This is a lecture about flash pulmonary edema and its initial management in the emergency department. It was given by Dr. Eric Zevallos at the Rural Emergency Medicine conference presented by GCEP on 9-26-15. He covers management updates including the use of high dose nitroglycerin, pulmonary ultrasound, and current controversies in management. If your treatment of this life threatening condition doesn't go much past oxygen and lasix, you are in desperate need of this lecture!

Great talk! I just saw this yesterday in the ER. I'm coming back to ER nursing after a 15 year absence. I seem to remember seeing this when I was working in ER in the early 1990,s and the term FPE. Seeing this yesterday I was looking for some info. to explain syndrome and treatment. So this was great. I started back only 2 days ago so was only helping with this pt. so don't have all the details. 50ish obese woman when I got involved was being I think admitted for r/o cardiac to telemetry. When my preceptor and I were taking over she had just been started on heparin and nitro drip had just been ordered and we were going to hang it. Again sorry I don't have all the background details. Literally as we were setting up the pump the pt got significantly SOB and of course anxious. Internist was seeing at the time, ER doc came in and took over and tx proceeded. I didn't hear FPE mentioned but any way treatment ended up being nitro. lasix attempt at CPAP and ... sorry lack of info. Cut to chase she was headed to the ICU(no beds) so the long and short of it we cured her in the ER when we got the ICU bed she didn't need it and she still went to tele. Pressures during episode 230/140 etc. sats. 50's, 3-4 hrs. later 1000cc diuresis sats on 2 liters 95% BP 145/84. Given the treatment not sure if the ERMD was thinking FPE or following different approach. I plan to discuss with him and possibly turn this into a teaching opportunity for my fellow nurses. Thanks again!

Tremendous! I can't get the nurses to go above 40mcg/min. Might try the bolus! Anyway, thanks so much!

I think that lower doses of NTG are sufficient to decrease BP

No more love for nitroprussise?? Too expensive? Too toxic? Not as effective?
