Well this explains why I didn't even know Packie was in gta online! I remember stumbling upon that police van but then it glitched, the radar icon disappeared and it just drove away
GTA IV fan fact: Niko changes his facial expression when wanted. We already know this of course. What may be lesser known is that his "wanted" expression differs from his other "angry" expression. Easiest way to tell the difference between the two is to observe his face as you gain and lose a wanted level right after making Niko angry. This needs to happen swiftly however, as Niko will only remain angry one in-game hour (two minutes). Additionally, Niko will get angry whenever: 1: Busted 2: Wasted 3: Failing a mission 4: Other reasons (Not entirely sure what causes it, whether it's traffic accidents, damage to Niko, or both). Easiest way to make Niko angry will be to call Brucie to arrange a race and then cancel it, as this will count as failing a mission.

I was doing a setup where you had to get a tractor trailer away from the cops then drive it out to Sandy Shores area.. It had a silver box, and the whole time it kept changing different skins (logos) on the top of it.. even showed Suburban clothing logo. Never seen something so weird lol.
Rescuring Packie is the worst glitchy mess I have ever seen. It's straight up broken, you have to be lucky for it to spawn, lucky for everything to work properly, and even then game might decide to give you the drop off point in Paleto with 2 minutes on the timer. I don't know who programmed this but they didn't do a good work.
Patrick McReary Random Event is still bugged nowdays. It may spawn for you and for other players. Usually it gets interrupted immediately (sometimes I saw some other guys in the van, one of them was wearing a Sombrero hat). I've managed to complete it when I was on a setup for Casino Heist with a friend.
The Packie McRreary random event is exactly that, a random event that spawns in the game on a timer. Even if you already saved Packie, it'll always spawn. If you stop the van then there would be no one in it but the cops. I just joke by saying, "oh there goes Packie getting busted again."
