If you're applying to get your U.S. passport book or card for the first (or a subsequent) time, then you'll need to fill out the form DS-11, Application for a U.S. Passport. First time renewing, do I need to go somewhere in person to send this like an official office? Or can I just put everything in the envelope and send it off at USPS? -also, do you have a checklist of all the things that needs to be in the envelope? Thanks!

I ask because my birthname, it is entirety with spaces, won't fit in just one of the 17 character spaces. Or would I be best to attach a paper with my birth name on it?

I applied to be a flight attendant but I need to get my passport "renewed" or a new one because it's passed 15 years since the date of issuance. Can I get my passport expedited under my circumstance? Hi, I’m in a pretty weird situation right now. I’m under 18 so I apply for passport along with my mom back in December last year. However, mine passport was lost and only my mom’s passport was sent home. I called the NPIC and they told me to fill out DS-86 in April. A month later my passport card came home. It’s August now but I haven’t received my passport book yet nor any of my supporting documents😭What should I do now? Also in the worst case, can I use the passport card to apply for the book even though I never received a newly book before?

Filling out for my husband who is currently married to me but previously divorced once. In # 11, (I put my information down as his most recent spouse), but when it ask the question right after about ever having been divorced, its asking in general correct, if he has ever been divorced (not specifically asking if divorced from me correct)? So I would still put his divorce date from previous marriage on there even if the other spouse information is mine?

Do you know what will happen if I left one question blank? (Question #14) In my case, when I filled out the form online, question 14 (place of employment) was not required. However, when I arrived at the facility center, I had to clarify one of the questions and ended up manually filling out the form. I simply copied the information (except the one I needed to correct) from the printed form, and question 14 was still not required in this case. Now, I'm concerned if this will be an issue or not.

can a person who is a resident of Washington get a passport while they are out-of-state say if they are in Texas for an extended amount of time. Basicallysince my wife didn't license is good until 2030 and a Texas driver's license has to be renewed every couple years I ain't giving up my Washington lisence. You can attach an additional sheet of paper that indicates you’re unaware of the specifics

You can apply for their death or birth certificates and there you will find the information.

Yes you should indicate it where necessary. Did you ever report it lost or stolen, though? You would have made the report using DS-64…

@NickDemski Hi Mr. Nicholas my name is Emmanuel and my children have a CRBA appointment in my country Ghana and I was wondering if you could answer some questions for me, I was wondering the DS 5507 the section were it says part 2 and 3 and that it should be left blank and will need to be filled out in front of a consular, since I’m not in Ghana during the interview, so I leave it and they will have my wife sign it or do I have to get signed by a consular at an embassy and what if I don’t have an embassy near me to do that part of the process.

If you’ve lost your passport, you have to report it lost first. That is another application, DS-64, that you should file first. Once that is done, you’ll have to get a new passport with DS-11 and file it in-person. You can’t ‘renew’ a lost passport unfortunately.

For place of birth I spelled out the whole city and spelled out the whole state, should that be an issue? And for the mailing address line 1. I simply put down “W” and “Dr” instead of doing west and drive, should that be an issue also?😭

So, just to make sure, we do NOT fill anything in for D O S NFR, End. #, or Exp. in the upper righthand corner of the first page of our DS-11 form, correct? As long as your Primary Contact Number (item #7) is listed, an additional number isn't necessary

1) Why did you use all caps on the first page and lower case on the second page? 2) If my mailing address is a suite....and I also need to do a In care of..." do I put both on that line? Also, those are not backslashes...they are just slashes. Backslashes lean the other way.

If I took my own passport photos and had them printed, should I take those with me for the section, “Stop Continue to Page Two” or should I staple/glue it on beforehand?

Don’t glue it. It should be stapled in a specific way. But you could wait to turn it in and ask the passport officer about how to do that when you turn it in to them. I also have a video about passport photos that might answer any more questions
