Incident Investigation: Young Worker Run Over by Forklift | WorkSafeBC

This video slide show recreates an incident at a rebar manufacturing facility in British Columbia where a young worker was seriously injured after riding on the side of a forklift and falling off.
Riding as a passenger on a forklift is prohibited in most cases. It can expose workers to risks such as falling off, being struck by objects, or being crushed between the forklift and another object.
Using animation and photos from the site, this incident investigation slide show illustrates to employers and workers the dangers of riding as a passenger on a forklift. It reminds employers of the requirements to provide proper training to forklift operators, as well as adequate orientation and training to young or new workers.

Yep, small indoor forklifts are heavier than you might think. It's kind of a "If it runs over your foot, you won't have a foot anymore" situation. Any guesses on how much a big forklift like that weighs? Here's a hint, it can pick up and move an empty semi reefer trailer (18,000 lbs)

I was trying to figure it out, I'm thinking 40,000lbs. I've driven 8,000lb and 15,000lb machines in factories, and this thing looks quite a bit bigger.

That's a Kalmar in the 12-15 tonne lifting capacity range. Dead weight is going to be around 20 tons give or take a couple of tons either way depending on exact model. Young lad may have received serious injurys, but he's *EXTREMELY* lucky to even be alive. Been a forklift mechanic for 20 years, they're vicious machines and something I always tell cocky young operators is forklifts can and do kill but worst yet, have a nasty way of hurting you in ways that would make you wish you'd died.

I've driven cat p36000s which weight 45,000 empty. Add in 20-30000 lbs of trench shoring and it will flatten anything.

@@Samsaknight No, not usually. And even though they are almost "impossible" to flip over, you at least have a chance at survival in the enclosed cabin. Also, it is likely to have a "roll bar" type reinforcement built into/around the cabin that can support the weight of the vehicle if it does tip/roll over. That is also why it is critical to wear the seatbelt when using large equipment so that you stay INSIDE the cab if it does flip. There is a very distressing video of someone who was using a very tiny backhoe (like 1 meter by 1 meter footprint) on top of a tiny dirt mound, the unit flipped, he tried to jump out of the way, but the cab area landed onto him and the comments said it killed him. The backhoe probably barely weighted 1 or 2 tons, and it didn't even hit him "that hard". The weight killed him. Don't mess around with powered equipment, read the instructions, follow the instructions.

My wife's 18-year-old cousin died in a similar incident, they were closing up shop for the day and he tried to jump onto the fork to get a ride to the shed and he missed and was run over.

Fuck that’s tragic. We often forget how often the incidents that happen are just like that. Having some fun at the end of the day and how quickly a situation can turn and change lives, and end them too. The importance of safety and awareness around the machines we use every day can never be overlooked.

Lord Jesus those forklifts are EXTREMELY HEAVY. I don't know exactly how much, but the thought of someone getting crushed by that is horrifying

Usually between 3,000 to 8,000 pounds because of the counter weights in them to carry heavy loads. Terrifying to think this happened.

PROPER TRAINING IS SAFE WORK. I exactly know how it feels if somebody that you look up to doesn't listen to your needs. In business, an inadequate orientation causes the employee that is being compensated with remuneration to behave in a way that they otherwise would show respect to authorities had the instructions been facilitated appropriately.

Doesn’t it defy logic how such events take place. I’ve spent a life time as a safety professional. Seeing this event come about hurts cause deep down you know no matter what the professional would of said, the employer would only laugh it off as nonsense….

I worked in a factory where a worker had his foot run over by a forklift. Said worker had to relearn how to walk. Luckily the steel toe boots saved the worker from toe amputation. Somehow the steel didn't bend but the wheel deflected off of the toe and up the foot.

Fork lifts are heavy as hell, have solid tires (usually) and have tons of hard steel on the outside, you don't want to play around and get run over by one, it won't be fun.

The employer killed this person. Their negligence and lack of care for the safety of their workplace killed them. They took someone's child away forever, just because they didn't care to follow some basic safety regulations or to have a single drop of common sense.

Luckily the video and description say the worker was "seriously injured", not killed. Hopefully he survived and recovered from his injuries.

And stay AWAY from it, even if you think you are in a safe area, consider taking an extra step or two away from it. They can turn soooo fast and catch you. People really need to over estimate how dangerous they are, and how much distance they can travel in seconds.

It's relevant because the worker that was ran over was young and inexperienced such as myself. I'm an operator at 19 and I've already messed up two or three times. Nothing to serious thank God. It's all in the learning process though. And yes older "experienced" men and women do some stupid stuff thinking they know everything about their equipment.

Rule of thumb for any forklift driver if you and the pedestrian can extend your arms and touch your to close to the lift if you can touch the forklift as a pedestrian at all you are to close DO NOT LET ANYONE NEAR YOUR LIFT WHILE IT IS RUNNING AND CURRENTLY WORKING NO MATTER THE CASE there are videos to where people die because they decide to get close to one these machines are made of pure metal its better to be slower at a job than a human blood puddle on the floor because once its all said and done that all you will be is just a puddle of the remains that was supposed to be you.
