A "Hammer-Toe" deformity, describes a pathological condition of abnormal or exaggerated contracture at the metatarsal-phalangeal and inter-phalangeal joints of the toes. This is mainly due to an imbalance between the muscular flexors and extensors as well as intrinsic interossei and lumbrical muscles within the forefoot.
A hammer toe deformity may present as one of three morphological variations. A true hammer-toe deformity will exhibit dorsiflexion at the metatarsal-phalangeal joint and plantar flexion at the proximal interphalangeal joint. Whereas a mallet toe solely results from a plantarflexory contracture of the distal interphalangeal joint. A simultaneous combination of these two conditions is thus known as a claw toe.
Clinically, a hammer toe may present with hypertrophic callosities on the plantar surface of the corresponding metatarsal head and the distal/plantar tip of the toe in addition to a painful corn over the proximal interphalangeal joint.
A radiographic analysis of a hammer-toe deformity in the Anterior/Posterior or Dorsal/Plantar view will reveal a hallowed point or gun barrel appearance of the middle phalanx.
Although conservative care may involve shoe modifications, padding, strapping, and custom orthosis; surgical reconstruction may be required to alleviate painful and immobilizing hammer-toe conditions.
This surgical management of the hammer-toe deformity is performed by variable means of "Sequential Reduction." By this, a hammer-toe contracture is alleviated through various procedures in order to re-establish a functional position during active motion as well as rest.
This process may include, a lengthening of the extensor tendons, followed by a resection of the extensor hood. An "Arthroplasty" may be utilized to increase useable joint space within the proximal inter-phalangeal joint by removing the head of the proximal phalanx.
In more extreme deformities, a tenotomy of the flexor tendon may be utilized. This may be accompanied by a fusion of the joint itself, known as an "Arthrodesis," whereby the base of the middle phalanx and the head of the proximal phalanx are combined to form one continuous bone mass.
By balancing the forces of plantarflexion and dorsiflexion at the joints of the toe, a Hammer-toe operation may result in a drastic improvement of the functional mobility of the foot and leg during gait.

@@phillipjacobs9982 surgery went well I suppose. There was a pain she had to stick in my toe that stuck out about an inch or two, when she went to pull it out it broke halfway through my toe, so that pain is still in there

Essentially, a hammertoe surgery entails a combination of soft tissue (skin, tendons, joint capsule) release, as well as bone resection (removal or remodeling) in order to straiten out the toe and improve function. The video above, demonstrates this with an anatomic explanation.

Hi DrGlassDPM, I have this condition that affects the second finger of my right foot. Moreover, I suffer since many years ago of plantar fasciitis at the same foot, that "flat foot" condition was always blaimed as the guilty of. However, is it possibile that this longer second finger than the allux is the real guilty of my plantar fasciitis? I am asking this because I read that it is able to cause this problem too, and because I have both "flat feet" but just the right one (that has second finger longer than the allux) suffers of plantar fasciitis, that I "manage" with orthopedic insoles plantar and avoiding run more than 10 kms (despite it was one of my passion years ago unfortunately). Dear God I don't know if I should have watched that. I got this surgery done June 4th. It's true about not being in pain the day of surgery mostly because they will give you an injection in the back of your knee to make everything numb below it. Once it wears off the next day I was in so much pain I felt like someone hammered my toes... no pun intended. Now I'm just waiting for them to pull the pins out of my toes... Way too nervous..

You don't. I had this done to 2 of my toes on my left foot. I couldn't wear a standard shoe for 8 weeks then, once the pins were removed, it took a few more months for final swelling to go down. It is strange to not be able to curl those toes again. Just pray you don't hit those pins on anything while implanted. It was unbearable.

I wouldn't wanna live with the inability to ever move my toes again. I think i would rather live with the Hammer Toe than to think about the idea of having my bones fused together so i can never move them again.

@HaligonianType1 Sorry for the typo. Yes, Toe nails as it were... If they continue to grow back ingrown/incurvated, despite multiple removals: Have you not considered a Partial or Total Matrixectomy? By this, they actually kill the nail root and the toenail doesn't return. This is an option for troublesome nails. Since you mentioned not wanting to waste your time with multiple appointments, I would imagine that bypassing an attempt at nail spacers and such are may be more right for you.

The joint that is fused can not longer be bent. It is immobile after the pins are removed and the joint is fully healed.

@Chiangsterr It would not be termed a "neccessary" procedure without the presence of pain or discomfort. That's mainly because of the risk of complication. Imagine a painless toe now being painful after the operation; it's a rare side effect, but still possible.

@audayhussain2 This particular procedure demonstrated is not a wedge osteotomy. This is mearly a "flat" resection of the Proximal Interphalangeal Joint (PIPJ)

Dem bones dem bones! Very well done, brilliant Miss. The foot alone is an incredible engineering accomplishment, let alone the entire body, I don't buy for one minute that there wasn't Devine design at work. I guess if one had enough claw toes they could perch on a tree limb as a big bird.

@allisonportillo The taping of the toes doesn't neccessarily "continue" the healing process, per se. It is intended to hold an optimal position for the toes to heal properly. The configuration and positioning of the toes, via taping, would be dependent on the doctor's procedure and post op orders. Chances are, he/she has you follow-up with them in clinic and the toes will be taped as needed.

@KSWR500Z The muscular balances involved in most hammertoes can present at the time of birth, but not always. Surgery won't affect the shoe size directly, however.

How is the metal sticking our removed after it heals? I want to do this one one of my toes, however 20 years ago my mother had a traumatic experience where the "Doctor"pulled the pin with no anesthetic and pliers! please tell me we have evolved since then?!

I have hammer toes , both feet, second toe longer than my great toe and the 3rd toe on 1 foot also longer than great toe. I have pain now and then but I’m not sure I want to lose the ability to stretch my toes out and bend them and use them to pick things up!! 😀 I have a hard time with shoes and have a really bad habit of curling my toes over the top of flip flops.

@DrGlassDPM its the one that curls up. the long one beside the big toe. its just in that toe and no where else. it feels like it in the first joint at the foot and the 2nd joint. but it feels like the tendon but idk. when i get health insurance im going to go to the doc to see if i need surgery..

It can vary from person to person, but bones usually take LONGER than tendons, to heal, so it is not likely to increase the healing time overall.

@DrGlassDPM I remove my own big toe nails. Have been since I was 15. No pain killers or numbing. Nothing. Then it's just a matter of keeping it cleaned and dressed. Pain is a non-issue for me.

TheSayd007 the pins are there to hold the bones together as they heal. After that they are fixed together so now they can’t flex/extend their toe joint

Just last week I had 6 toes done in the same op. They released the tendons too. Can anyone tell me when the pins get taken out, will I be able to walk a day or so after without the shoes on? I find my special shoes very big as I have thin feet haha

Just had hammer toe surgery on both my feet and let me tell you I’m in excruciating pain. 4 days later and it’s painful to walk to the bathroom, take a whole 10 min moving like a turtle to pee.

Unfortunately I have done the exercise and the insoles.. Didn't work. And unfortunately I have both curled hammer toes on both feet. So I'm going to see a consultan/surgeon to see if he can fix this

@DrGlassDPM I am a Type 1 diabetic, so I am very careful when it comes to keeping them clean. Altho I have never had an issue, and I heal quite fast. And yes, feet rock. Hah... I am off to bed... Sleep safe.

I doesn't really hurt if it is done in a proper way. For interest watch out on our channel. There you can watch a hammer toe treatment from the first interview to the final rehab.

This is an excellent review of hammertoe with a well-thought out layout. I plan to use this video for a presentation although I think I noticed a flaw with the "gun-barrell appearance" part of your video. When watching the part of the radiograph analysis (1:03) it seems as if the camera is directed to a superior view and the toe bends toward the camera. Am I viewing this wrong?

My 2nd toe has a lot of pain and most of the time it makes a clicking noise when I walk it hurts really bad and hurts even when I touch it in a Certain spot I haven't gone to the doctors because my parents think that I just sprained or broke it but I think that it is something else Could it be hammer toe??????

Doctors diagnose is arthritis. It's begins with this pain, few years later will be appeared hammer toe or after Morton neuroma. Plus sciatica. You need vitamins E, B & iron. Also hygiene for foot.. Change your shoes for real leather. It's provided oxygen for foot. Bath them with Epsom salt. Check if skin is dry or sweating. Use pure talk or other medicine. Ask pharmacist what is for. And offcourse contact doctor.

I have had the fusion of one toe and the tendon release of 3 toes. You are off your feet for about 14 days and in bed for about 7 days. The operation is really not that bad.

Had my big toe done , when I was a child. My bit toe is now bent toward my other toes which don't touch the ground. With a sock on, my foot looks like a Woman's pointed High heel shoe. My entire foot is 2 inches shorted than my other one. No pain though.

Deano vincent That would really depend on what you're doctors plan is. It's commonly a few weeks for recovery

I've been told he is going to break and pin I'm 35 and it will be the 4 right foot little toes. Do you have any pics of protective boot please.

I broke the 5th metatarsal six months ago. I then immediately developed a hammer toe and have had surgery. Can the broken metatarsal be the cause of the hammer toe. My toe was okay before the break. The ball of my foot also burns now. Lorri

Lorri Rowe That is why I had to have hammertoe surgery on my left foot several years ago. I never had burning though. Now I have to go in for bunion and hammertoe 2nd-4th toes surgery for my right foot.

Did your toe ever heal? Mine hasn't yet and I had the surgery on Mar. 2nd. I still can't wear my regular shoes and now have to buy 1/2 size larger and have the other one shrunk.

Yes mine did heal. Doc put me in a cast and he would change it once a week for 4-5 weeks.

this procedure seems pretty intense... wonder if the toes will straighten if put in a brace for a few months.
