High flow vertebro-venous causing compressive myelopathy

Case of extremely high flow vertebro-venous causing compressive myelopathy. The left vertebral was tortuous and hugely dilated along with retrograde filling from the right vertebral. Huge challenge to control the embolization material in such flow conditions. We placed two triaxial systems in the left vertebral artery to create stable access and used large size 035 fibered coils to trap and occlude the AVF. Strategy and access played a key role in doing the embolization with precision (and reasonable costs). First time in my life I placed six catheters in the vertebral, but after the access, it was a short procedure :)
