By asking if they understand the concept, she’s saying that she realizes grandma is certainly not in on it. I know the theme is about Karen who is infamous for being rude and negative but I think this restaurant should make an exception for elderly guests by being less harsh to them. And the staff should also stop their ruthless acts when they do a few tries and notice that an old lady has no idea of what this place is. I mean there is nothing wrong by using our brain and analyze things every now and then.
