Sir que 29 it was ecg shown and his bundle ecg . Q was p wave correspondent to h spike or j spike

Sir q 29, about bundle of his, an ecg and his electrogram was given. We were supposed to identify A wave which was correspondent to P wave of ecg

sir question no.22 format was that diagnosis of COPD was given... options had different curves.. and the image you showed was among the options..

Yes doc there are four possibilities Only : Obstructive , Extra-thoracic obstructive , Restrictive and intra-thoracic Fixed obstructive. The curves are discussed in details in flow volume curve part of the app in pulmonology section. Thanks for the feedback

And in 21st question, it was given that 'patient's fingers turn white to blue to red when exposed to cold temperatures and she's also having ulcers on fingers and something like that'. I marked the secondary raynauds because of the ulcers!

No doc secondary is due to association with connective tissue disorders like scleroderma or sle or dermatomyositis . Primary is idiopathic wala.

@@bharatraj1031 doctor the concept of clinical presentation has to be clear. Trust me all fall in place. When you read a topic think as if you have to give advise to a relative and hence have to be able to answer all their whimsical questions .

Read from text book and not google. ARNI is the major mortality reducing drug in HFrEF .

Sir why in the psychiatric counselling question, hyperthyroidism is not the answer? It can also give rise to anxiety like symptoms.

Hyperthyroidism causes sym stimulation plus many other features which were not given . Pheo causes exclusive sym stimulation features and is the first DD to be thought in case of anxiety non responsive to medication .

Sir, I completely agree with your bouncer theory but in cricket, just 1 or 2 bouncers are allowed in an over so in ODI 50-100 bouncers are bowled out of 300 balls and we were prepared for those but that wasn’t the case in this exam, every second ball(even more) was a bouncer !!!

Sahil I 100 percent agree. Now what is the solution. Fight the Indian system ? Follow the Indian system ? You are young and pumped up . So was I at your age but as a middle class medical student with ageing parents I decided to follow the system and get out of this rotten system to do my own practise and then teaching .

Hello Dr. Parmar, We recommend the English version if you are attempting the June 2021 exam. However, if you plan to attempt either Dec 2021 or NEXT 2022 exam then we would recommend the Hinglish version.

Sir in que. 31 aldosterone causes salt and water reabsorption so why not hypernatremia?

Hello Doctor the trigger for aldosterone release is low sodium values. Aldosterone cannot over correct and lead to hypernatremia. It will bring sodium to normal. So the original deficit is hyponatremia. Lastly alkalosis causes hypocalcemia . For details of same i suggest to watch electrolytes video of 2 hours in kidney section in PGMEE. It is concise and will help you in internship also .

I got the PHD (pheochromocytoma ) question correct, but other than that I am sure I messed up rest all of them Most of them I overcomplicated it in my head especially regarding various management. I hope they keep the similar pattern for Dec exam better than by hearting one liners. Thanks for the explanation Doc. Loved you said that real life is not Greys Anatomy haha. Have a blessed day.

Yes Doc they have to ease the pressure on young minds. Doctors are always suppressed , under paid and under estimated. But still there is something crazy in our neocortex that makes us go back to books daily. It is only a doctor who does gets guilt feeling if he does not read books one day. Non-medicos/ relatives/ media/ you tubers will never understand us.

Blood for Cryptococcal antigen was not in option Sir.

it was LFA Cryptococcal antigen. It is done on blood . Two cultures are not rapid so ruled out . India ink has low specificity . So has to be the last one. Most are solved by exclusion and not directly .Lateral flow assay is being used in COVID rapid detection too

Sir I remember searching for Cryptococcal antigen but as much as I remember only blood and CSF antibody was in the options hence I had to choose Indian Ink. I might be wrong Sir. Thankyou.

Sir, in the 20th question, SDH wasn't in the options! I'm definitely sure. I marked the intracerebral hemorrhage.

4 options were cortical venous thrombosis , extra Dural Hemorrhage, intra cerebral Hemorrhage and subacute cortical venous bleed. Choice 4 is same as SDH

No doc the fetus needs oxygen which is an additional work load for mother. A 8 month pregnant lady has reduced TLC as gravid uterus limits diaphragmatic excursion and this is made up by increasing the respiratory drive and rate

@@mdazimahammad8206 yes..even prasan vij sir has explained it clearly that respiratory rate remains constant in pregnancy,apart from this TLC , vital capacity also remains constant.. however tidal volume increases by 40 prcnt,inspiratory capacity also increases,FRC decrease by 30 prcnt..oxygen consumption increases by 20 prcnt while as co2 decreases since woman will be hyperventilating..

definitely, dr. deepak Marwa sir, is the best teacher in the world. I cleared the exam back in 2011 with the help of Diams coaching. I don't why the fmg screening exam is getting more and more complicated. we need more and more doctors in this tuff times.
