Great video Dr. I appreciate the thoroughness involved about this Rx. I currently take two 8mg tablets a day. I have ran out of these life savers in the past. Rapid withdrawal is a major problem that I don’t recommend. My detox was sneaky, I was unaware that Suboxone would provide a detox. My feelings during a detox include: anxiety, irritability, major fatigue, anger, feeling of total isolation and no appetite. I was able to continue w my script after three days off it. No desire to try weaning to a lower dose.
When I was trying to get off prescription opioids years ago, I asked a doctor for Gabapentin and Clonidine. He handed me an RX slip and I thanked him. Then when I got to my car and read it, it was for Naproxen. I also once complained to a doctor about the chronic diarrhea I was having from trying to taper my methadone dose. His response was "just eat more fiber." A lot of doctors just can't bring themselves to offer any type of help or prescriptions to an "addict."
They have zero Care about their patients, they Hurd us in like cattle spending 1.5 minutes with you typing on their laptop the hypertension meds not even giving one thought as to why the patient's blood pressure is high. I think the medical machine in the States is poor at best. It's a business and their priority revolves around MONEY PERIOD!!🤬
I just want to say, your videos are amazing mate. Im over in Northern Ireland and currently on suboxone moved over from 100mg methadone now im on 16mg suboxone. Im going to start getting off but im scared so its comforting watching this. Thanks Doc :)
Can I ask how you went with the switch? I've been on methadone for too long and am trying to make the decision wether to switch to suboxone or just keep reducing on methadone.
Let me tell you I have been on Suboxone since 2007 mostly and my two attempts to get off of it or for the wrong reasons and I have destroyed my life those two times. One of the main reasons was acceptance into 12-step programs that claim you're not sober. We need more doctors like you in the community wish I was closer.
All these for a month: 16 to 12 12 to 10 10 to 8 8 to 6 6 to 5 5 to 4 4 to 3 3 to 2 2 to 1.5 1.5 to 1 1 to .75 .75 to .5 .5 to .25 .25 to .125 Jump after a month on .125.
I might even say every other day for 2 weeks at the last dose and then every 3 days for 2 weeks and then jumping... But, that injectable he spoke about that gradually leaves your body after 2 weeks for the next 6 weeks.
Quit because u don't want all your money and time going to a clinic. You dont want to be handcuffed to the clinic. Call in, gotta ask to go out of town to enjoy yourself. Gotta be back by a certain time. Your a prisoner to the clinic. You want you life back.
So idk. The clinic I go to my insurance covers. I’ve gone from 16-6 months. I’ve only been tapering a few months. When I tried this with methadone I only ever went up. Heroin same thing Xanax same thing I just always ended up taking more. I hate subs so much I hate the constant headaches. I hate the anxiety the mental effects. I’d like to just be clean. I hate subs
I’ve never had to go to a clinic for mine. Been on it 10 years. I used to have to go to my psychiatrist in person once a month, then it switched to every 3 months, since Covid it’s been every 3 months over the phone. And I just pick up my monthly amount at the pharmacy at Walmart each month. we don’t have suboxone clinics here in Houston tx where I live, (or Ive never seen any or known anyone to go to one) only methadone clinics!
To be fair, that's still a lot easier than counting your regular opioid supply and making sure you have enough to last while you're out of town and running around getting enough to make sure you don't have to cut your trip short because, well, you know. But, once you're on subs for months and years, you forget all the crap you did before. I'm in a money crunch and the cost is what I'm trying to reduce and I did stop to think about the crap I've done to pay for my habit before.
@@pitbullgirlxo I've been going to my doc every month for almost 5 years and he insists that doctors MUST see the patients every month and cannot write refills or multiple prescriptions at once.. I've not relapsed. I've only had to change an appointment once. In fact, it's every 28 days, not every month. . It's 175/visit and the cost of the meds on top of that. Out of pocket for me. He takes two weeks off every 3 months and makes you come in before he leaves even if you just had an appointment two weeks earlier and he writes the Rx to be filled in day 28. It sounds like this may not be true about patients must be seen every 28 days no matter what.
Hello Dr. Jack. I’ve been on Methadone for a long time (second time around since 3 year’s without.) My opioid ordeal started when I was 13 (TBI.) I’m 46 now, but the pain is still real. That being said, I tried Suboxone, but it caused me to break out in hives! Just thinking of the smell/taste of it makes me ill. Anyways, have you ever experienced this in your practice? Btw, I’m a new subscriber and really enjoy your channel and nonjudgmental approach. Thank you🙂
My Substance Use Education class and I just discovered your channel... Thanks for making such informative videos! Myself and others will definitely be learning from them so keep it up!
Best advice ever heard on the topic. Been on since Suboxone came out. Especially can relate to the train analogy. Do u treat patients out of state by chance?
if you do a slow taper the process isnt even that bad. usually can cut down to 6-8mg if you are stabilized higher than that pretty quickly. then just cut down very slowly until down to .25mg then start skipping days until you no longer feel necessary
ya its always easier to say then live with the slow tapered withdrawal. But its way better then cold turkey off opioids thats for sure. But i found if i listened to my body and took it slow enough the withdrawals from tapering were minimal. @@leonidtodorovski635
@@leonidtodorovski635every recovery is different and it all starts in your mind. Someone with a small addiction can have more trouble getting clean than the heaviest user if the heaviest user is fully ready to take on the withdrawal
@@leonidtodorovski635I tapered off in November of 2022. Insomnia was challenging, but it wasn't too hard! Time was on my side and i knew it. I quit smoking 7 months ago and now I'm REALLY FREE😂
@@leonidtodorovski635 Hes right took 8 mg . 2 weeks later 6 .then .4. After .2.. really slowly reduced 1mg sometimes just every 2 day or 3rd. It works
I wish you were my doctor 2 years ago. The doctor I was seeing wouldn't refer me to someone closer so I was forced to quit due to lack of transportation. He wrote me about 7 days worth of blood pressure medicine and suboxone and that wad it. Did it completely alone. Worst and best experience of my life.
Often times what seems like a curse is really a blessing in disguise. I'm on day 9 today cold turkey and I'm doing much better than I expected after having tapered down to 2/3 of an 8mg strip per day or 5.3 mgs. Going back simply isn't an option for me anymore it's been 8 long years.
@@mikentile08192mg is nothing! When you decide to stop let me know! I can show you the private clinic you can be clean in 10 days, problems free. I was on 4-6-8 mg. I felt nothing!!! It’s Around 3000€ in total. If you live in Europe, if you’re in Usa 🇺🇸 ticket till my country will be expensive
I want to quit Suboxone but the clinics aren't allowed to have that discussion with you. Suboxone affects my appetite and i keep losing weight, way too much. I tried quitting but mistakenly tapered down too quick and 5 days later i went back to the clinic. Im prescribed 1 and half of the 8mg strips a day cutting one in half. Starting tomorrow im going to begin taking less but not too drastically over a few weeks. I have to succeed in doing this and i will.
Thanks Doc for trying to help us all,how I got to this point was a back injury about 10 years or so ago was on pain pills for a long time when it got to where i no longer needed them for back pain i got sick as a dog anddint know what was going on with me until i found out i was addicted to pain meds freind gave me a strip and i was fine for a couple of days so i went to see a doctor and thats been ten or eleven years ago now and its time to taper off this stuff now too.
Yes it's called dysautonomia. Sodium ascorbate (a form of vita C) mega doses every two hours. Imodium (which has a small amount of opioid in it...yes... Imodium) and turmeric. Hot Yoga and cold plunges.
I tried lowering the self medicated dose while taking suboxone and switching back n forth to lower the withdrawal symptoms then did cold turkey. Now 8 days sober
Awesome stuff wish I watched this before kinda, Last night was the worst but a shower 3 x /hr for 8 hours helped. Only thing I could do. A video on vitamins and things to help restore body or stimulate the dopemine to reset.
II'm not sure if you will see this, but I have just quit after 10 years, tapering down to 0.25. I am on day 5. I have heard several different things about brain healing or how long it takes the brain to heal from Suboxone. I am 33 years old.. Do you have any information?
Dear DocI just "found" your channel and I have never ever had such great explaining I'm watching your channel here in Germany and I have the feeling there are no doctors that have such an in-depth knowledge … please keep your great work explaining on RU-vid .....wish to find a doc like you seem to be… people be very very thankful !!!!!!! Send you my regards and a very very big thank you for the work you do!🙏🫶
I just quit fentanyl 12 days ago. I couldn't take any sub until 5 days of fent detox. I was so afraid of precipitated withdrawal. But now on day 12 I take 2mg sub and knocking it down to 1 mg tomorrow. Then 1/2 mg day after etc..
I don’t want to quit opiates. It seems like; never. I already asked my provider for HP Dilaudid shot twice a day. I think I would do well on it. Especially if I change my living conditions; staying with family because they don’t inspire me to make my way.👨🎓♾️🧑🎨
At 18:00 Does the injectible only come in 100mg injections or are there lower dosages that can be given? I know the films do not have uniformed distribution of medication but it seems most people do cut them. If someone is down to 0.5mg daily is it possible to use an injectable at that dosage?
I basically microdosed subs till I was about a week off the 30s. I think that really helped me the best, but my recovery isn’t your recovery. Everyone is different
I think that might be helpful for many people. I had paws for months afterwards and there was no end in sight. So, I decided to try Subs and it was a night and day difference
@@B_Bodziak yeah I don’t wanna say it would or wouldn’t helpz, but it definitely helped me. I started with a half a strip my first dose a little after 24 hours then the next day I took a forth. Next 24 hours I took an eighth and then I pretty much only took another eight if I absolutely had to! It really helped me to get through those tough times that I relapsed at previous times
I lasted 4 months of withdrawals when I came off methadone then stupidly gave up and went back on it. It was sending me nutty. I'm wondering is there much of a difference with the tapering and then the withdrawals between suboxone and methadone?
Suboxone withdrawal can normally last months. Methadone and things like oxycontin withdrawal is around 3 days to a week. If you're feeling withdrawal for 4 months from a drug that only takes a week(max) to be back to normal then you might wanna check out some other factors that could be causing it(if u got nothing to do all day yhe withdrawal will be noticeable but if you're busy it will be less noticeable). Motivation is key. Supplements like NAC , ormus minerals will help with withdrawal. Lots of healthy food, water and cannabis can help to. U must treat the mind tho/be ready or the process will take longer. Things always take longer when bored. I hope you're doing well 🙏.
I'm on 11 days without Suboxone and it's been quiet a feat!! Somebody suggested taking kratom to help with the effects of the withdrawal and it helps me sleep and I believe I'm on the road to victory
Kinda Hard to hear but (everybody knows) im just 8 month clean ..off heroin (then few weeks of subox. But shortly after 100% clean its everyday fight..how did you stayed clean so far?
Im clean since 8 month after 15 years of Heroin+Coke & crack use all what I can tell the best decision my entire life . But the Real fight begins after all withdrawals sympths first Over.. Its a fight every fck day
1/16th of a 2mg??? Holy crap that's beyond low!! I take 1mg or an 8th of an 8mg daily and thought that was safe enough to jump train in an emergency such as jail or something
I believe he said it could take a year to do the actual taper. From what I've seen, he doesn't provide a timeline for how long anyone should take subs for after beginning
Stay on as little subs as possible. Under 2mg is ideal if you can. Studies show everything above 2mg isn’t that effective on your brain. So trust me, stay low. Don’t do the 8+ mg’s unless you absolutely need to
@@Justin-gb6qh I don't go over the 8 mg. in 24 hours. Some days it is lower. It depends on my back and my right shoulder. I have titanium in my right should. It can be very painful on some days.
Can anyone tell me if this has happened before? I was using fentanyl heavily but suddenly i couldn’t get more than .15 a day for about 6 days so i waited 16 hours and took Suboxone which started precipitated withdrawal. So I got more fentanyl enough to stop the withdrawal but it’s been 64 hours since i used anything and i feel wonderful. Shouldn’t I be sick?
Imodium in incredibly high doses is said to give a very mild opiate effect. High as in 75 tablets at once. He said taking Imodium to help with the loose bowels of withdrawal
Quit taking Suboxone for five days. On the sixth day take Dilaudid. You can take the Dilaudid as a new replacement; or, a taper dose. Ask your doctor how.👨🎓
Yes, I've heard of people using a similar method where they take a short acting opioid and naloxone or vivitrol with it while the buprenorphine is leaving the receptors (takes 4 weeks). But, that seems risky for addicts.
That would be your best plan , let me tell you quickly what happened to me , I used pills and H for maybe 3 or 4 years , I went to prison and started doing subs , When I got out to prison I went looking for sub and found it , started using it daily and it would get me high , itchy , noddy , and energetic for work. 6 years later , I want to get off sub and I can’t. I haven’t tried to seriously stop but I literally tell my self tomorrow I’m not going to do my piece in the morning and soon as the morning comes , I do it.

So the doctor started me on 4mg twice a day. I think I’m going to start with 2mg twice a day and see what happens. If I can start low like that, I think it’ll help me taper faster. I know it’s a process and each person adapts differently, but at least I’m committed and taking that step. It’s definitely time to jump off the opioid train
1/8 of 1mg. So you would need to cut the film into 16 equal pieces (0.5mg/piece), then cut one of those pieces into 4 equal pieces...one of those pieces is .125 mg.
I was on Suboxone for 3 months, I stopped it when I relapsed 7 days ago. I left my sober house and have shot 3-4 grams of dope this week. My last shot was yesterday haven't used today. I wonder if I should get back on it. I don't feel great right now
Get back on it. I made mistakes too. I relapsed. I'm on 10 months of it now, relapsed a few times, but I'm not a drug addict and I can function in life.
@@just_peachy6582Post acute withdrawal syndrome PAWS is what she's talking about. Age, amount and length of use dictates the severity of acute and post acute symptoms. Dont be afraid of paws. You will face some similar symptoms insomnia etc but to a much lesser degree as compared to acute as time passes. Once through the acute phase you've done the hardest physical part. I've struggled with opiate/opioid addiction for 40 plus years. I also have a couple of diploma's and have worked in addiction treatment for over 20 years. This isn't any type of medical advice. @@just_peachy6582 So I used to be on 8mg. I’m down to 1/8th of a film. I tapered over a year or two. I highly recommend going slowly. At a pace that suits you. I’m lucky enough to have my husband helping me and holding on to my meds and giving me what’s needed ( kind of out of sight out of mind mentality). If you have someone you trust that can help you I would also recommend that. Unfortunately once you’re at such a low dose it’s time to start taking r every other day, then every two days and cut out until your at taking it 0 days a week. I’m currently stuck at the 1/8th and having a hard time going to every other day. But I’m so tempted to just stop cold Turkey and suck it up because I’m sick of it.
@@just_peachy6582I never recognized BAD symptoms by tapering just the occasional restlessness at night. I noticed the days I was busiest the easier it was for me to relax at night. Also comfort meds are a thing. Like Zofran for nausea, clonidine for WD, even MAYBE a muscle relaxer like cyclobenzaprine for restlessness
Subs come in different doses , I’ve been on subutex tablets for 6 years , and I’ve 👃 them , Would you say I’d have the same amount of sub in my system as a person who takes the medicine in their mouth ?
