I know you said that there’s no drug you can take to safely help you lose weight, but if you have undiagnosed adhd, is it worth exploring adhd medication and would this help maintain focus towards a healthy diet and stick with a good exercise routine?

RUBBISH. YOU HAVE NO IDEA WHAT YOU ARE TALKING ABOUT. This is why general GPs should not just be talking about things they do not know about. Please listen to endocrinologists, obesity doctors, SPECIALISTS. GPs/generalists speak from a general basis. Obviously better than the lay man, but still not the best on topics they know very little about!!!

*Incorrect, Ozempic does NOT stop you from eating.* I've been on Ozempic for a while now and I still get very hungry. I never experienced those side effects, except for nausea and that's only if you eat too much, so yes you can still over eat while on Ozempic. Ozempic is a Miracle Life Saving Drug for people who cannot control their Diabetes. I only pay $7.70 for it here in Australia. You exaggerate the side effects. Minor side effects only effect 5% to 10% of those using it. Muscle loss happens when you lose weight and don't exercise, so ALL Diets lead to muscle loss. I am Diabetic and Ozempic is my last chance to control my Blood Sugar levels or die from Diabetes complications, and yes I have tried everything else with no success.

My hubby's doc prescribed wygoby for him. He's not very over weight, about 20lbs, He said the doc told him "I've prescribed this to many of my patients" At $1,600.00 a shot I told my husband three things: 1. Eat better and exercise. 2. Your doctor is getting kickbacks and 3. Get a new doctor. Hubby cut out sugar, processed foods and dropped 13 pounds already.

Yep. My mother had a really high A1C after having a stroke. I had to care for her, and changed her diet. The doctor had her on Metformin which gave her horrible diarrhea and yet never brought her blood sugar down. After I changed her diet, her A1C dived to a normal reading. When she was in a nursing home to rehabilitate from a stroke, she was listed as a diabetic diet plan. The food was identical to everyone else. She received a big glass of Coke and a dessert with her meals.

Dr. Dhand, I am a pharmacist and am so thankful for Drs like you. We see things very much the same way. What ever happened to conversation and debate over differences of opinion in the medical field (and elsewhere). I have seen many BAD aside effects from Ozempic, Saxenda and Wegovy, such as ileus, pancreatitis and muscle wasting that didn’t get better after stopping the drug. Everyone wants a fast fix. I don’t agree with how it’s being pushed…and believe me I tell patients.

In a weird way my new diagnosis with my second autoimmune disorder was a blessing in disguise. I was wanting to go on Ozempic but my liver count was way high. So I started to eat less and reduce my carbs. I am now eating whole foods, lots of protein, no processed foods and hardly any carbs. I have dropped 40 pounds and my liver enzymes dropped like a rock. My autoimmune disorders effects are vastly reduced and I haven’t felt this good in decades.

I took care of my mother after she had a stroke. She was on 12 different meds daily. Just about every side effect listed on the med happened. Most of the pills did nothing for her. My advice to everyone is ASSUME all the side effects will happen. People think they are just "rare events". They aren't. When they say "if you can't breath, call a doctor" for a drug that gets rid of a rash....don't take that medication.

Our medical system is completely broken seemingly since covid. I will never have trust for the medical "professionals" in the US ever again. I have heard insiders confirm that the medical industry is now based solely upon $$$ & nothing to do with patient health & care. The insurance companies make the decisions on what medications patients get, not their doctors. & doctors only have ~10 minutes per patient, which is not always enough time to discuss all of the patients concerns. It is a scam unless you are in the ER receiving life saving treatment.

Ephedrine was more recent. I got a personal trainer fired when he sold me a supplement he said would help burn fat. I told him that it couldn't have ephedrine in it because my system is very sensitive to uppers of any kind. They hid the ingradient under a different, natural sounding name, so I didn't know that the product, indeed, contained ephedrine. At 2:00 AM. the next morning, I was up having some kind of anxiety attack with thoughts I was dying and my heart racing. I called 911 and the EMS, who took my heart rate, and BP said it was like I had drank 4 pots of coffee, and it hit me at once. They pointed out ma huang as the source of ephederine. I went back to that gym, and the trainer was fired instantly. That incident ended up making me want to be a trainer so I could safely train people and give them good advice. I got my National Academy of Sports Medicine certification in 2003 and never stopped telling people that it's what you feed yourself that is 80% of your problem.

I took it for diabetes for two years. It was great for my A1c and I lost 40 pounds. Of course, I was usually mildly nauseous and bowel movements were so infrequent I saw my gastroenterologist. He gave me fiber, which made it worse. Last January it became unavailable. Since then I have been on a healthy low carb diet with lots of exercise and my A1c is actually lower than it was while on the drug. However, I gained 30 pounds in 2 months, even with the healthy lifestyle. Really messed up my metabolism! And I lost muscle mass. Six months after Ozempic, my body is slowly returning to normal. Lost five pounds last week for the first time since quitting the drug. At my last endocrinologist appointment, the doctor was encouraging me to try Mounjaro. NO WAY! I don't need it. I strongly recommend against taking these drugs. My biggest regret is that I didn't get serious about my diet 20 years ago.

I've been doing intermittent fasting consistently for almost a month now, along with no having added sugar. In that time, I've lost 7 1/2 pounds! I do not feel hungry when I'm fasting, and i comtrol my portions during my "eatinf window! I think that my results so far are pretty darn good for having made two very minor lifestyle changes! I've always exercised, so the combination of all 3 actions has been working quite well for me. I would not want to lose much more than that in one month's time. Fasting 72-96h is the fastest way to both lose fat without losing muscle and also to reverse t2 diabetes quickly. The fat you lose from fasting always comes from the liver first because the liver uses it to produce blood sugar and ketones for the body! The many benefits of occasional extended fasting and lowering dietary carbs: Your body recycles up to 1/3 of all immune bodies in a 72h fast, rejuvenating your entire immune system. This helps with autoimmune disease, cancers and cytokine storm. Fasting stimulates phagocytosis, the ingestion growths pathogens and plaques by macrophages and other immune cells. This will remove spikes quicker, whether natural or unnatural in origin! Clots and plaques are removed over time due to accelerated phagocytosis. Fasting reduces cortisol over time and improves your circadian rhythm. Fibrosis aka scarring is reversed over time, including in the heart, lungs and clots. When insulin is high, vit D stays locked in the blood cells but fasting quickly lowers it. Vitamin D plasma levels are increased as fasting improves metabolic health, and vitamin D in turn increases autophagy. Fasting restores NAD+ and increases nitric oxide release to open blood vessels. Reflexes and short term memory are increased. Fasts from 36-96 h increase metabolic rate due to norepinephrine release!. Blood sugar and insulin are lowered when fasting, reducing inflammation and allowing the immune bodies to move freely through the body. T cells and T reg cells are vital in fighting cancer, autoimmune disease and infections. With age, the thymus stops making as many of them but fasting releases stem cells, which then can become new T cells. It also releases growth hormone, which regenerates the thymus itself! Fasting increases anti-aging Yamanaka factors and increases average telomere length in stem cell pools. Fasting can help with MS, Depression, BPD, Autism and seizures. What breaks a fast? Anything with protein or carbohydrates in it will break a fast but most teas and herbs are OK. Supplements and meds often break ketosis directly or contain a filler that will. Many meds are dangerous to take while fasting. Does fasting lower testosterone? No, it raises it when the fast is broken by increasing lutenizing hormone. Fasting also increases insulin sensitivity, which helps with muscle building. Fasting activates autophagy (literally self eating), which causes cells to recycle damaged proteins and foreign matter such as viruses. Lowering insulin via fasting virtually eliminates chronic inflammation in the body. Weight loss from daily caloric restriction has 1/4 to 1/3 of the weight lost as lean tissue while many studies show fat loss from 36 h fasts without losing any lean tissue! The obese will lose loose skin while fasting, but the frail will have increased growth hormone release, which helps to make more lean tissue and reduce frailness. Fasts of 36-96 will not affect short term female fertility or affect menstrual cycle. They also may increase long term fertility for some women. It increases mitochondrial function and repairs mitochondrial DNA. Good mitochondrial function also has the added benefit of increasing your metabolism, fighting infection and cancer prevention! 24h of fasting can cut your leptin levels in half! This reduces leptin resistance, which impairs immune function. Over time fasting normalizes stomach acid levels and during the fast acid levels go down to allow for the healing of ulcers. Fasting reduces pain and anxiety by stimulating the endocannabinoid system, just like the effect of CBD oil Your brain prefers to burn ketones at a rate of around 2.5 to 1 when they are available in equal measure to glucose. Except for brief periods of very intense exercise, your body mainly burns fats in the form of free fatty acids. Fasting increases telomere length, negating some of the effects of aging at a cellular level. Fasting releases BDNF and NGF in the blood. This stimulates new nerve and brain cell growth, which can help a great deal with diseases like MS, peripheral neuropathy and Alzheimers. When not in ketosis, the brain can only burn carbohydrate, which produces a great deal of damaging ROS the brain has to deal with. Fasting stimulates apoptosis in senescent or genetically damaged cells, destroying them. Senescent cells are responsible for many of the effects of aging and are a root cause of the development of cancer. A fasting mimicking diet for 3-5 days in a row provides many of the same benefits as water fasting. FMD usually has 200-800 calories, under 18 g of protein and extremely low carbs. Exogenous ketones can aid with fasting, allowing some people with specific issues to fast in spite of them without worrying as much about hypoglycemia. They also help with dementia and many other issues even if you take them while not fasting! Glycine and trimethylglycine can also be useful supplements while fasting that won't break ketosis and have many benefits. Children, pregnant or nursing women should not fast for periods longer than 16 hours. People with pancreatic tumors that excrete insulin or certain rare forms of hypoglycemia generally cannot fast at all or only with doctor supervision. Type 1 diabetics can also fast but it is more complicated and should be approached with caution as it could lead to ketoacidosis and done under doctor supervision. If you experience extreme symptoms of some kind, especially dizziness or tremors, then simply break the fast with a low carb meal. GLP-1/ GIP agonist user here, for 5 months. pre diabetic and overweight, not obese, at the start of treatment. Personally this stuff has been the biggest gift ever for me. From day 1 I was eating 120g of protein a day, 30g of fiber, and doing resistance trainign every day. And yes, I had done these things before, but just couldn't be consistent and would self sabotage with binges. Now I don't. Now I can do all those healthy things CONSISTENTLY, without falling into that awful cycle of diet, fail, binge, depressed, diet, fail, binge, depressed, etc... I can now concentrate on more important things, like being a nicer person, a better mother, a better wife, a better friend. My brain capacity is not entirely taken up by planning, worrying and obsessing over food, weight, etc. It has take away the addiction. Yes Dr, you are right, the food environment is to blame, and this medication makes me immune to the allure of it. Not bad.

Very few things flabbergast me more than the willingness of people to reflexively abandon all concerns for *long term* safety for a quick fix, especially given the notorious history of the pharmaceutical industry promoting drugs as "safe" only to be later discovered, years after, that such drugs were not safe. When it comes down to two routes to achieve a goal, it is prudent to choose the one that has a longer history of safety (diet and lifestyle) than the option that has a shorter history of safety (weight loss drugs).

I've eaten clean keto for 15 years. It is a great way to stay healthy and maintain your weight (and muscle mass). I'm now reading that it also supports mental health (I think of 40% of college kids feeling depressed and anxious). I also do intermittent fasting. We don't (nor did we evolutionarily speaking) need to eat throughout the day. You only have to do this (I'm starving!) because people are carb dependent. Fasting is so important not only for weight loss/maintenance but also for autophagy (cell clean up and repair).

If you dont want to feel hungry and have cravings you should avoid most of the carbs and sugars and the ultra processed foods. Replace them by healthy fats, low carb vegetables, cheese, nuts etc and a normal amount of protein. You will get a stable blood sugar, no cravings, eat automatically less and lower your insulin level. A low insulin level makes it possible to release the fat from your fat cells and you will loose weight. You can eat a lot of nice foods without carbs and sugars. Don’t forget you don’t need carbs, your liver can make it if needed and there is always a little amount of carbs in your vegetables etc. Much healthier and cheaper and no hunger! Then buy yourself a vacation with all that money you saved!

100% correct. Every time you eat a carb (and protein) there's an insulin response. There can be very little sugar circulating (around 5 ml) in the blood. Some, but not a lot is stored in the liver and muscles. So once they're full the rest is stored as...fat. You aren't fat because you eat fat (yes even saturated). You're fat because you eat carbs. People need to ask themselves. Why can't i use the body fat I'm carrying around for fuel? It's like you have a full gas tank but can't access it. You body can use fat for energy. You just have to stop stoking the carb furnace.

This might be a different perspective. I’ve been firstly on Ozempic, then Wegovy when it took over in 2023. I switched to Mounjaro earlier this year. I had been keto for years +10yrs), carnivore for a while (4yrs), learned about fasting from Dr Fung over 10yrs ago. I had a small amount of weight to lose, yes I lost a bit but it has relieved me of my 30yr eating disorder. I stopped bingeing about a week after starting. I cannot tell you how freeing it is to not think about food all the time. Food doesn’t scream at me. Nothing ever helped - carnivore helped for maybe 4-5mons then that urge returned with a vengeance. Fasting makes me 10x worse. I’ve never been diabetic, I was insulin resistant but fixed that with keto and fasting many years ago. I understand the risks but at the moment, I will continue taking it for the mental break it gives me. I remain keto/ketovore, still workout including heavy weight training but the binge-free brain is giving me space to re-train my relationship with food. I will hope to come off this, tapering back down maybe in a year or so and am hoping that time away from bingeing and fixing my brain issues, will mean I can be like a normal person.

Its the same as the bariatric surgery craze (gastric sleeve or bypass). Too many see that as a magic fix. They dont have to change their lifestyle or do the tough things. Well...that surgery can be defeated if you dont change your habits. 3 out of 4 friends that had the surgery gained ALL of the weight back as they soon stretched out the remaining stomach they had. And 2 of 4 also later had to have gallbladder removed.

Ozempic doesn't fix a bad diet, it doesn't fix a lack of exercise. You just eat less of an already nutritionally deficient diet which just enhance all the issues you may have outside of weight. If you can just eat a high protein / low carb diet that avoids added sugar and seed oils with some minimal weekly resistance training your already 80% of the way, there. Ozempic could help with the cravings then. Compounded Semi is a far cheaper option.

I have lupus and keeping my blood sugars down with a GLP-1 when nothing else worked may be the difference between a kidney transplant or not. These doctors and media need to stop with the scare tactics against these drugs, SCIENCE AND RESEARCH IS SHOWING THEY ARE HELPING, with typically only 1% having serious side effects, and the most common side effects are easily manageable.

I totally changed my diet, when you stop to think the main purpose of consuming food is to provide your body with nutrition and people are treating it like an amusement activity, to eat to please themselves. And hence they consume so much garbage just to "enjoy" food. It's really no different than doing drugs, taking advantage of the pleasurable feelings your body gives when consuming something that adds nothing to your health. I had a wake-up moment, why am I eating this garbage?? Sadly, because society condones and encourages people to treat eating as entertainment, most people will only wake up when they have a heart attack or severe illness. We are victims of marketing and advertising. And the health care INDUSTRY only benefits from it.

Carnivore diet is like a vegan: human invention and can’t end well for people on it. As there’s no such an animal that’s perfect vegan ( as they all consume animal protein), there’s no such animal that is a perfect carnivore ( all of them eat veggies). This diet can benefit you short term but will come on your health with high interest rate when you do it long term.

@@saraandstuartshannon2160 I disagree. Paleo-anthropologist can find NO evidence that people only ate "vegan". Our nearest evolutionary cousins have very different digestion systems to deal with a plant based diet. There is overwhelming evidence that meat/fish was the basis of our diet. The seeds, nuts, fruits/veg were supplemental (and seasonal). Native people preferred animal fat (and protein) and Inuit, are it exclusively (with no cancer). I just read a book call Change your Diet, Change you Mind. While Keto is recommended, for people who don't find enough relieve, carnivore is recommended. This way of eating is natural and healing. Vegan diets that are naturally high carb (constant insulin drip) are not.

As for myself, mostly because of a knee injury done decades ago playing baseball, I was starting to experience a lot of pain when I'd go hiking, which I love to do. I as way over weight, approaching 300 lbs. I decided if I wanted to continue enjoying the outdoors I needed to lose some weight. Taking advantage of weight lose every time I got sick, which was actually fairly rare, and making sure I didn't gain it back and finally reducing my carbs and rarely eating fast foods and stuff like TV dinners I managed to drop almost 100 lbs. I didn't rush it, took almost 15 years. Doing it that way it was amazingly easy. It didn't hurt that I actually enjoy cooking.

Dr. Dhand, you conduct yourself in an intellectually and morally sound manner. Diet and exercise is the ONLY healthy way of losing weight and protecting muscle mass. In addition to the side effects you mentioned, the drug will saturate the cellular walls of your cardio vascular system thereby depositing toxicity within the particle alignment. The chemical compound is ubiquitous for it will branch from the central nervous system to all areas of the body. If you are taking this drug and consuming foods which spike triglycerides, it will lead to further long term health complications. You have continued to keep yourself in "Tip Top" shape. This is what patients want to see when they visit a doctor. Excellent Video!

I know from experience how painfully slow the process of fat loss is. Once you get to a position where you are obese, you are then in your own self imposed prison. Going to the gym, swimming, walking etc is initially painful, time consuming and yes, even embarrassing (because some people stare, make comments and laugh at you). The process of changing your diet is also painful and frustrating. Cutting down on carbs is frustrating and eating more proteins can be difficult because they are an effort, can be monotonous and more expensive than cheap carbs. You have to have patience, and be very disciplined. Many people do not want to go thru this and opt for the Ozpic instead. I can understand why, but in the long run, sarcopenia, nausea, vom'ing and decreased gut motility is a very very bad option

You're right. It is difficult but worth it. I eat keto and intermittent fast. My food bill isn't any more even with grass fed beef because I eat less high quality food. My body wants fat first, then protein and maybe carbs. I'm full. For example I eat around 1 pm. For lunch I have 4 eggs cooked in coconut oil (sometimes another protein) and a veg. Dinner fatty meat, a salad with nuts and a few berries, a veg with butter. Occasionally I'll have some nuts or almond bread. Not expensive because all the other junk never gets in my cart :)

You can try to cook a little bit of carbs en mix them with a lot of veggies and legumes you prepare in a wok. Use a bit of olive oil to bake them first (with chicken, other bits of meat..) and than don't let it burn to much by adding fresh tomato's... fresh courgettes... on a low fire. At the end you can add a can of lentils. You can add add herbs and spices as you wish. It will taste well and don't give you the feeling to be hungry. Same for cooking in wok and adding a bit of rice or potatoes. 😋

Semiglutide is seen as something that can keep you slim while you still eat junk food. It motivates people because everyone wants to be thin fast. How many people have you seen say I am using Ozempic for weightloss with the help of a dietician and a personal trainer. None of them change their diet. So instead of eating 10 burgers they now eat 5 plus best of all they can drink alcohol. So they think by being thin they are healthy but you can be thin on the outside and fat on the inside. I will only be impressed if someone says "I lost 50kg on Ozempic..... and they add with the help of a dietician". If they don't then you know they still haven't sorted out the food problem. I am 60 and we grew up thin. My parents taught us how to eat and we knew that losing weight is a slow process and it is linked to food.

Both my sisters-in-law are on Ozempic, despite my warnings. One already had her Type 2 diabetes under control eating ketogenically, the other just likes her mac and cheese too much. It's lazy and so typical of our pop-a-pill society. Ketogenic/Ketovore/Carnivore eating cures Type 2 diabetes, but that involves personal responsibility and we know how that is in short supply. I eat mostly carnivore and it is no more expensive than buying numerous other foods like pasta, sauces and seed oils. You throw you money from those to buying beef and eggs, etc. We do it on fixed income. I used to be almost 200 lb at 5'5", influenced by hypothyroidism but I no longer struggle with my weight, haven't for five years since eating this way (recommended by one of my favorite doctors) and take only Synthroid to control my condition, no other medications at almost 68 years old. My BMI is 23 now. Neither of my sisters-in-law look good and their skin is dry looking and pasty. They are a few years younger than I am. Sad. I feel they will regret taking that poison.
