** What happened **
This crazy incident started off with a bag lock malfunction that quickly turned into a one-in-a-million freak accident -- the hard housing passed through the 3-ring assembly -- which had to be cut way with a hook knife to avoid an entanglement with her reserve.
** Why did it happen **
The Initial Malfunction
We can’t see the initial malfunction, but the jumper said it was a bag lock. Bag locks occur for a wide variety of reasons, but most are related to improper packing procedures. Often the mistake is putting too much excess line through a stow and creating a large "bite" that can then catch and lock onto a neighboring bite on the d-bag during the deployment process.
The Incomplete Cutaway
It appears that in, what can only be described as a freak accident, this jumper’s hard housing passed through the second ring on her 3-ring-assembly and prevented a clean cutaway of her main parachute. Her rig was inspected, and it was confirmed that her container was properly assembled. One rigger we consulted believes that the exposed hard housing was possibly whipping around and could have passed through the second ring before the reserve deployment created enough pull to cause the cutaway to occur.
** How could it be prevented **
Bag locks
Diligent packing is the key here. When packing a parachute, skydivers should make sure they’re doing it slowly, taking each step one at a time, and not rushing. Complacency often occurs even with the most experienced jumpers because they’re chatting with one another during a pack job or rushing to make a call. Jumpers should make sure to not put too much excess lines through their stows; no more than 2 inches, which can help prevent the stows from potentially catching on one another. One instructor who viewed this video also noted that forgetting to cock a pilot chute could have potentially caused this incident; the main could have left the container but, without the snatch force from a pilot chute to pull through a tight stow, it could have stayed in the deployment bag until the jolt from the reserve opening finally started the deployment.
The Incomplete Cutaway
As noted earlier, this appears to have been a one-in-a-million event that can only be written off to divine intervention. Some incidents cannot be prevented but, fortunately, this jumper was heads-up enough to recognize what she needed to do.
** Additional Notes **
Hook Knives
A DZM I know once argued that hook knives should be a mandatory piece of equipment included in gear checks. This video is the perfect example of just how important a hook knife can be to saving your life. Notably, the jumper in this video said she only started carrying a hook knife a week before this incident and never thought she would need to use it. We’re really happy she did.
Canopy Separation
We also wanted to note that this jumper does an excellent job preventing these two canopies from entangling. She knew that the spinning main canopy posed a risk and did a good job keeping it away from her reserve while she got out her hook knife for the (literal) cut away.

That's a hell of a hook knife ! I've been a career firefighter for over 25 years and we tested at least ten different "seatbelt cutters" (hook knife), and never found a ONE that was worth a s**t ! They would all bind up under load. This just amazed the hell out of me. Nice job !

@randyporter3491
@randyporter3491 4 года назад
Brian Peck You're right Brian. When we conducted testing, we used tubular webbing and flat webbing, of different widths, including standard seatbelt strap. We put them under varying tension, from 25# up to body weight. Some of the cutters did cut, but not consistent, or always cleanly. Some of the most expensive models, cut the worst. We were also testing pocket knives with a cutting hook, which were mostly useless on most materials. The dual blade razor cutters did the best. But, like you said, under tension.

Then road accident statistics should stop you taking driving lessons. Considering the millions of jumps per year: skydiving is a very safe sport. Many malfunctions are caused by jumper error or packing errors rather than equipment failure. As long as the canopy is in good condition packed correctly and deployed correctly, there will be no issues. This video shows a malfunction that rarely happens and was dealt with accordingly, she landed safely under her reserve: how many near misses do drivers have but don't tear up their licence?

Great video however she reacted slowly which could’ve put her in a worse situation if the parachute was in a spiral entanglement. She’s very lucky this malfunction wasn’t worse or it would’ve had a different outcome. Should have had the hook knife on her chest strap or somewhere easier to reach.

This is probably a super dumb question, but is it ever possible, if this was to happen, to pull in your main, gather it up and like hold it bundled up like between your legs or something?
@skydiverclassc2031
@skydiverclassc2031 4 года назад
There is an incredible amount of tension on the risers and lines, even in normal situations. Add to that the centrifugal force of a spinning canopy and the short answer is no. On top of that, the time spent in trying to reel in a partially inflated canopy is altitude lost, a significant factor in skydiver fatalities. You can see how low she is when she finally gets it under control.
@yurigui6080
@yurigui6080 4 года назад
Sadie E I’m both a skydiver and a paraglider pilot. As a skydiver, I was never taught what to do when the 3 ring won’t release. So I guess it depends whether you have a hook knife on your rig with you. As a paraglider pilot, the main is connected by carabiners that are hard to disconnect, and the standard procedure in a two-out scenario is to do a canopy transfer onto reserve, then reel in the collapsed/entangled main.

@ryananderson4437
@ryananderson4437 4 года назад
Yuri, If you ever get a riser stuck at the 3 ring during a cutaway the procedure is to first make sure your cutaway cables have completely cleared. If still stuck, grab your riser and pull it. Sometimes during cutaway the force isn’t enough to clear the three rings. Sadie, it would be very challenging to pull this parachute back in. This jumper handled the situation correctly. Tried to clear her three rings. When that didn’t work she used her hook knife. IF she didn’t have a hook she could have tried pulling it in but it would be extremely difficult plus the more important thing by that time would be figuring out the safest way to control her reserve parachute
