HealthTran: Provider Sustainability and Patient Empowerment 

Community Asset Builders’ “HealthTran” volunteer driver program began to take shape in 2013, funded by a Missouri Foundation for Health grant. By the end of the pilot in 2016, HealthTran had received 4,725 referrals. The provider-funded program currently provides about 1,700 mobility-challenged clients in more than 40 rural Missouri counties with a way to get to and from medical appointments.
