For health insurance premiums, do I put the annual total? Do I put the total before the premium tax credit or after? Because it's either $4000 or $600, and that's a major difference.

These should all be annual totals. According to the EITC Assistant, you should: "Enter any premiums you paid pre-tax for health insurance that you didn't include in the HSA, FSA, or other “cafeteria plan” section."

I dont understand one thing when do you we need to pay the refund back?

If claiming the earned income tax credit results in a tax refund, then you would not have to pay the refund back. Is there a particular payment that you're concerned with?

Greetings sir,quick question please. I had file back the questionnaire in the form of paper work so when I received my refund will i get it by mail or the return will go directly to the bank,because I file tax with a prepared by electronic.

I don't understand much of your question. But if you're trying to determine whether you'll get a paper check or an electronic deposit for your tax refund, then I would believe that you'd get a check unless you gave the IRS enough information to make an electronic deposit to a valid banking account.
