If Ben were to die, would the Omnitrix die with him? 

The omnitrix now has a fail safe that does what it can to prevent Ben from dying by quickly changing him into the alien to survive

Im unsure about the final official Omnitrix but if vilgax thought that the prototype one would become unusable if Ben died he is likely correct due to how much he knows about it... I mean vilgax was even able to forcibly remove it!!! That's quite a feat and if he can accomplish that he can certainly identify if the Omnitrix would be usable after its bearer died

If Ben did die, the Omnitrix would cycle through its playlist and recreate Ben's body. That's what happened in the final episode of Omniverse and the Classic Continuity.

I am of the head cannon that if Ben were to die while wearing the Omnitrix, it would do everything in its power to prevent or counter it. If Ben wasn't wearing the Omnitrix, like the roughly 5-year period between the classic series and Alien Force, then Ben is essentially as good as dead and anyone that can use the Omnitrix is able to.

We have seen the recalibrated omnitrix used by vilgax and asmuth. If ben dies as we seen in alien force the omnitrix would prevent it by transforming into an alien to stop the death. If he has died somehow he would probably just come back. But let's say the omnitrix is detached and he dies. We'll I guess it would just be able to be used by another person. Most of this is off of the recalibrated omnitrix though

The Omnitrix will create a new body for Ben to inhabit if it ever gets that serious!.

The omnitrix has a failsafe to wear if Ben is going to die, it will do anything in its power to prevent it, as seen in the final episode of omniverse the omnitrix successfully prevented ben from dying to a device that destroyed a universe. While this is the complete Omnitrix the prototype might have something like this.

I've always enjoyed the idea that the Omnitrix is either sentient or has such an advanced AI that it learns and grows on its own. The standard answer to this would be what we see in the finale of Omniverse where the watch will cycle through all the transformations and forcefully transform Ben into the alien that has the best chance at surviving (and since Alien X is an option there is very little if anything that could kill Ben). However, I like the idea that if for example Ben's arm was cut off, the Omnitrix would expand and replace the limb. I recently read a pretty good fanfic that followed this where Ben's watch arm was blown off in an explosion that would have killed him. But the Omnitrix detached itself from the severed arm, attached itself to Ben's body, and grew to replace the arm while wires and circuitry went inside Ben and acted as a life-support machine. A bit gruesome but a pretty neat idea. The other idea I like is the Omnitrix naturally fusing itself to Ben more and more in which case we get Omnitrix!Ben.

The Ultimatrix is essentially an Omnitrix. When Chromastone was done for, the Ultimatrix rebuilt ben, saving his life. Then he died due to magic, the ultimatrix just remained attached to Ben's body. If the Ultimatrix has that kind of behaviour, so will the prototype, tho the prototype is slitghtly better at protecting the user than the Ultimatrix. And for the OV Omnitrix that's likelly impossible, not even the big bang could do.

Hmmm… that is a good question. Besides the self preservation function, perhaps it functions similar to a Guyver Control Metal.

I think it's still working. Y'all remember the episodes And Then There Were None and And Then There Was Ben? Ben Prime died, and No Watch Ben could still use the completed Omnitrix (and did never misuse any aliens).
