Sir, heat application increses inflammation also ryt? So how can it help in reducing swelling? Sometimes patients come with swelling and he gives history of giving hot pack and it has increased swelling.
i didn't say heat application reduces swelling...heat or cold application for space infections is controversial topic..spread of infection mainly depends on host immunity and local barriers..namely bone, periosteum and muscular attachments..mere application of heat can't worse the spread...main reason behind heat application is heat produces vasodilation so that blood circulation to the infected area increases which favours our defense cells and antibiotics to reach infected area to fight with microorganisms to reduce infection.
there is no need to give just prior to i&d...if the patient is nontoxic then we can do i&d and then prescribe antibiotics for 5 days...if the patient is toxic then one or two days of antibiotic usage before i&d is helpful...prolonged antibiotic usage before i&d is not recommended because it may result in antibioma ...
