A great problem is having TOO MANY OPTIONS!
So with many veins to choose from, which one do you choose???
In this video I outline my approach, especially as it relates to the operating theatre environment
1) Try the most distal site that satisfies the purpose
2) Choose between hand, forearm, cubital fossa and arm - each has advantages and disadvantages
3) Consider Left or right (patent preference, logistics of theatre, surgical requirements)

Thanks for video. Please make another one where you discuss difficult cannulation; how to deal with things like rolling wins , multiple valves , scar tissue etc .. also what do u think of ultrasound technology for vein cannulation ? And did you say your assistant???? Here in USA you don’t get one ! You cannulate quickly then get your airway

Fully agree with the environment dictates the location. Working as a paramedic my usual practice is to aim for access in the right side as this is the side we have access to in the vehicle and avoids me standing to access the line. Also agree with the forearm, can be harder to anchor veins but in terms of flows it is usually better. Sometimes anchoring by holding your hand encircled around the limb can be useful especially in the elderly. It’s just important to not use so much pressure it collapses the vein.

Can u explain sometimes the valves of the blood vessels causes blockage of vein ...hw can we prevent that

good question! check out my 3 ways to fail a cannula video. Essentially. if I come to a valve, I pull back a little and it the cannula works fine.... But may not be allowed depending on your hospital protocols ie. if there is a part of the cannula showing out of the skin. I use it for short term iv access.

Hope you've found help elsewhere but if not... I suggest using study materials from Respiratory Therapy/Tech as it is a regular feature of that job. Just remember how much more liability (health & legal) involved as compared to venous. Good luck!

Where would be a good site to look for a vein for cannulation on a patient who's vein may not be visible despite applications of a techniquet or by slapping/flicking the vein?

You truly need to learn to palpate (feel) not only to find a vein but also for it's qualities such as size, depth, valves, etc. While everybody has slightly different anatomy with some of the twists/turns/bifurcations/valves that exist, particularly in distal veins; knowing the basic anatomy is still helpful and should give you a good basis to begin with when finding a vein. One of the best ways I've found to practice is to try finding veins on anyone that will let you... friends, family, classmates, etc. I'm not saying to start IV's on all of them but to simply learn how many different types of peoples veins feel. All it takes is a tourniquet, some time, and it will absolutely help you get better! Also, when you are starting an IV, stabilization of the vein you are attempting to cannulate will absolutely increase your success! I hope this helps some... best wishes to you!

A little late after you asked this but ill chip in here for anyone else reading. Due to the development of lower arms in fetal development the cephalic and basilic on the lower arm are usually in the same spot on most people you find. Thats why i like the cephalic by the wrist as a paramedic, I know right where its gonna be, so i can find it very easily in unstable patients where its hard to palp other veins. and it can usually take large catheters.

Is he a doctor? In Sweden all IV cannulations are made by the nurses. Nursing specialized in Anesthesia being the best and most proficient at it.

Yes, he is. In many countries nurses do not do cannulations or draw blood. For example I'm in Germany and here nurses don't even learn it anymore since they are not allowed to do it anyway. They have to call a doctor everytime a patient needs access and a med student will come each morning to draw blood. Only specialised nurses in ICU and the ER (I think) learn and are allowed by law to do this

Sweden seems similar to the United States: I'm a 15-year RN and nurses do all of the peripheral IV's; specially trained RN's are certified to insert PICC (peripherally inserted central cannula) lines at beside with ultrasound for placement and fluroscopy for confirmation. Anesthesiologists or CRNA's (Certified Registered Nurse Anesthetist) place central lines. I'm an ICU nurse but am often paged to other floors for difficult 'sticks' (cannulations), I became very good at IVs by simply practicing constantly--I'll never turn down the chance to put one in. Some nurses won't even try or will balk at what may appear to be a difficult stick. This is truly a skill that takes large amounts of practice to master.
