I am filling this out for my blind wife (Trying to see if she requalifies for SSI), and I was wondering - is it I who should primarily fill this out with my information because I do work but we are struggling hard, or is it her information who should be put on here primarily

I believe that the person signing this is giving the SSA permission to go through his or her own earnings information, as provided by payroll providers. But I would double check with the SSA to make sure that this is the correct way to go in her SSI application process.

Hello, what if you don’t give permission will they stop your benefits?

The SSA cannot stop benefits that you're entitled to, simply because you're not allowing access to payroll provider records. However, this form is likely used in situations where earnings history is incomplete, and the SSA has a better opportunity to obtain historical data from a payroll provider. In cases like this, *not* allowing access would probably result in lower benefits, since the SSA might not have other ways of obtaining this information.

Would this form be used for reinstating Medicare Part B

This is not the form that you would use to request reinstatement for Medicare Part B. Depending on your circumstances, you may be able to request reinstatement through the Medicare Good Cause policy, but that is not the purpose of this form.
