The sick come from those folk wandering around outside in the middle of the night. Zoomed in graphics always shows plenty of people out and about at all hours of the night. The big question I have no actual answer for, is WHY those folk are wandering about. The only suggestion I have is that they are heading to storage depots to find meals. The cookhouses are closed, so they cannot get food from there at those times of the night. This is why it is important to get the steam hubs as powerful as possible - they take the edge off the ambient temperature. (Helps, but does not prevent illness.) The biggest swarm of ill seems to arrive around 5am. Perhaps folk getting ready for the day's work? It is noticeable that the game always slows from whatever speed to minimum, at both 5am and 7am. So there would seem to be "significant" happenings at 5am - perhaps folk waking up and leaving their warm beds to search for breakfast before starting a work shift. Also, if the work shift is extended, it starts at 6am, so they have to leave home early to be at work for the start.
