In this webinar for patients, HSS orthopedic surgeons Dr. Ernest Sink, Dr. Danyal Nawabi and Dr. Anil Ranawat and physiatrist Dr. Peter J. Moley discuss what the hip labrum is and how a labral tear can cause pain that requires surgery. They also address who should have surgery for this condition and who fares best afterward.

Great information and a wonderful team of doctors. Awesome that you consult with one another. Would like to have seen excessive femoral version touched on as this can be a cause of hip pain and labral tears as well.

Dr. Danyal Nawabi at 3:55 states that the hip labrum "has a good blood supply." But a "good blood supply" suggests that the hip labrum also has a potential to heal on its own. So hip pain resulting from labral tearing could become asymptomatic if given enough time to allow for healing, and if the patient ceases the activity that caused the tear. Would have appreciated some discussion on this point regarding labrum's blood supply and healing potential.

Pretty sure that once torn there is a gap in the tear so it can’t heal fully.

Thank you for your presentation! I myself have had multiple lower back surgeries by a experience neurosurgeon, going to him, this time I did have my MRI with contrast, and while speaking to the radiologist I did mention that hip area. As a point that I wanted him to concentrate in. I am presently going to physical therapy and hope that I don't need any large surgery. I have had shattered vertebrae's in my neck being replaced with titanium. Successfully. Also, antagonizing synovial cysts in my lower back area., To reverse total shoulder replacements, and I plan to see a couple of different doctors to get the analogies. I certainly want to think the doctors for the video! This was helpful

I went to chiro, acupuncture, family doctor, PT....with more back pain than hip. Finally found a Pelvic floor doctor who was a God send and found the tear. Have been dealing years with this discomfort mostly from the back and pelvic floor compensating. Headed to surgery this July. Praying for a complete recovery to go back to running and just basic living 👏🏼🙏🏽

Great webinar! I have been to the ER about 6 times over the last 5 years, 3 in the last year. The last time was for back and hip pain. Went through PT for back. Still have hip pain. Saw a hip doctor who recommended a MRI. Dr. saw a subtle tear of the superior labrum. Scheduled for corticosteroid injection on June 15. I am a very active 72 yr old male. I run 5-6 miles per day and bowl 3 times per week. I really hope that the injection works.

First, this webinar is welcome relief to me that there are medical specialists who absolutely believe in a team concept to really get a patients problem and ultimately the solution. This taught me so much and I happened on it by accident as I await an MRI for literally an 8-9 on pain on a 10 Pain Scale. Like many who likely get to this point, this likely should have been handled months ago after I was injured. I was in Orlando and was walking in a backyard at the place we were staying at night. The place had a pool. It was dark outside and I accidentally, while walking at a quick pace, stepped into the pool off the edge. For lack of a better explanation, I essentially got split in two. The right leg went all the way in the water, while the left leg bent awkwardly and i caught myself after right side submerged on the edge of the hot tub. Hot Tub was a part of the pool. I instantly knew I was hurt. The pain was terrible. I couldn’t walk or put any weight on my right leg (leg that went in water and hit the bottom). I couldn’t straighten my foot. We all thought I broke my hip. Went to urgent care and they sent me straight to the hospital, but when urgent care said, if it is broke you have to have surgery, I didn’t want to be stuck in Orlando for surgery if it was broke. My daughter was competing in world’s cheer finals the next day so I knew if I could get through he night and the next day, I could fly home and get to a hospital I knew. I could not walk those two days and we bought a wheelchair. It was a tough deal but didn’t want to make a huge scene. When we got back, I went to our specialized orthopedic urgent care. They took an X-ray. The PA attending that evening said, “It’s not broke, just a bad hamstring pull”. Take some ibuprofen and stretch. I left completely frustrated. Still couldn’t walk and knew it had nothing to do with my hamstring. My hip, by now, was black and blue and it was also cut where I hit one of the concrete structures. Everything was swollen from my hip to my elbow. The pain persisted so I saw my Spine Specialist and pain dr. I am fused l4-s1 and have neck fusion. I have a high threshold for pain. This hip (or bad hammy) was seriously bothering me. My Dr, did an MRI of my lower lumbar spine. It showed a big herniation and it penetrated the thecal sack. I got a steroid injection to see what relief I might get. I get steroid shot in S1 and various Lumbar vertebrates and Cervical spaces. I know they take a while to work in some cases. It seems to have helped with the outer leg pain, but the groin pain continued to grow and I had now been limping since the first week of May. The pain in my groin is awful….thought it was hernia maybe…..anything. It just continues to get worse and to the point where it hurts so bad you feel like you should go to emergency room because something has to be going on……yet, I don’t want to do that and take the place of a heart attack patient or someone in really bad shape. Went back to my dr. Thursday. Can’t lift my right leg from sitting position and every way he tried to move it about put me through the wall as i jumped out of my skin. My dr. Said you may have a hip labrum tear - let’s get an mri of the hip and see. He also told me that in traumatic emergencies, sometimes damage isn’t picked up right away on X-ray. The X-ray the PA used as the absolute - it’s your hammy. I have an mri Wednesday (no contrast) to see if there is damage in the hip. Listening to you guys gave me hope. I have known pain in back and neck. I used to be a runner and now with this, I can’t even react quickly without my leg giving out completely. I feel the pop and i still have to put my right foot out to the left (opposite of pigeon toed) to even limp. I trust my dr. 100%. He has always been right and when he wants to run things by other dr.’s, he does. He is very thorough. He doesn’t want us eating meds and he wants his patients fixed. I really think it is a tear of some sort. I pray they figure it out because this pain is the worst I have felt ever. Back surgery walk cake compared to this. Thank you for giving me hope that Dr.’s do care and ones like you go the extra mile. My daughter wants to be a dr. And I hope she is as thorough as you. Great examples for those young collegians who aspire to medicine such as this. If anyone else has had these symptoms…….severe groin pain, knee starting to get involved, can’t walk, and can’t lift leg - let me know what your diagnosis was. Appreciate y’all.

@@tedshep I'd say it was a success. Had the surgery 4/2021. It's now 6/2023 and I've been so much better. 100%? No but I'm able to walk without a limp and very little pain. However, I will eventually need a hip replacement. I'm just hoping I get a few more years out of the surgery I did go thru. Oh, one more thing... my rehabilitation was quite long. Started going back to work in 12/2021 for 4 hours per day. Then in March 2022 I started back full time.

I find it incredibly funny how much these guys act like they care. But in the real world, you have a hip injury and you actively seek medical help for it. It takes a year to get a requisition for an MRI, another 14 months to get an MRI. another year to get you to a PT that understands your injury, and by the time you end on a waitlist for surgery it has been 5 years. You still don't know the first thing to do so you start finding your own help through youtube.

Doctors, will having a cortisone injection negatively impact what you see from the MRIs/scans? I've had chronic Pelvic instability ( left hip labral tear and L4-L5 issues) and walking creates pain, especially when on crowned dirt roads or non flat yards. But driving, sitting, and being in sexual positions also are painful. 🤔 I'd 💕 to know better positions to enjoy intercourse without pain. 🙏🏼 you'll revisit this post and reply to comments on this thread. I bet this issue is more common than most even know, huh? 👍

@@diannahill9430 Telling the male doctors that this pain was interfering with my sex life was what finally got the surgeons' attention. Can you believe it? Who would've known THAT was the priority to hear so they'd take us serious. Don't hold back. The therapist I'd seen after returning to work was a younger female athlete who hadn't yet born children but was actually 6 weeks ahead in recovery of the same surgery. She'd shared that in women especially which had a hip labral tear repair surgery it's usually just as matter of time before the other side needs to be done too. The left side gets repetitive motion due to getting in/out when driving cars. Ooh, also be sure you know what your bone density score and your vitamin D level are. Many are not in the optimal 60-80 (Vit D) range and often have osteopenia or osteoporosis, too. I'm learning it's often connected to hormonal imbalances, thyroid dysfunction, and thus leading to early menopause. (I was 42. Later discovered I had hashimotos, osteopenia and Osteoporosis. I try to educate young women the importance of doing weight bearing exercises, eating healthy fats and proteins (less carbs & sweets) to avoid this. Using bioidentical hormones, Vitamin D3 and K, being better hydrated now is foundational. 🙏🏼 Follow Dr Mary Claire Haver on social media and see her newest book for help connecting the dots regarding hormonal balancing.
