How to Assess an Unconscious Neuro Patient Like a Neuro ICU Nurse

I'm Kati Kleber, MSN RN CCRN-K and I worked in a neurocritical care unit for 4.5 years. I was a charge nurse and preceptor. I recorded this video to give those of you floating to neuro or new to the area some practical tips because assessing an unconscious neurologically compromised patient is much different than assessing an awake neuro patient. This is me assessing my husband, who is clearly not unconscious. Please imagine there's a ventilator attached to him.

Update 5 years later! I just accepted a job with one of the worlds largest Neuroendovascular medical device companies. I'm reviewing and revisiting neuro vids and wanted to give my appreciation. AH YESSS! Congrats!! Neuro ICU is seriously the best. It will be a big learning curve, but it feels amazing once you're flying solo and feel comfortable in the role. So pumped for you!

This was so useful! The beer recs and the nursing info helps. As a nurse manager, What qualities and skills do you look for with new nurses in the neuro ICU? Do you have any stories of good or really bad candidates? I'm starting my externship there next month. Thanks!

Great question! So I actually sat on the peer interview committee and wasn't actually a manager. But I do have some stories... once, had a candidate continually bring up examples of when other people messed up to make herself look better. Another came after a clinical and looked like she couldn't care less. Another one provided really short answers that didn't provide any details/depth. GREAT candidates are authentic, typically a little nervous but I'm cool with that because they care, ask good questions, and provide honest - not cliche- answers! All the best to you!!!!

Thanks so much for the video!! I work in a very large, very busy level one trauma center ER, and I am a newer nurse. We get all kinds of patients, but I feel like I have neuro patients a lot. Unfortunately Neuro seems to be my biggest weakness. The assessment tips you shared were super helpful though! I'm hoping they well help me to better assess, and take care of those types of patients!

Do you think a new grad can handle neuro ICU? I have an interview in a few days. Its my absolute dream job but it does seem intimidating!

I'm sorry, I know this is 10 months late. New grads in neuro ICU is challenging, but doable. You really have to be on your game, looking up stuff at home, and taking extra time to learn stuff... because when you're at work worrying about learning how to do the task kind of things like starting IVs, using pumps, etc. it's hard to comprehend complex neuro anatomy. So you must be all in when you're there and looking at stuff at home.
