You need to completely change your internet provider. He pulled your information through your ip and essentially hacked into your email and everything to get all the info. I speak on this from a cyber security background. Also begin using VPN. I know it'll cause some lag but it'll be worth it. I've been a victim of this too man. It sucks. It's horrible. And the fact that they can get away w it is worst.
Database leak seems more likely. You don't get all that information from an IP address alone. An email / db leak gives you an entire line of info on a person
