Would this quilt be ok with a border? If so any recommendations?

Yes, You could add a border. I'd recommend the dark gray print with the stars, moons and witch silhouette or the charocal print with the pumpkins & witches

I was very particular with my cutting of fabric, but my blocks are not 12.5”. I’m assuming my .25” seam was off. Should I just square up each rows block to be the same size?

I think your seam allowance was larger than 1/4", so Yes, that's what I would do - the blocks that are not 12 1/2" may need to be trimmed as well if you do that - just go a little bit at a time with the trimming so you don't trim too much off.
