I struggled with this. a nice person from Age Concern came to help, and filled in the form for me! I found the application to renew my badge and bit daunting, being 80+ with arthritic hands, no easy task!
Hi Fred. It appears that online is the preferred method, however it may be worthwhile contacting your council to see if they accept paper applications. If you have a phone with a camera you can take a photo of documents and then upload to the online form , or better still with an iPad or tablet as better definition.
Hello Vali, thanks for your question. The form should be filled in in the name of the applicant that has the disability. It doesn't matter if your mother doesn't drive. As it can be used with a vehicle that is primarily used to assist her. All these best with the application! :)
