I have mixed incontinence and had pelvic floor surgery, which helped a lot. Unfortunately I also struggle with constipation, which my doctor told me after surgery it was imperative to avoid. I did well for an extended period of time until I got ill and suffered an extremely bad episode of constipation. Since then, incontinence has gotten progressively worse again. I use pads daily, but have never been offered or discussed medication for the urge incontinence, which is the most bothersome. My concern would be if constipation is a side effect, it could end up causing more damage, couldn't it? I don't know if a second surgery is even an option.

Could you do another video on this topic for women who've had hysterectomies? There are various scenarios with this as well. Also, a historical perspective about the difference between vaginal hysterectomy vs. Laparoscopic hysterectomy, oophorectomy, etc., and how this impacts a woman's ability to void or not. We need this knowledge! Ty.

The change in anatomy from a hysterectomy is important to cover especially in regard to incontinence and leaking . They should be covering this in this video . Surprised they missed this .

I had incontinence issues for a long time am 61 yrs old. What I find to eliminate it completely , was doing squats really it eliminated my Incontinence problem just simple squats . I'm a naturalist and I always find natural approach to do everything all you have to do is spread your legs and do 50 squats only nothing else, that's what eliminated my Incontinence and strengthened my pelvis muscle and I don't have to take pills, I don't need to do any surgeries . Try it ladies you have nothing to lose. P.S. keegel exercise makes it worse so don't do them !!! 😮

Squats require you to bend your knees , I can’t do that anymore, replacement on right and left is bone on bone, I take a pill that don’t work very well. Dr.suggested Botox shots. Leary that it will freeze the bladder and I won’t be able to go

What works for you don't work for everyone. So telling someone kegel doesn't work abd you are not a doctor is bad advice. Do what works for you

At the age of 50, I was ‘lucky’ to be diagnosed with bladder cancer. Lucky because I had no symptoms, nor did I fit any of the demographics- male, older, smoker, overweight . While having an ultrasound to screen for ovarian abnormalities, my tech noticed a growth attached to the top of my bladder and recommended I see a urologist. Fortunately the growth, which was cancerous, was small, had not spread too far into the bladder wall and was easily removed surgically. I’m lucky because by the time you notice the symptoms of a bladder tumor it can be much more difficult and invasive to treat. Nearly 20 years have passed cancer free and it’s all a distant memory. Except one thing. My urologist told me to Stop holding my pee for so long. Whether I felt like I needed to or not, he suggested that I set a timer and urinate every two hours, gradually increasing to three hour intervals. Among many other pieces of advice, all of which you covered here, this is the one that had the most impact. Thank you for opening a conversation about incontinence. Sorry for rambling on for so long!

Thank you for the timer idea. After a groin catheter to insert a Stent in my heart, I has a large blood cloth in the vein where the catheter was inserted. I had much better bladder control for about 5 months until the blood clod finally dissolved with the aid of arnica oil applied several times a day. 1.5+ years later, my bladder control is worse! Has your bladder control improved since you cancer surgery during the past 20 years?

I underwent the surgery. it helped immensely my urgent incontinence, but not the stress incontinence. My Urologist also gave me a patch to put on your stomach, to help me relax the bladder. It really helps with long car and plane trips. Thank you for outstanding & informative presentation.

Great video thank you. I had a brain tumour and my incontinence is really bad. My brain does not send a signal that I need to wee until a minute or so before I need to go. I wet myself almost every time. I'm fortunate to live in England and my pads are provided free via the NHS. It's made such a difference, I feel I can go out safely now without wetting my friends couch or the chair at church and so on. Thank you for the help you're giving to so many. Blessings

Im glad you talked about overactive bladder some as that's what I have. Eliminating soda and certain foods has helped. I had to cut back on the amount of water i was drinking and stop at 500pm. I was stopping eating at 700pm but I have found that I have to stop at 600pm. I have also found that processed foods irritate my bladder. The urogynecologist that I went to says my OAB is due to menopause. The doctor has had me use a tens unit placing the electrodes on my ankle. I think it has helped. It's all been trial and error and a number of not so good night's sleep over the past year. I still hate and its frustrating at times.

Have you tried Gemtesa yet? I was on it for a couple of years until my insurance decided it no longer covered it. The only med that didn't make me swell or screw with my blood pressure.

@@rainbowzebraunicornpegasus2962 I'm on Gemtesa; been on it for 2+ years. I've had limited success, and only with frequency, NOT urgency or ability to stop flow. It's better than Oxybutynin (anticholinergic), which I was on for 2+ years; that gave me horrible dry mouth and eyes, and led to dental problems. Myrbetriq (2nd drug of choice) is contra-indicated due to hypertension treated with beta blocker and an ARB. Had to do Gemtesa; insurance be damned. Special authorization was needed; retail value of 30 day of Gemtesa is $500. Insurance pays 100%. I have a great Uro-gynecologist; presence and position of a small rectocele means P.T exercises don't work. Uro gyn. is now awaiting general & cardio clearance from my primary care to schedule installation of nerve stimulation implant. Also tried 10 weekly in-office sessions with stimulation of pelvic nerve via electrode at ankle; negligible results before a minor brain bleed caused by a bad bonk on the head forced the end of this therapy. It took 9+ months and severe Rx adjustments to get Neurology ok after the brain bleed. I'm hoping the implanted device, with a constant stimulation pattern, will finally fix this. Good luck to you!

You left a BIGGIE OUT… Sacral Neuromodulation. I have an Interstim device for fecal urgency but most people get the identical thing for bladder incontinence. It’s a pacemaker that sits near the sacral nerve. The sacral nerve controls bladder and bowel function. Some people’s sacral nerve misfires, and that is what the device is for. Mine was surgically implanted by a Urogynecologyst, just like the doctor in this video. This has been the most wonderful thing for me. It’s been life changing and there are thousands of women who have gotten help from this technology and able to have a normal life again. It helps women who have been “messed up” during a difficult vaginal birth as well as other reasons. I am so sorry they did not discuss this minimally invasive procedure as an option.

Had surgery 2 weeks ago to implant Medtronic. What a relief after 40 years of searching for relief from continuing bladder leakage. Am so enraged that my primary doctor didn’t recommend this. Years of using drying pills that left me dry eyed and mouth dry, but little relief from my bladder problem.

I just started a somewhat similar, but less invasive therapy. Once a week I go and they do nerve sacral nerve stimulation through my ankle. I was waiting for her to discuss those options as well.🙁

​@relsba Your primary md isn't a specialist. "Enraged"? That's a pretty angry & off the wall stance to take. Your primary has enough to worry about. It's impossible for the primary to know every alternative out there. You should have gone to a urologist for an examination & options. This surgery is a newer procedure so stop being so judgemental. I have had incontinence for years, too, & have used every prescription available. My urologist recommended this surgery about a year ago. I asked my neurologist his opinion on it & he was not crazy about the idea at all. Now that I have seen a few responses here about it, I may look into it again. But, back to my point.....stop being Enraged & just be thankful you finally got relief. Don't be a Karen.

A very informative video Docs! This is like the videos you've previously done for the male viewers who might be too embarrassed or insecure about talking to anyone about their issue. The sad part is when women are dismissed by their male doctors.🧐 Don't get me wrong, I've met some very good, patient and compassionate male gynecologists who are great at explaining things to patients without being condescending or dismissive. But reading through the comments, many women have faced this very problem. We're very fortunate to have doctors like you who want to educate patients and point them in the right direction! If only we all had doctors like you!👍 You spend a lot of time and effort making these videos for us! We can tell you love what you do! And we love it too! The doctor didn’t mention a neurotransmitter or Botox in the bladder. I have been having Botox injections in my bladder for four years every six months. From the first set of injections I have not worn a pad once. My incontinence started in the 3rd grade and I am now 78 and couldn’t be happier with this decision.

Years ago my dr tried all the meds, but I didn't do well with them, and they didn't help. I had a historectomy and at that time they did surgery to support the bladder, and the rectum. It helped some, as I had had a couple babies, and the 2nd one was quite large. She is now 61. Now many years later, I'm plagued with the urge incontinence, and fecal incontinence is becoming a problem. I am a normal weight, but am old, and work on strength, balance, and yes, I do the keagels when I think of them. This was a very informative video. I learned a lot.

I have had Botox injections which helped for 3 years. I have an appointment in March to get it done again. During the process, the doctor let me watch it on the screen. I found it very interesting.

@@DeeDee4always Baking soda in water-hold your nose, and unsweetened cranberry help. The acidity causes pain. I found baking soda helps right away.

Pelvic floor physical therapy is an amazing experience. I have found that the specialists in that field are some of the best physical therapists because they listen in a way other therapists do not.

Occupational Therapist also teach pelvic floor exercises. I was trained to teach, "Beyond Kegels." Works great!

Great presentation, lovely Doctor in every way, I really wish to say that I wish she had warned that anticholinergic drugs have been proven in several reliable studies to cause dementia. I will not discuss my personal history, but let’s just say, I’m so pleased I found this out 3 months ago. No more of these meds for me. Love ❤️ you Docs!

I had several falls in the last few years, which pinched some critical nerves. I can no longer sleep thru the nite, due to "potty breaks". Incontenence/piddle pads are now part of my wardrobe.

Like others here, I was interested to see this topic come up for female incontinence and especially with Dr. Best (my Dr.) I'm using a pessary due to a bladder prolapse but experiencing leakage/dribbling. Switching to a pessary designed to help curb the leaking. Looking forward to Wednesday aka Pessary Day which makes me laugh and tinkle. I have found Dr. Best works with you when something doesn't work out, no shaming or being annoyed. She's very kind and wants to make you comfortable. I would like to see a video on surgical options.

I have never had children but noticed stress incontinence since I was an older teenager. At first it happened occasionally with jumping. Not long after I started leaking during orgasm. Now it happens when I sneeze, cough, lift heavy things. I didn’t really drink a lot of caffeine until my late 20s and I rarely drink alcohol. When I turned 24 I started to have trouble emptying my bladder when I wanted to. I was able to get the stream started, but had to bear down to empty my bladder. I went to several doctors and when I explained my symptoms they all looked at me like I had three heads. I also get UTIs every time I have sex regardless of following all the rules you are supposed to follow (wiping front to back after using the toilet, voiding immediately after sex, not taking baths, wearing cotton underwear, etc.) and every time I went to the doctor for a UTI they gave me this same list of “don’ts” and acted like if I was getting UTIs I obviously wasn’t maintaining proper hygiene, which I found not only frustrating but very insulting. They did an ultrasound and they did find that I was not emptying my bladder fully. They prescribed Flomax for that which as far as I could tell did nothing. It didn’t seem like medical professionals had any answers for me and I pretty much gave up. I’m 41 now. I got so frustrated with all of this that I pretty much don’t even go to the doctor anymore because it seems like all they do is gaslight me and accuse me of being gross or stupid or both. I wish there were more doctors like you guys.

Try a detachable shower head to clean up after sex with soap and water. Wiping front to back does nothing for after sex UTIs. I am not sure baths are harmful unless the water is contaminated.

I was incontinent for many months wearing nappies. Finally after a visit to the doctor and a course of strong antibiotics then a course of medium strength antibiotics my inconvenience cleared up. I am now back to wearing nickers again. Yeay!!!!! Incontinence no more.

I had the bladder mesh done in 2008, wasn’t successful. 😢. I’m now 70 years young. My urologist explained that I have mixed control issues. I drink water, go to bathroom at least 3 times just from 1 glass of water. Can be exhausting. Don’t drink anything an hour or so before going to bed,up at least twice per night going pee. I participated in a study, tried meds, but they were short lived. I give up. Also was explained to me with having Ulcerative Colitis, can sometimes create pain in trying to empty out. I’ve found most GPs don’t understand nor take time to explore symptoms or send to a specialist.

@@TalkingWithDocs yes this has, thank you. I think in these years since I had the mesh done, a lot more information has been brought forward from various studies and the experience of Urologists. I’m going to see about pelvic floor physiotherapy to see if I can help improve things myself with proper training. Thank that nice Dr for spending time explaining things. Please have her back to explain even more and tell Paul to quit getting so excited that he pees his pants! 🤣🤣🤣. Can you also ask her about frequent UTIs? I go through stages where I get many infections and some are harder to clear up, especially the K-pneumonia bacteria. That one does me in.

I've had UTIs from your friend Klebsiella (2 different species in that genus), as well as E. coli. Several times. Can't tolerate the Dr's. favorite fluoroquinolone antibiotics which cause crippling and severe tendinitis in my lower legs, and I'm allergic to penicillin, so I've had to take extended runs of sulfa drugs, mostly a horse-sized monster called Bactrim. Also Macrobid. So I know about waiting. I was prescribed Methenamine in hopes of acidifying the lining of my bladder so no bad guy bacteria want to take up residence there, but the very first dose made my urine smell like formaldehyde and I had a purely coincidental brain bleed after the second dose, so no more of that vile substance. I also know about C. diff; contracted TWICE because of too many antibiotics. I'd say I empathize with you. My plumbing - both systems - is a sad joke.

EMDR can help with physical stimuli with urgency incontinence. I couldn't go to the basement to check laundry or hear the garage door go down without needing to go urgently. But if I got distracted, wow, no problem. EMDR definitely helps. This is done by psychologists and therapists trained in it.

Great explanations. I have the stress problem. After twins and another birth I had no real problems but after a hysterectomy at 50 the stress incontinence has been issue. I noticed it with every kind of coughs/sneeze and exercise. I appreciate all the expert advice! I am going to buy a pessary and do kegle exercises correctly

The doctor I went to gave me two choices : botox shots or a battery in my behind with a set up to work like a pacemaker and was pushing for the battery. None of these other methods were mentioned. Thank you for this information.

Thank you for this great video about a topic that is so common. I was wondering if you could do a talk about Lichen Sclerosis with Dr. Best. It is a condition that is more common than you think. Even as a nurse myself, I had never heard of it until I was diagnosed, as well as a friend who is also a nurse. By joining a support group on Facebook, I see how common it is and how underdiagnosed it is and how few doctors know much about it. It is another topic that we are embarrassed to talk about. Love all your videos. So informative!

Thank you for your information .You are right when you state to empty your bladder every 2 to three hrs,I prefer every 2hrs it works better.Also drinking 4 8oZ of water is sufficient, if I drink more than that ,then I am going more frequently. I believe we are overloading our bladder with too much water and maybe damaging the bladder with excessive water,but that can be answered by a nephrologist.

I have mixed incontinence with the worst being stress incontinence. I am a runner and cannot get through a run without at least a partial bladder release and sometimes the entire bladder. I was given both types of drugs you mentioned for the overactive bladder. The first relaxed my esophagus so much I couldn't swallow. The Mirabegron was OK, but not a huge improvement and since I have IBS, there were issues there. I was advised that my pelvic floor and vagina in general was very week and that, at the moment a pessary might cause me pain. I did go to 6 months of pelvic floor therapy, with only a slight improvement. I think they are recommending surgery, but since I have had 7 abdominal surgeries over the years, I am not really leaning that way. I will probably push to try a pessary, for at least my runs. I do leak many other times a day too just standing around, but nothing like when I run. Anyway, thank you for doing this segment and btw, I am one of those women who overshare with her other post-menopausal friends about what is going on with my body. 😆

Look up Uresta device. It works for me as I walk everyday for at least an hour. Was releasing urine in my walks. Now with Uresta device that has stopped and I have piece of mind.

I went to a Urologist for urgency and incontinence. I was diagnosed with IC (interstitial cystitis). I had never heard of it until then.

I have a mixed bag. I not only have the multi intermittent incontinence, sometimes my uterus puts so much pressure on my urethra I can't pee at all. I have developed some unorthodox solutions that work,... ultimately I will no doubt have to have surgery, until then, I'll keep moving on.

I have the same problem! Urgency incontinence AND I can't pee, a lot of the time. I was diagnosed with a destended bladder while in hospital, but they did nothing about it and said nothing. I though it might be a POP, bladder, or uterus. Can you give me an idea about how you are treating this not being able to pee? Thank you, I hope you can get some permanent relief.

I've been through all of the steps mentioned today. Evaluation and testing, pelvic floor exercises, medications, (Vesicare), PTNS, treatment, Interstim placement and now considering Myrbetriq but out of pocket, even with exceptional coverage, it's $436 per month. What's frustrating is that I still have continance issues! I guess its now a lifestyle. It is what it is so I'll just, um...., go with the flow.😂

Pelvic Floor Physical Therapy worked for me. There are Awesome practitioners in North Idaho. I have a pessary for vaginsl prolapse. It’s very comfortable. Before I had the pessary, I had terrible urgency incontinence.

If you don't mind me asking, what are some of the symptoms of this? I have pain in the bladder, frequent UTI, as well as an inability to fully empty my bladder.. along with the the urgency incontinence. The pain is worse when I can't urinate fully. I was dx with a distended bladder while in hospital, but nothing was done or said about it. I'm thinking cystocele or rectocele, some kind of POP. Thank you.

​@@daffodilunderhill7066I have had a hard time with the diet. I have to keep a food log and keep track of foods that trigger flare ups. So far it's been coffee, chocolate 😞, sparkling water, tomatoes... it's a work in process. Plus I always have to be near a restroom since I have to "go" frequently. So you sort of create your own diet by eliminating your trigger foods. This is what my urologist is recommending so I m trying it. Sorry you're going through this. It's not fun at all.

​@@LDiamondz my pain is similar to yours. It feels like a bladder infection, only it's not one. It's a new dx for me, so my urologist has me on IC diet. Basically I am eliminating any foods that trigger symptoms. I hope you feel better and find healing 🙏🏼.

Thank you for your informative videos. I was given a pamphlet re Kegels, but couldn’t discern which muscles to flex because there is no feedback. So I started holding my urine midstream for 10 seconds, at least twice, depending on the quantity of the flow. I regularly held my flow. That exercise gave me muscle flexing feedback. If I have an urgency, I will flex the muscles and hold until the urgency dissipates then go to the bathroom.

I've had Coaptite injections 3 times. It works wonderfully. Then slowly, it wears off. For me full benefit for approximately 8-12 weeks then over about weeks it disappears.

My wife has Urgency Incontinence. She suddenly as an urge to go RIGHT NOW and sometimes doesn't make it. She's been prescribed Oxybutynin 5mg (x2 daily). She's been told she needs to see a specialist and have an internal exam but she won't do it (she hates internal exams). Her lifestyle/mental health is a factor here. I wish I could convince her to see a specialist.

There are gyn docs trained to treat patients with trauma and fear based issues. A lot of women have this and it's not insurmountable.

What I really didn't appreciate was the giggling, teenage boy humor from the two men. I suppose it must seem very humorous to men who don't experience childbirth or cope with the lifelong aftereffects.

One thing I did not hear you mention, and that is Botox injections in the bladder, and outpatient procedure which totally stops bladder spasms and the sense of urgency though the bladder is empty. It totally works. Only downside it must be repeated every several months

Thank you for addressing this issue which remains largely untalked about. I had no idea there were pessaries available to try. Due to age, pregnancy and overweight I suffer from this when I have a cough and cold or certain exercise - it has greatly impacted my sexual relationship with my husband too. After seeing this video I am going to go and see someone about sorting this out on a permanent basis. I agree that advertising leads to acceptance that this is “normal”.

I wish they'd talked more about the effect of hormones on incontinence issues. I'm 33, have had amenorrhea for years due to an eating disorder, also have MS. I guess i mostly get urgency incontinence, saw a urologist about it a couple years ago who chalked it up to MS and put me on mirabegron and later Gemtesa, neither had a noticeable effect for me. The next option was Botox injections into the bladder, i can't stand even the thought of that so decided not to do it. Physiotherapy was never discussed. Interestingly, i noticed that at times when i restored weight (even though i still had amenorrhea), the urinary incontinence issues improved A LOT, even if not completely gone, so it makes me think that hormones are involved as well. No idea where to find a doctor who could even consider the interplay of these issues though, so I've kinda given up on it.

My doctor put me on meds that helped some, then she mentioned Botox of the bladder and also TENS. I went with the TENS and though I am not cured, I am way better. The TENS unit cost $35 on Amazon and I just bought 20 extra patches for under $10. I would highly recommend the TENS. I didn’t like that the Botox is a pelvic treatment and only last 4-6 months.

I did Kegel exercises during my pregnancies and after the births of my children during my mid-30s. Great. Now, in my mid-70s, it doesn't matter. I live with overnight ultimate urinary pads and adult diapers. Yippee. (Sarcasm here.)

My sister was a smoker and she got bladder cancer- the doc told her that the chemicals in the cigarettes sit in the bladder and causes the cancer, he also said almost all bladder cancer patients smoked but not all smokers get bladder cancer. So yeah check that out if you have issues.

Thanks for an informative video! One thing I didn't hear covered is catheters. My wife has been cathed many times...at least 18 times during various surgeries/hospital stays. She feels the leakage and having to go about every 1 1/2 hrs is a result of the many catheterizations. Are there any treatments to 'fix' this condition?

Same for me after one surgery related catheter. I did research and there is a 50% bladder injury rate with catheter insertion.

Please educate your nurses and care assistants...a lot of them tell patients that uti's are related to sexual activity. Which causes embarrassment to pts.

Good hygiene just prior to sex should be done. Also, females should urinate just prior to sex (and then clean self) and right after sex.

I developed stress Incontinance after the birth of my first child because I had extreme tearing of both the muscle and skin during vaginal birth. Then fell pregnant with my second child just a few months later.. I tried the pelvic floor muscle exercises my midwife recommended but it couldn't fix this issue. As I understand there is not much you can do to strengthen a damaged muscle like that.

Back to the diaper eventually…. Let’s be honest! I use a Pessary, I’m 67, and had 7 full term births. Doctor suggested bladder lift & uterus removed. I said nope! It all sucks ladies so pick your route. Lol I’ll tell more….. I always use thin pads, Amazon sells incontinence underwear, and a couple sheets of TP. Lol, cough/ sneeze oh lord! Golden years? GO in golden? Yup!

I had a bladder suspension surgery back in the 90s and have had bladder spasms ever since. I have good pelvic floor muscles according to my dr, I just have these spasms all day everyday. No idea how to resolve this issue.

I’m 56 and have had stress incontinence for at least 6 years. I have had to stop exercising due to leaking but I can still walk on the treadmill. I have thought about an ostomy so I can run and exercise again. Stress incontinence is truly debilitating.

One thing you didn’t talk about is the effect of radiation. I never had an issue until after going through radiation for cervical cancer. It caused both urinary and fecal incontinence. The fecal has improved but not the urinary. The problem is I no longer have any sensation until it’s almost too late. I’ll take on board the dietary suggestions, they make total sense and should help but that’s another cause.

Also, chemo which can cause changes to the uterus- thickening and lifting. Seems that might also affect the urinary tract.

My bladder shut down. It leaked but didn’t completely empty when I went. I was getting lots of infections so now I need to catheterize every 4 hours. But I also still leaked so I had to wear a diaper. They had me on Botox injections for years to help the leaking but still needed to catheterize. The Botox stopped working so they put in a pacemaker in the left cheek of my bum and the lead goes into the uterine wall. It’s helping some. But I also have suffered with constipation my whole life. Then a Dr cut both my sphincters in an operation to make my colon bigger to help me go. After being diagnosed with celiac disease after many years of suffering with constipation. (Yes! That’s right! Not diarrhea! It can go either way.)I was still the same way. I went once a week and not much. Since having the pacemaker put in I go every day sometimes 2 or 3 times. My question to the Drs is where was all this Pooh before? And where is it all coming from? My eating hasn’t changed and I don’t eat that much. I’m in a wheelchair now. I’ve been diagnosed with 4 lung diseases(asthma, COPD, chronic bronchitis & Interstitial lung disease-never smoked) , a heart condition called Angina, eroding bones in my feet(thus the wheelchair) & spine, DDD, Hashimoto disease, fibromyalgia, arthritis, osteoporosis, psoriasis and the newest one just last month Epilepsy. They are the kind where I just stare off into space and don’t respond. They’ve been going on for about 10 years without medication so now I have brain damage. My short term memory is going. That’s why I saw the neurologist. He said I may not get that back but now that I’m on anti-seizure medication it won’t get worse. None of my specialist know why this is happening. Oh I was born with one kidney that works. It works backwards. When I sleep it works double time so at night I take DDAVP. Have any thought? Would appreciate any help to stop what’s happening. Thank you. 💝🥰
