Bad riding line on accident, maybe riding to fast? I am not an expert but I offroad on heavy bikes and have noticed pick your line is the most important part. Also the less I fight the handlebars the better I notice with heavy bikes if you just kinda point it in a direction and let it go it figures itself out pretty well. I don't do offroading with a passenger though that makes everything 10x harder.

Too fast. If you plan to do this type of riding frequently you should get some tires that are more offroad focused.
