How I wish I had seen a version of this before I started an interim job with an NHS body a few years ago. A splendid exposition; well done the Kings Fund. Good example of a critical friend. Also the animation, information display and production is a great credit to all involved.

I worked in the NHS 1965-2012. I saw so many changes (not always an improvement imho). Proud to have been a part of the NHS.

GPs made to manage their budgets, hospital services given to the private sector affecting the hospitals budgets, PFIs that eat Hospital budgets, "Monitor" overseeing NHS contracts going to the private sector ( 70% of them in 2017) and a Government that still denies that it wants to bust the NHS up for itself and their corporate friends and liberate NHS money to the free market, shareholders and cut patient care to a bare minimum, that's if you don't get kicked off your GPs list because you cost to much to look after.....

except that there have been further changes (PCNs, ICPs) and more afoot (ICS/PHE functions transferring to NHSD or elsewhere) since this was made so in urgent need of an update...

Nightmare burocracy - over 200 senior NHS staff in one group (PHE) - which she didn’t even mention - on over £100,000 per annum ffs... and the government gets blamed for ‘underfunding’ - that’s £20,000,000 on these staff in just one of these quangos. The NHS structure is clearly with this level of confused management which seems to be ever growing, a and possibly the significant part of the funding problem. That’s before we even talk about the insane procurement models they use. One point - if over 90% of all drugs the NHS uses are patent free why don’t they own a robot factory producing them for 0.000001p ? If a shop can make profit and sell paracetamol for less than a quid and the NHS is charged several pounds (and happily accept it) clearly the brains are not connected to the body. Before you rip my throat out I am not criticising front line staff (my sister is a 30 year A&E Doctor and another sister a nurse of 25 years and they both agree with the above). I just want the focus on the real issue which isn’t how many billions any government (of any colour) give the central NHS - it’s what they do with it.

Also both the public and parliament should be able to scrutinise ACOs / ACSs before they are rolled out. The govt has just agreed to a public consultation, but (it seems) are still trying to roll out ACOs in certain areas before the consultation completes. It is unclear if they are actually delaying ACOs and the legislation it requires, although obviously they should do so if they are having a national public consultation, otherwise the consultation is meaningless. No parliamentary scrutiny is on offer currently, which is not acceptable given the nature of the changes being proposed.

I've watched this twice and it's brilliant but still confusing. However, it's confusing because the NHS is a confusing mess and does not function properly. Yes, the frontline doctors and nurses are amazing, but the NHS itself is shit. Here's why. For the past 30 years successive governments have failed to incentivise young people to become doctors, nurses etc. You incentivise them by greatly reducing the rip-off university fees for medical students. Thus we have a massive shortage of doctors and in particular CONSULTANTS! Consequently the only way a person with a serious non-urgent condition can see one in less than a year is to PAY and boy do these guys (they're almost all guys) charge! Usually it can be around £250 - £350 for 40 minutes! That's around £450 an hour. Even at these rates you still often have to wait a while to see them! So, if you haven't the cash you just suffer and suffer and maybe the condition does irreparable damage while you're waiting - that's one big reason why the NHS itself is actually shit.

This was a really informative video (Although I would like to find out more about the different bodies mentioned). It really communicates how immensely complex and innefficient this organisation is. It also makes me wonder if there is a better way of providing free healthcare to 68 million people. After having lived in France for 3.5 years I was pretty shocked at the difference in healthcare quality. The quality of healthcare I received in France blew that of the NHS out the water, and it's totally free for the entire population. Surely there is another way of doing this without handing our NHS over to American VC firms?

NHS is a dynamic organisation and has to adapt to public it serves. Changes are essentials and must be well led and planned, which is clearly not happening. Also, change in such a big organisation cannot turn the previous structure upside-down and change direction every few years: the structure has to have time to adapt to the new direction. And finally, changes in the way health is delivered cannot be driven by a choice 'a priori' to cut down the overall cost of the sector. If there were area of inefficiency in the previous model, they will be remodelled, but the idea that giving less money to the NHS will make it more efficient is frankly laughable! What if the new model of care requires the same money (or more money) than the previous one? On what evidence the reduction of Health Care budget from 8.4% to 6.9% GDP was decided in the first place?

It requires more, a lot more. The NHS is vastly more expensive than it was - because of the privatisation and the way the Tories run it. So much money is wasted and it is unnecessary. Worse than unnecessary, because patient care is cut to afford this newer, more wasteful NHS, taxpayers' money is wasted and people die. We CAN afford the NHS, to say we cannot is untrue. If it was run differently there would be enough money to pay for it.

@@hazelangus no the government cannot afford it. The government has been running massive deficits and has increased debt by quite a bit. This is partly blamed by the ridiculous and absurd healthcare and social service, the prospect of paying that debt off anytime soon is unlikely as the moment the government gets a surplus it will waste it on more healthcare spending.

why not give each hospital a set amount for each service (taking into account the type), so they can manage there own and not be bailed out if they waste

I use to do work on the nhs hospitals and property , I would recive a call from a person at the property , ,number 1 ,, then give a price to a 2 nd person, they would forward it on to number 3 ,, who would get intouch with number 4 , who would ring number 1 and talk to them about it, and then number 4 would send an email to number 2 , who then rang me and sent me an order number ,to do said job, I would ring number 1 , then do job, send invoice to number 2 , who then sent copy to number 4 , who then sent invoice to a separate part of the nhs hundreds or miles away up Leeds, who would release payment , so really I do not see wear all the money is going because they run a tight ship????????

Someone said in the comments that the US needs a system like NHS… I came here to understand how this is better than US corporative healthcare system that we have . Let me tell I did not understand much. Basically branches of branches with sub divisions that control/oversee quality and service?

Why doesn't the NHS provide Xray images to patients yet in Australia the NHS's equivalent Medicare allows patients to receive their Xray images in digital format via email? How can you be sure if you cannot sight the actual Xray images, if the report is accurate and if it is your Xray! due to a lot of administrational errors in the NHS, I had my blood tests mixed up with someone elses in A & E! I also had strange Xray results through the NHS that did not pick up areas that were shown in Australian Xrays!

Well done to The Kings Fund for doing their very best to explain this mess, but this organisation is completely bloody ridiculous! Their video should be called 'How DOES the NHS in England work?' Way too many pen-pushers and mouse clickers. 'Jostling for success is NHS Improvement' 'Who's really in charge'. And way too many acronyms and initials ; ACCs, CCGs, PACs, MCPs, AHSNs, CQC... What about this one: WTFIIAA? What the fuck is it all about?

Noted during lock down with no visitors allowed into a local hospital. Car park full to bursting, not any old cars, very nice cars. Assume staff owned. Reception areas within the Hospital, on every floor, ward, ! Why. Entrance corridor picture gallery of staff. Few doctors, few nurses, untold admin managers and their assistance.why. Cleaning staff, hanging around corridors, mostly of Asian origin, Fact, not racist remark. None doing very much but talking. The NHS is fast becoming an expense the tax payer can not afford. Someone should be looking closely at the present staffing levels at all levels other than Doctors and Nurses. Every migrant crossing the channel, gets freebies, at our expense, why.

It doesn't. A nationalised health service brings practitioner's pay down so less people graduate and work within the NHS. More go private or emigrate. An obvious consequence is the need for medical staff is solved by open border immigration policies which fill the positions with lesser qualified staff but also increase pressures on the NHS via higher demand/more patients As a result we wait longer for a lower quality of care and nationalisation means no competition in medical research and a slowed progress in medical developement. New labour already started the privatisation process over 10 years ago to line their pockets creating an inevitable ticking bomb situation or an abyss into which current and future governments must throw increasing amounts of our taxes.

@@dallysinghson5569 I know. God forbid the general public should have to look after themselves, not panic about mild symptoms or be required to be responsible for themselves. Above all it just needs severe revamping and regulation, like all socialistic aspects of English infrastructure, they've become misused and abused at the expense of those who contribute consistently.

This video is awful. Its message is 'the NHS is too complicated for you' , showing the NHS in a linear fashion of stepping stones for less than half a second per stone with freshly tossed word salad for the viewers to slip on.

This video is actually really helpful for someone starting to work in the NHS... I found it so useful to make sense of what was going on! Though the NHS is so complicated that I don't think anyone can make a video about it without confusing people 😅
