The HEARTH Project is a quality improvement demonstration project of the existing transition of care process from Department of Defense to Veterans Health Administration Post-9/11 Transition and Case Management. The HEARTH Project uses a predictive analytics model to identify and proactively provide care coordination and case management services using Critical Time Intervention, Whole Health, and Trauma-Informed Care principles to Army Veterans at high risk of experiencing homelessness within 12 months of their military separation or retirement.
