How to create Weekly Home Learning Plan (LWD) 

This is a sample Weekly Home Learning Plan for Learners with Disabilities under Transition Program. It includes all the activities to be done by my learners in a week but since their conditions vary, the plan may be adjusted to suit their individual needs. This was made out of my effort to provide a guide for sped teachers like me, however, I am open for ideas and suggestions for improvement.
