This incident investigation video is for health and safety purposes and does not include any graphic imagery.
This educational video describes an incident where a lack of maintenance and improper servicing of an industrial steam kettle became a recipe for a disaster. The kettle exploded, injuring three workers and destroying a kitchen.
Using animation, this slide show describes the importance of safe work procedures when maintaining operating a steam jacketed kettle used in commercial kitchens. It also highlights the importance of regular inspections and of ensuring that servicing and maintenance is done by qualified technicians.

Same age old screw up: The electrician defeated all the safety devices then left without telling anyone to run the kettle of and on to keep it from exploding. That electrician needs his ass handex to him.

i used to work in a restaurant. didnt take me long to realize how dangerous a lot of the equipment is. and those never offer health insurance.

@@bimmersbbws6863 In America they can avoid even that , as many will drug test after any injury and a person fails the test , the company/insurance will not pay and is off the hook. Doesn't matter if the employee injured was at fault or not.

I make these things for a living it wouldn't have been that expensive of a fix the safety valve is most important part of that machine you think you was least check it to make sure it's working right

#1 Always have a qualified person check the pressure relief valve for proper operation. If there is ANY Doubt this valve is faulty (leaking by or stiff operation) replace it with a new valve of proper pressure as per the manufacturers specifications. #2 Before operating with the kettle cold make sure kettle water level sight tube is at the proper level if there is no sight tube make sure the low water indicator lamp is not lit. It is also very important to check the pressure gauge to make sure it is in the negative pressure range while cold usually green to assure the proper vacuum is in the kettle jacket so the kettle will not overpressurize during the cooking cycle. If not negative or green on the gauge kettle water level must be checked and jacket must be purged. Turn the kettle on at a low temperature setting and listen and watch to make sure the kettle is cycling on / off in the proper manner. Always use the kettle with temperatue set at lowest range possible for your cooking needs. Never cook / simmer at a higher temperature than needed. Some kettle manufacturers recommend 3-6 ozs. of a product like NAPA Mac 1300 antirust mixed in with the kettle jacket water. (Check with manufacturer about adding this product to your kettle.) Hope this will help you operate your kettle safely!

Thanks for posting this. I just bought a used steam kettle and was wondering about safety features and possible risks. I will definitely check with electricians that they are familiar with the safety features while they install.

FWIW many electricians don't know shit about mechanical blowoff valves especially specialty valves. YOU should have a continuity book YOU prepare with INSPECTION AND MAINTENANCE REQUIREMENTS PER MANUFACTURER GUIDANCE. Have the literature in the book so everything is in one place.

It's a good idea to check the safety devices in a second hand kettle like that. I also bought a kettle with similar working principle. When examining the pressure relief valve I found out it had been plugged with beans.

If you can't find a steam vessel repairman, maybe find a welder or someone else who works with pressure vessels? You should probably call your state or province's safety bureau and ask how to find the appropriately licensed repairman.

Would have made a great twist in that movie Ratatouille if the soup he was making simply detonated in the movie's first act. Then the rest of the movie was just a narrated accident investigation by Work Safe BC.

I work with these everyday at Fazolis. They are unbelievably hot. You have to make 100% sure you've turned off after you've emptied the water. Otherwise it will overheat and if things don't work right boom

I worked for a large gas utility in MASS. My first X as a service tech was kinda funny and no-one got hurt. I arrived and was heading to CO for details. Each firefighter gave me a look that meant your going to love this. Recent immigrants were cooking on a gas stove. They were making a sauce I believe and used a large covered cooking pot. The sauce must of over flowed and put the burn out. Gas built up in the space between outer cover and inner walls of the oven. Luckily no one was in Kitchen. When gas hit oven pilot it went boom. I entered the kitchen and sauce was all over the ceiling and on wall from middle to ceiling. CO from FD had me turn gas off and put lock on pipe till it passes inspection. Why do I put immigrant in comment. For a lot of new arrivals many of our comforts are new to them

Reminds me of the pressure well fryer I used to have to work with thing would be throughing alarms left n right under pressure n over pressure n constantly had to pull the pressure release pin manager refused to get it worked on until one day he did 2 weeks after itd just push steam out the locked lid n fill the floor with water

I wonder why pressure vessels aren't equipped with burst disks as well as (or instead of) relief valves? Either of those can fail, but a burst disk is failsafe (deterioration due to lack of maintenance is likely to cause it to rupture prematurely, rendering the vessel unable to hold pressure) in a way that a relief valve isn't (deterioration due to lack of maintenance is likely to cause it to seize and remain closed even at dangerous pressures); the main advantage of a relief valve is that it's reusable, and that isn't really a significant issue for something that's only there in case of emergencies.

But the burst disk would send the signal back to the control circuit, that he already had bypassed. Unless they make them such that they directly handle the current going to the heaters. Which would cause a short if it burst.

@@Roboto129 I think he means a dumb physical one on the outside of the jacket set to a certain pressure. If there is enough pressure, the burst disk fails, no matter what has been done to the electronics. Like the ones you see on chemical reactors.

My electrician friend worked on one of these and fucked up the valve... customer needed usage and he ended up blanking it off and carried on being used 😬 a lot of people don't realise just how dangerous pressure vessels are...

Any Electrician worth his salt can at the very least recognize electronics used for control, I suspect this "electrician" lied about his credentials and has no formal training. He should be in jail.

gee whiz, that dude made a bomb. The pitting itself , should have been a big red flag to say , this pressure vessel is no good and dangerous. i hope he lost his electrician license at least and sued for negligence.. Don't need anyone that ignorant doing even plain electrical work.

Kitchen is dangerous place... check security valves an other elements is very important. Iam electrician and mechanic. The temporally repairs is not recomended.

I am losing it over the safety videos it sounds like every job site I have ever worked at I'm not complaining but there is never any communication shit is field fixed and that becomes the permanent solution and then the new guy doesn't know about the temperamental qualities of the device and ends up getting hurt

Use qualified tech support? How about using people with the minimal amount of common sense or concern for other human beings? It does not take a high degree of technical knowledge to know not to bypass safety devices, much less in such a permanent manner. It does not take an expert to know that anything that will overheat out of control is inherently dangerous, this, without even having a notion of the dangers of steam. This is not an 'accident', this is criminal negligence. And, chances are the owner allowed him to 'rig it for now, we will just keeping turning it on and off manually'. That would make him an accomplice.
