feels so much better to know I’m not alone. I did talk to my doctor about this and it was basically ignored. I’m glad you have given me more tools to advocate for myself and help my situation. Thank you so much!

Yes, vaginal births can increase the possibility of incontinence, however I know women who went through labor and needed an emergency cesarean that also have issues with incontinence. Something you might want to mention in your video.

The leaking after urinating can be helped by after urinating stay on the toilet 30 seconds longer then you'll have a mini urination that empties the bladder more.

@@patscorner7183 I thought I knew how to do kegels but a dr’s explanation on youtube explain it differently by tightening and holding both #1 & #2 areas, Big difference huge difference especially if you do kegels at least a couple times out of every week. I had a sling procedure at age 38. I'm now 63. It didn't work, I didn't go back to doctors. When I left the hospital I was told to not get constipated that was all the information I was given. You are so right in what you said. I now don't have sex as it is so painful and wear pads every day. I actually don't go out much. I think I just may go back and talk to a doctor. I will look up how to do the pelvic floor exercises and do them. Just recently during lockdown for Covid lost 90lbs still got 30 or more according to the charts for my height. But I would like to be intimate with my husband of 47 yrs. THANKYOU for this video. X

It is unfortunate when education is neglected. They should have done better. I hope you persevere and reach your new goals. Your husband should be supportive.
