Incident Investigation: Vacuum Truck Explosion Injures Worker | WorkSafeBC

As liquid hazardous waste was being unloaded from a tank connected to a vacuum truck, flammable gases and vapours ignited, resulting in an explosion.
Using photos from the incident site, this video describes the events, underlying factors, and unsafe practices that contributed to the incident. It also emphasizes the importance of knowing the flammability and other hazards of waste being transferred, ensuring there’s a safety data sheet (SDS) or hazardous waste profile sheet, ensuring proper risk assessments have been done, and minimizing workers’ exposure to hazardous substances.
This educational video provides workers and employers in British Columbia with a valuable health and safety training tool. If you deal with hazardous materials, know the dangers and how to control them.
Please note that this video was released prior to changes to WHMIS in 2015 and uses outdated terminology such as MSDS instead of SDS.

That's a hero I did the same our gas went nuts on a water heater I got everyone out then for some stupid reason ran back inside building and shut gas valves off before the flame got to main tank saved us all I was on my back reaching up to close the valve when it whistled all I saw was blue flash I rolled it just missed me but I felt my ass getting real hot.they called me a hero I looked at the supervisor said that was the stupidest thing I've ever done

It's cool the truck was equipped with air intake shutoff and all. Runaway diesels can be a problem. Glad dude didn't die.

@@dickJohnsonpeter I think it is mandatory for such trucks. Usually a Diesel engine is shut of by starving it from fuel. With those flammable vapours it gets fuel through the air intake,it was no longer running on diesel. I think there had also been several incidents at refineries where such things happen, usually you will see the engine reving up like a race car.

Driver recognized the danger (late, but he still recognized it), told the others to run, then took action that stopped a much bigger explosion. Dude is a hero.

Wow, I have a lot of respect for the driver, even though it sounds like they tried to pin some blame on him for the manifest bs. I hope he survived and was able to make a full recovery. I thought for sure he died from the looks of the pictures. He knew he was in danger but he was brave enough to try and stop a much more serious explosion

He died,,,,I think the switch inside the cab is what was the spark,,,not the engine...I do know if you have a natural gas leak inside a house and flip a light switch on,,that can be just a small spark...but enough to cause an explosion. Had correct procedure all through the haul and loading and the paper work....all were important,,,esp the paper work,,,,They didnt know it was so explosive!

He is part of the multi-stage process which resulted in extensive injury and property damage. A CDL driver is responsible for his/her vehicle and its contents. Sure, there is some culpability on some other workers who also overlooked safety protocols, but the driver carries a lot of responsibility in any situation because he is the one in control of that vehicle.

I did this kind if work for a sketchy outfit in Ontario. Whenever we were dumping at waste facility, they would always sample our load to determine it’s contents. I have no idea why this wasn’t done in this case, it makes no sense. My training was highly questionable and only given to me after I repeatedly asked for it and complained for a year. Pressure, intimidation, long hours and threats were the way things were done. I’m happy to be done with it and more happy I got out safely

I was in commercial/industrial construction for years and in HazMat Emergency Response for more than ten years. This is a great training video stressing the need for constant vigilance by all parties involved. Great job!

Seriously, who uses a Supervac to load or unload flammable liquids. I worked for many companies that had these types of trucks, but it was a big No No to ever load highly flammable waste products on these trucks do to all the different area flammable vapors traveled, hence the cyclones, bag filter housings. That is complete stupidity on the company, management, and even the driver. These trucks produced so much static electricity it isn't funny. I learned the hard way when I first got into the haz waste industry, working a job sucking fine sand from around an old 10,000 gallon steel heating oil tank, we were using 100' rolls of black corragated "slinky hose", well unaware of how much static electricity this creates, the driver asked if I would walk the 200 plus feet of hose and check for cracks in the plastic pipe. I straddled it and cradled the hose underneath and carefully advanced forward fixing any cracks with a roll of duct tape, at some point my body had collected enough static electricity and next thing I knew I was waking up in the hospital. Yep the driver thought it would be funny to see me get a little shock, well I got zapped so bad in my giggle berries that it knocked me out stone cold! It took me over 6weeks before I returned back to work. Moral of the story if your not qualified or not comfortable doing something refuse to do it, it may be the difference in going home at the end or to your grave!!

Tho gear pumps and diaphragm pumps are the safest way to pump flammable liquids however there are vacuum trucks that can do it safely with the use of scrubbers or like a king vac that uses cyclones of water in a special pump to produce the suction. I have hauled 1000s of loads with both. the biggest issue in this case is flame liquids dumped in a open pit. Most disposal places not only do you dump into a closed line you connect a ground strap to prevent statics charge. as the trucks empty simple physics says it has to pull air into the tank in order to drain. Not all but most places have a nitrogen line that you connect to your tank and is used to fill the tank as it drains. no oxygen no boom. Sounds like many things are to blame from the pick up to the end. So sorry to say but you are very incorrect about being able to use vac trucks for flammables as it can be done safely. But i think you are one of those i know everything story tellers that has no real experience except what you read see online. I mean six weeks for a static electric shock. not saying its not possible but not likely unless you fell and hit your head or got another injury due to it that took some time to recover I cal bullshit. If you are going to comment and tell all us nobodies how smart you are make sure you no something about it. I have hauled every hazardous material you can dream of with the exception of explosives. I did it for 15 plus years was over 50 drivers for a has waste company when i left due to health reasons. If you cross the t's and dot the I's and dont cut corners this stupid shit is easy to avoid.

Reminds me of when I was young and didn’t know how to say no to a boss. Woke up one day moving around 5000lbs green timbers and making a huge timber framed structure that was going to go above peoples heads. Also was tasked with trailering 5 of them in a crazy 5ft visibility storm. I was an 18 year old child, I have no idea how I didn’t kill somebody while driving or myself while building the thing.

I work at the plant where this happened. The truck went to multiple sites and sucked from multiple barrels and neither the operator or the consultant knew he had sucked on NGLs. When the driver dumped at disposal the tank was so pressured up the fluid shot out and hit the back wall splashing back. There was about 5 seconds between opening the valve and the truck exploding.

The burn pattern on the back wall suggests that a jet of burning fluid and gas was spraying against the back wall, what a nightmare. The static electricity generated by a fast moving flow like that can potentially be an ignition source in itself.

You have to assume that everything on the lease can at least burn, and most of it is explosive. I do feel for the driver. I spent 37+ years in the patch, and have seen a few wrecks. You can't be complacent even for a minute, but of course we all are. Except for the grace of God...

"Akron steam plant's troubled history." In Dec 1984 a dump truck from New Jersey with a load of sawdust mixed with oil paint was allowed to dump on to the large conveyor feeding the boilers inside even after other drivers commented about the obvious hazard. 3 people killed and several injured. 1994 an explosion resulted in arrests of haulers and employees.I serviced the burners in 1985 and heard stories of the hell that day.

I worked over 30 years as a truck and transport mechanic but I specialized in working on trucks and trailers carrying hazardous materials. I also had to inspect and certify these types of tankers on a yearly basis. Clearly I knew what I was doing but they always scared the heck out of me. I guess having this respect for these things is one reason I never had a problem with them although I know of people who did while working on them and it's never a good outcome.

Is there any measure to reduce the pressure in a tanker?i drive tankers and i believe there is a valve that releases and can also suck in air depending or excessive or inadequate pressure

Most of these accidents happen because they are procedures that have been done the same way without incident for years. It’s the pressure workers deal with in having to make the choice between doing what’s safe OR doing what’s expected (by your boss and peers).

Not really as you get inputs from a number of sources like a oil water separator at a car wash will have hydrocarbons but heavy ones suspended in water so needs treatment but not a fire hazard as it is.

In the oilfield yes. Unless you are hauling just straight water, if not it is considered flammable, which most chemicals and liquids in the oilfield are. Even oil on water can still ignite if given an ignition source.

A diesel motor will continue to run with no outside help as long as its getting fuel. shutting the key off stops the diesel fuel in the truck from getting in to the motor, but if there is combustible gases in the air it will continue to run. Hence the Positive air shutoff which does exactly as the name suggests literally suffocating the motor until it dies.

I once was loading from a open top water tank from a compressor and I was running a vacuum truck which if your loading the vacuum pump has an exhaust on it which releases vapors from the tank. Well I parked crosswind so that the vapors were being taken away from where I was working which was the rear of the trailer. Well I had jumped in the truck to fill out my field ticket and about 8 mins into loading I looked up and said to myself that smells like gas which the windows were rolled up so that was very strange needless to say the wind stopped taking the vapors away from the cab thus leading to the vapors being pulled in by the exhaust to the engine thus making the engine run. I tried turning the truck off it didn't work I jumped out then remembered my phone was in the truck so I hopped back in to get it then I remembered what a fellow driver had told me a while back which was to stall the engine.. so I put the truck in 6th and stalled it out thus saving me and the truck and the site.. why did I go get my phone well I was working nights in the middle of nowhere and it was a good 2 hour walk to the closest person... so this might save someone else... pop the clutch and stall the engine out if its early on otherwise run .... from that point on I always hook a hose to the exhaust on the vacuum pump and run it about 10 to 15 ft away from the truck crosswind when pulling from a compressor/gas plant

How does it stall at a standstill, is the transmission fluid coupler able to withstand that or something? Or is the drive to axle disengaged and it is running the pump?

@@00bean00 stick shifts (aka, manual transmission) are direct drive, no fluid coupling, only a clutch that can be slipped if needed. Putting the transmission in an excessively high gear gets you either a stalled engine (best case) or grenaded power train part(s).

I used to run 100' of hose, off the exhaust side, when vacuuming on gasoline storage tank bottoms, at refineries...my boss thought I was being overly dramatic. But, I never blew up, or otherwise FUBARed a piece of equipment...AND, I'm still alive, and retired!

They must have been in a rush. I would have never used the open hopper. Electrostatic grounding was not mentioned in the video either. That facility should have a strict policy on closed hoppers for flammable liquids. If blockages were an issue, create a larger input. Failing that, build a shroud for the open hopper to enclose the hopper while offloading flammable hydrocarbons. From my experience, pumping flammable liquids very slowly is always good. Less chance of static discharge.

Lets say this was stuff pulled from the sump pit so still filled with hydrocarbons but suspended in water. As they said in the video the pipe discharge is prone to blockages so if you thought you had a chunky safe load dump into the open pit is the better option.

They thought they were carrying water waste not high LEL vapour. So potential for suspended solids was possible. This was a complete failure of communication of the shipper and the driver.

It seems that the ignition source was not the engine of the truck. The ignition source was the switch contacts that would close off the vapor laden air to the engine. Given how the air shut off valve is used when enough fuel is in the air to keep the diesel engine running after shutting off the fuel injectors, the wiring for that valve needs to be explosion proof.

That shutoff valve is used to stop a runaway diesel, normally due to a turbo failure causing it to suck its own oil. I don't doubt there are some made for explosive enviroments though.

I've unloaded into that bay many times and we actually had to suck up all the water from the firetruck that spilled into the basement a few days after. They had the truck hidin with 400 barrels placed around it and knowbody was allowed to take pictures cause the investigation was still active.

If only they had the MSDS paper work on a clip board to sheild the operators from this explosion! Seriously this is a failure of common sense. The paper trail is to save you and your company's ass in a court of law. Common sense procedures and training save lives! My condolences to all involved.

The flammable vapors in the air were sucked into the trucks air intake, creating additional combustion. There was no time to shut anything off because the truck started to immediately “run away”. As someone else stated though, flipping that valve killswitch created a bomb in the engine instead of shutting it off.

The toggle switch was the source of ignition. It's open contacts. When they are close together a spark jumps the tiny gap. It is for this reason many newer switches have a high value resistor (~3Mohms) connected across the contacts to prevent the arcing caused by making or breaking the contacts.

The valve to cut off the intake is probably about the worst thing to activate if you know how engines work. He was all but guaranteed to have the combustion go out into the exhaust or intake since it reduces the cylinder pressure and slows the combustion down in event of a runaway due to externally ingested HCs. He'd still be unharmed if he had just not touched it. A better way is to just get the hell out of there. Another case of the cure is worse than the disease. If theres enough VOCs for the engine to race on its own then you need to get out, period.Edit: Could have been a sparking switch, but still shame on them for allowing high current to be switched by it instead of just a signal to an external explosion proof relay.

Sorry for my ignorance because I’m new to vac truck work. Should have the driver and facility hooked up with a hose to discharge the tank? I’m not sure what should have been done here to stop this from happening. I’m assuming the fumes wouldn’t be able to ignite if the driver would have controlled the way he discharged the truck. Thanks for the help so this doesn’t happen to me. Oh and make sure the paperwork is filled correctly so the facility knows exactly what to do I guess. Thanks

And this is why they should have a ground mounted hydraulic pump they can hook into to raise the tanks letting them turn off the engine. Would also put the controls at the hands of the staff rather then needing the driver to help.

Different methods could have avoided this. Distance from the hopper could have avoided this with wind direction in mind. If he was just gravity feeding to off load the engine didn't need to be running. If a little pressure was needed to offload the operator could of used hot offload procedure to avoid ignition sources. There are to many operators working these trucks that are not properly trained in the safest way to accomplish different scenarios of the different types of job applications.

If he was just off loading into an open pit gravity feeding would have been all he needed with the bed raised. Never a good idea to pressure lightends off. Diagram pump in a closed loop system is the way to go.

I have no idea if the paper work thing was the driver's fault or not, but either way they dude stayed in the danger zone trying to prevent the explosion and told others to get out. Dude was brave and got burnt because of it.

First problem, they didn't unload in the risers, I've done this work and have been to newalta many times. Any liquids in the tank gets pumped off first and solids go into a separate area. Never once had an issue and we've hauled everything from crude oil in dirt and sand to condie in dirt and sand. This, is workers being lazy.

My dad used to drive for an envrimentl co. The main product haueled, by drivers at his terminal anhow, was chemical lime. They operated two types of trailers. Bottom dumps and "air trains". To discharge his load, drivers would conectect a large air hose to a valve on the smoke stack of the tractor. Then they would would idle and rev the engine thus using the exhaust to presuraize the trailers, before open valves on the trailers which would alow the discharge of the load. though, I have seen this vid before, it just occurred to me, that this method could be quite dangereious if there is flammable vapors at the discharge site.

No matter what manifests or MSDS sheets the driver is given, those trucks carry a witches brew of interacting chemicals. Not a wise choice of occupations if you want a long healthy life

So the engine was burning the vapor as a runaway diesel engine, which the positive air switch arrests and then instead of being burned controlably it created magic smoke.

The driver never should have left the shipper without the msds,the shipper should never have permitted the shipment to leave and the reciever shouldnt have touched it.

They said he had no msds but did have the manifest.By law the driver must carry a little red book called a hazardous materials compliance book and Emergency Response guide. the red book has all the information info needed for every chemical thats allowed to be transported in the US. Then the ERG tells what to do in event of a spill and so on. if all info was not with the load the driver should of refused. once you accept its on you right or wrong same when you deliver once that manifest is signed it is the responsibility of the person taking possession thats why they manifest has places for all parties to sign over to the next. Laziness is the cause from paper work to dumping in a open pit. also he could of tested the materiel if he was in doubt. this is on the shipper driver and off load facility for sure. I hauled haz waste for about 15 yrs never a problem but you cant be lazy and cut corners its not just your life when shit goes wrong people get hurt.thousands of loads get hauled every day with no issues most the time. there is a lot of responsibility and paper work to driving a truck but about 10 times as much to haul haz mat. to renew a CDL in oklahoma is like renewing a DL pay your money every 4 years and no retest. unless you have hazmat you have to do a fedral back ground check and take a written every 4 yrs. the laws change so fast. this is why most places pay really well to pull hazmat. This was a bad deal never should of happened.

We as workers need to fight that urged to try and save things for instance when sonmthing heavy is leaning and falling let it fall dont ris hurting yourself or in this case with the alarms going off he knew enough to tell everyone to clear out yet he didnt follow his own instructions

You need the engine running because it turns a PTO that unloads the truck. Most vac trucks work this way.

The operators did not understand that they were dealing with highly flammable materials. That’s the point of the first part of the video. There was no MSDS provided to inform the operators of the danger.

I would say the truck driver saved his co-workers. I would probably have done what he did to save his truck not knowing the explosion would happen just when he shut off its air. Thinking back he did not save his truck and would have been much better to run with his fellows making sure everyone was clear. Should have been thought out by him beforehand as I feel I would have done.

The workers should not have done any unloading with out proper paper work. In this case it caused an explosion that should have never happend if the works would have done there jobs correctly and safely. instead they got careless or complacent or even lazy and caused a major incident with injuries to occur. in other words the human beings failed to things the safest possible way.

Ahh, those fracking dimwits struck again ... :/ Also, wouldn't it be a good idea to have the positive air shutoff valve close every time the truck is turned off via the key? That way, when the engine is in "runaway" all the driver has to do is to shut the truck off with the key. Also, the explosion risk is still there after the engine is shut off, because the engine parts are still hot.

You really have no idea what you're talking about. Diesel engines are all required to have positive air shutoff devices installed. You cannot run the engine with the air shutoff engaged. The engine is required to run the pump to off load. Also, when you spell "Frac" with a K you give away your lack of knowledge. No one in the industry spells Frac with a K. The word fracture has no K. This is NOT a Fracture Operation. he was transferring waste from one facility to another. You should probably just watch and learn before you yack off out your word hole.

@The94GTC you will only find them on petroleum haulers or vacuum trucks. New volvos with common rail come with them aswell and indeed snap shut when the key is turned off

This has nothing to do with an MSDS or the driver, it has to do with a facility designed and operating in a manner not even remotely consistent with common sense. If you know you're going to be pumping flammable wastes on a regular basis, why doesn't the facility have their own C1D1 rated pumps to safely make the transfer? They couldn't afford to buy a $5,000 pump in a multimillion dollar facility? What a crock of bull.

All the paperwork in the world is of no use if you are unloading flammable fluids and gas in a confined space and that place he unloaded was confined. Besides I cannot believe that the driver or even swamper didn't know what they were moving was flammable please give me a break. Best that could of been done was prep truck or trailer for unloading then shut truck off. STANDARD PROCEDURE IN CONFINED SPACES!!! There are facilities that have ground personnel on location to manage the loads and help out with paperwork if necessary and that facility looked like it would have personnel at all times. Anything coming from plants or oil patch have to always assume flammable fluids along with gas trapped in waste is present. One load say 125 bls of saltwater can be brought in for disposal and 1 bl be tank bottoms there you go danger. Proper training above everything else is key to the job.
