In partnership with the Institute for Urban Indigenous Health (IUIH), four Medicare Locals in south-east Queensland are working together to fund and deliver an innovative program that aims to improve health outcomes for Aboriginal and Torres Strait Islander people.
In this video, IUIH regional care coordinator John Gill, and Colleen Costello, an Indigenous Elder from Stradbroke Island, discuss how the Care Coordination and Supplimentary Services program is helping Aboriginal and Torres Strait Islander clients with chronic diseases by improving their access to coordinated multidisciplinary care.
The program is funded by the Department of Health and Ageing. Working in partnership with the Institute for Urban Indigenous Health (IUIH) to deliver the program are the Metro North Brisbane, Greater Metro South Brisbane, West Moreton-Oxley and Gold Coast Medicare Locals.
The program is open to any Aboriginal or Torres Strait Islander person who:
* has at least one of the following chronic diseases -- diabetes, cancer, chronic renal (kidney) disease, cardiovascular disease, chronic respiratory disease
* has a care plan
* has a completed Indigenous Health Check
