At Florida TMS Clinic we treat depression differently. We use TMS or Transcranial Magnetic Stimulation. TMS is an effective, proven treatment for depression that is FDA approved and covered by insurance. TMS is safe and has no systemic side effects. While antidepressant medications and psychotherapy can be helpful for some, many others don't respond to medications and therapy. Here's a heresy: I rarely use even 100% of MT and get fantastic results and 100% compliance. No one leaves treatment.

Have you found bald patients experience more pain on neuro navigation TMS machines where there isn't a swim cap on the patient's head to act as a buffer between the coil and the scalp? Halfway through ITBS 50hz left DLPFC treatment I was at 95% MT on Nexstim neuro navigation (no cap) and then the next day switched to MagVenture (cap) at 120% MT. Despite the 25% dosage increase within a day on MagVenture, it was MUCH less painful than Nexstim. They re-did the mapping when switching machines. Could the cap on the head be reducing the magnetic field by creating a gap between the coil and scalp, thus I'm actually getting a lower dosage? Or could it be the slight variation in mapping that changes the pain? It's a significant difference within 1 day, I know pain is supposed to reduce over time but this is a drastic change within 1 day. The pain on Nexstim would vary wildly depending on wether my head was touching the coil or if there is a 1-2mm air gap. It was very "touchy".

Excellent question that is worthy of a video answer. The quick answer, for now, is: If all other factors are equal, pain may be different from one machine to another based on the pulse width. I will record a video answer which will publish in the next week.

Is it so that each machine has different strength capacity (measured as %)? My take was that all devices use the same range so that 50% on one device is the same as 50% on another. Is this correct?

1:14 “We set it at 60” - What are the units of the dosage numbers you were mentioning like 50, 55, 60, etc? Volts? Milliliters? Miles? Is it 60% of the machine’s maximum magnetic field output and the % sign is dropped to avoid confusion between % of motor threshold and % of machine max output?

You are correct. When we map a patient and we see MT is 50%. This is 50% of the maximum magnetic output of this particular machine that activates this particular patient's brain. When we treat. We aim for 120% of MT which in this case is 60% of the maximum output of the machine. If we change the patient to another machine, we have to repeat the mapping process for the particular machine and particular patient.
