There is a reason why pilots go through a written pre flight check list. I don't see why this wouldn't apply to other professions where lives are at risk.
Personal stories: a) I knew a big, burley guy once. A little overweight, but mostly a solid rugby/football player in high school. He was diagnosed with high blood pressure. Refused to go on meds. Wanted to find an holistic method. Was a 60s hippy in 2012. Ended up having a catastrophic stroke. Visited him with a friend who had gone so school with him. I remember riding the elevator after seeing the guy; silence. Both of us in shock, that this outgoing, incredibly gregarious guy could no longer speak. b) My brother had cardiovascular issues. (A family thing.) Was on blood pressure meds. Ate extremely well. Ran. Was 65-going-on-50. Was walking to the mailbox at the end of the driveway, had an incident (massive stroke) and was dead before he hit the asphalt. Lesson? Sometimes you can do nothing right, sometimes you can do everything right and your days are still numbered. That's not a nice lesson, so how about this one: Love as much as you can while you're able. 'Dance like there's nobody watching?' How about 'Love like there's no tomorrow. Family, friends...and yourself.'
What's frustrating about this is Shaun was literally watched them and Dr. lim told him to leave. How is Murphy supposed to know how to teach when they don't teach him? Like where are his leadership trainings? Ultimately they where being bad teachers to Murphy.
I think it was more to give them a chance to stand on their own two feet without their boss looking over their shoulder which would make most people really nervous. a good compromise would be for him to walk away but then come back occasionally to have a quick look to see how they were getting on.
Bruh i beat that, had a guy with chronic htn and refused to take meds and had a procedure that day. BP: 298/145. I look this man in the eyes and told him to leave
I wanted to bang my head on the table when I got a reading of 248/121 and the patient exclaimed, "Oh, that's pretty good for me!" 20 minutes of gentle education later, and I'm pretty sure she still never bothered to fill her BP med cocktail
Nooooooooo, DO NOT EVER GIVE TPA WITHOUT A CT SCAN. The CT isn't to locate a clot, it's to make sure that the stroke symptoms aren't from a bleed or other problem like a tumor, so you don't give someone tPA and then cause a fatal herniation or bleed. I know these shows are written for the drama, but this would never and SHOULD never happen. 🤦♀️
You're right. It's a drama. Also, it's a fictional representation of reality. You're watching it for the entertainment. If you want facts and knowledge, that's an entirely different genre of video.
WOW you're so smart. You managed to figure out Hollywood's secret: every show is basically the same. Nobody watches multiple different medical dramas for a new premise. They watch it for different writing and characters that have their own character development
@@jacobtorres1835 Thank you Jacob! I am glad somebody said it. Unlike House the central character Shawn is likable and just to reiterate what you said pretty much all of the characters on this show have shown development and growth to some degree or another.
I never thought its possible for a stroke to still make you look normal because the woman is speaking fine she can follow directions but eh you learn something new I guess
Stupid. Giving tPA without CT head to exclude Hg. Also you need to bring BP down before starting tPA otherwise the patient will bleed even if not haemorrhagic stroke to start with. Ridiculous!!! Also don't you need to consent the patient first?
@@kevinwaag9976 first it is standard practice for this. Second, it is standard because treatment such as tPA carries high risk of complications so consent is crucial just like having surgery.
@@AngeloDenwillPetate No...first off TPA is NEVER premixed and sitting around waiting to be spiked. Secondly this Dr does not know if the stroke is from a clot or a bleed...you need a CT scan to figure out which one because if it's a bleed then he just killed the Pt. Third and most important, Doctors do not give this med...EVER!...this responsibility lies with the RN who has gone through extensive training and knows things like lie the Pt down and put her on a monitor. Kind of long winded but there it is....lol.
@@lumberluc I mean when you see them running around like headless chickens who barely know their left from right, I would too. Although looking over them acting like their going to fail will probably raise that chance.
@@nekolalia3389 yeah . i never watched this show though and i kept saying he looked familiar when i watched this video . that’s when I looked it up ..he still looks the same , just older 😂
I know this isn't a my-dick-is-bigger contest... ten years ago I was admitted with 298/120. Put on an IV and something like a nicotine patch that crashed my BP to a more acceptable level. It gave me a crippling headache and never want another one like it. But its on the way back up - Wednesday I had 217/102. Now on different meds and waiting on a cardiologist appointment.
Yeh ikr? I suppose it's because he's good at organising stuff when it becomes important to him, like saving a person but teaching someone doesn't seem as important to him so he doesn't waste his time. The only reason he's saying it all out loud is because of his medical training I suppose. (This is not dact, this is me guessing, pls start a war woth me if you disagree, I love those 💛)
Yeah he violated a lot of guidelines here and is putting this patient at risk. 1- he didn’t get a CT of the brain to rule out a bleed. 2- there’s no evidence from when her symptoms exactly started tPA has a 1-4.5 hour window 3- tPA cannot be given until the top blood pressure read is less than 185. Man’s a menace.
@@S1rT1mmySh33p don't worry i don't do comment wars ^^ and i agree. when he's focus on the medical he doesn't overthink the teaching part he just explain in the moment what he does and why = teaching
No matter how smart Shaun is I am disturbed by him administering TPA without knowing whether it's a hemorrhagic stroke.... But then I guess medical dramas are more about drama than medicine.
She had no paralysis and that proved she didn’t have a hemorrhagic stroke she also had no weakness in her limbs so I’m pretty sure he knew it wasn’t a hemorrhagic stroke
@@devinthierault yes but patients show up bc they think there tongue is lopsided and have extreme anxiety bc of something they saw on tv so a little more realistic writing instead of being lazy would go a long way to building actual drama instead of artifical situations that could impact people who don't know better
Is it me or Shaun is another Dr. Melendez when it comes to supervision ? ( Without sarcastic comments of course) You should never trust your residents . 😁😁😁
Great continuity detail. Shaun was taught BY Melendez, So you would see some of Melendez traits on Shaun when teaching since he was also in the receiving end of Melendez' "challenges"
Sometimes these kinds of shows make me regret backing out on med school but then again I remember the amount of torment I suffered through IGCSE Sciences alone and I thank myself all over again for not getting influenced by my parents.
Basic first aid training and the sections in the SAS survival manual pertaining to field medicine are about all you need to stabilise someone in an emergency. At least until paramedics arrive.
Watch some ‘fire department chronicles’ where he green screens himself into TV programs and points out the major mistakes they make. All in jest but it is really funny.
There’s around a 20% chance she’s hemorrhaging instead of having an infarct which can’t be determined by physical examination. A CT scan is ALWAYS done prior to admin of tPA. Shaun would probably get fired for this.
0:58 why does that doctor on shaun's right look like he's stuck in an uncanny valley lmao. he looks like a woman did a bad job tying to look like a man.
Urm norman bates was a psychopath with delusional hallucinations..... this character has some kind of autism or aspergers completely different. I would,d trust a doc like this any day because the one person I met who had aspergers was like a computer, he was amazing, remembered everything, seriously intelligent he just has trouble with social interactions and social emotions and expressions. I guess the only analogy I can think of it must be like living as the only human amongst androids..... the behaviour he must try to understand is foreign and not something easily adjusted to let alone understand and they dont understand my behaviour nor emotions either adding to communication difficulties except the raw data ie words spoken or written can be understood but the tone used, facial expressions can't be understood. So robots wouldn't understand sarcasm easily.....
@@601salsa you realise this isn’t real. Like the medical profession would let someone with his issues near a patient. Freddy Highmore is making a killing without actually acting.
@@TheMightsparrow that's where you're wrong. Many people on the autism spectrum are doctors or nurses. Lawyers or teachers. The show isn't real but the genre is historical fiction. Or something like that.
@@Beesechurgerplease Totally! And I’m not saying it’s a bad show because this isn’t unrealistic or anything ... that said, this is a huge fuck up to anyone in medicine. This scene is specifically supposed to be him showing his super mega genius to his students, but it’s the exact opposite. This would be like if while watching a show about fire fighters you saw one say to the other, while fighting a fire “we have to put this fire out!” And then started spraying the fire with gasoline 🔥 In the real world that patient could have died without the head CT, and it’s just an odd decision for a show about a “good doctor” to make, just for the drama of it all.
Right sided deviation of the tongue would mean a Right sided Medial Medullary Sx. Blood supply Vertebral Artery> PICA. However he needed to confirm if it was a hemorrhagic stroke via a CT atleast, because administering TPA to the patient would kill her within 10 mins if it were one.
Yeah just based on the tests he did does not confirm what he concluded here. It doesn't rule out a bleed for instance. For every case like this, there is an ambulance chaser salivating at the opportunity.
It's not a good idea to learn from Medical dramas since they're not always 100% representative of actual real life scenarios They do try to be as accurate as possible but as someone pointed out in the comments Medical dramas are more of drama than Medicine If you really wanna learn, there are tonnes of channels on RU-vid, Osmosis being one, where you can learn Also Dr Mike has some good videos You can check them out
Read and learn the signs of a stroke also you can have normal blood pressure and still have a stroke. Also if she is on a diet plan and exercise plan, medication to help lower her blood pressure have her follow that also go to her doctor appointment with her ask questions.
@@sunmoon3147 hypertension medication (depending on where you live is not expensive) even in the USA. From Wal-Mart for instance a 90 day supply is like $6. But, medications aren't the only way to lower BP. Diet and Exercise can also help lower BP naturally along side medications.
@@kween5600 Exactly. This show is effectively ruined after the death of Melendez. Breaks my heart to see such a loveable character killed and such a good show gone to trash 😕
"I have high blood pressure. I have high blood pressure. I HAVE HIGH BLOOD PRESSURE!" "Do you think that it's a good idea to intimidate me?" "I have high blood pressure! I have high blood pressure! I have high blood pressure!!!"
