I am absolutely mindblown at the high quality information contained in a 15min video compared to my 3hours lecture. Thank you so much for making med school (and life) easier!
Very good lesson. Thank you! My attempt at summary: Three things in the brain. 1) brain mass/volume (brain cells I guess?) 1300 mL 2) Blood 130mL (supplied by a main artery) 3) Fluid CSF 130mL (Which suspends the brain matter? "Perfuse"? Gives the inter-cranial pressure ICP) 3:00 Brain pressure (CPP specificailly ) must be in a certain range in order to give brain cells and neurons enough OXYGEN and NUTRIENTS to surive. CPP = MAP - ICP (mean arterial pressure - inter-cranial pressure) So if the pressure is not right, your brain will not function well. 5:30 causes of head pressure (ICP) too high = if any of the "three things in the brain" increases. Firstly, brain mass can increase due to tumor, abcess, edema, brain tissue bleeding. @6:00 Secondly, brain fluid (CSF) can increasue due to the following. Note that CSF is constantly produced and recycled within the head/brain, the brain stem, and the brain venous system. So, too much CSF fluid production OR a blockage in the CSF drainage can cause pressure to increase. @7:00 Thirdly, cranial blood pressure can increase due to high venous pressure due to previous heart failure? @7:10; high CO2 level hypercapnia; @7:40 symptoms of high cranial pressure: GCS Glasgow coma scale decreases (indicates brain funtion) ? headaches - because eninges layer stretches. 9:00 vomiting - because vagus nerve is pressed or irritated 10:00 papilla edema (optic disc swelling) because venous drainage is not good. Cushing reflex triad (high blood pressure, irregular breathing, low heart rate) 11:40
I'm not a doctor, I watched this to understand my condition better. My ICP was 40mls, at my last lumbar puncture I can not stress enough how much pain my headaches cause, my eyes hurt everyday too. Hopefully treatment will help soon.
For those who suffer from intracranial hypertension and are not responding to drug therapy (acetazolamide and/or topirimate), check out venous sinus stenosis and the option for stenting. Dr. Athos Patsalides and Dr. Waleed Brinjikji are two of the leading docs you can find on RU-vid for further information on this condition that is prevalent in a high percentage of people who suffer from pseudotumor cerebri/idiopathic intracranial hypertension.
Had to comment and tell you how insightful and informative your video has been. Diagnosed with IIH in 2010 after I experienced extreme headache, vision disturbance, I was unbalanced, had tinnitus and the list goes on but you are the first person that explained the vomiting to me!!! Im not a medical student but a lot of this video made sense and for that I thank you.
This is a mLgreat, educational video for new nurses like me! Except the sound was low, making it difficult to hear, even with the volume all the way up. Please keep making more videos!
Thank you so much! Especially about the Cushing reflex! (The triad) I was diagnosed a while back but I don’t really understand it. I have these headaches everyday but don’t have the eye problem so don’t need the shunt (not that I’m complaining) I just wish the medications worked better. I’m miserable everyday. At least I understand a little more. It’s helpful.
I lean toward idiot cat-scan procedures- goes with THIS TOPIC- Doesn't it? Asked for a MRI with conrast- Dr Doofus scheduled a Catscan without contrast- now I know a LITTLE MORE but didn't rule out what I'm fearing - Chiari 1 manifesting at this 😉later time in my life- now I have to drag myself to a larger hospital- with people who possibly have the knowledge I need. Dr Doofus said " I've never had a patient with THAT!" WELL WELL H.E.L.L.O! Maybe You do Now!
Just diagnosed with this condition. Thankful to know what’s wrong but so annoyed it took so long to get diagnosed. I’ve had double vision, dizziness and tinnitus. I was told there was nothing wrong with me and that it’s impossible to have double vision in both eyes.
I just went in for a blood patch for a csf leak as I get positional headaches. Turns out my csf pressure was actually high (27). They lowered it and I didn’t get a blood patch. I’ve been dealing with morning headaches for about two years. Although I’m not sure what the root cause is, this video blew my mind.
" I want to know why, anyone with Cranial Swelling is not put on, cold Packs, Under Arms, in Groaning area, and behind the Head in Neck Area FIRST, to help get the Pressure and Swelling Down, while finding out what is causing the Swelling?" " To me this is A "MUST FIRST!!!!!!!!"
i want to ask something sir regarding meningeal stretch and the pain stimulated... i learnt from anatomy book by snell that only dura matter is susceptible to pain while other meninges are prone to stretch reception only and the dura matter supply for pain is by some sensory division of Trigeminal nerve(5th CN)... CAN YOU HELP ME IN THIS REGARD?
Wow... My doctors said my sudden high Blood pressure had Nothing to do with my high ICP. Although it came up together with my headaches and increasing icp. My normal bp was always 110/65. Then, 150/100-110. So, when they normalize my icp, my bp Will go down as well? Never had bp problems, not even during my Five pregnancies.
Can a spinal cause this condition? Can being obese contribute to this? Can retraining water such as edema cause this problem? What about going to a chiropractic, would that help.
Can intracranial pressure elevate when taking drugs like Accutane? I have never expierienced problems with headaches or eyes etc. I am 1 year after my accutane course, and I have been seeing doctors and hospitals for headaches, espiecially pain at the back of my head, feeling of floating without losing balance, severe eye strain, where I can simply not look at computers at all sometimes but my vision stays perfect. I have been checking my eyes, and my eye pressure was elevated but not severly yet. I also have anemia so I am very fatigued. I feel like accutane caused some kind of imbalanced but my doctors wont really see it as a cause. What should I do to check the presssure in the skull? I do not want to get any syringes inside my head :(
I forgot to mention that my eys feel very heavy in the evenings,. its almost hard to open them up, and 3 months ago I used to have attacks at night when my eyes would just shut open suddenly like there was something dangerous. It does not occur as frequent now but really got me worried
my mother too, and then after 2 blood patches it seems to have resolved (MRI indication) but now it's HYPERtension symptoms that are creeping up (and MRi shows possible hemmorage) . Does your brain feel funky too? as in, vibrating sensation whenever you move? and motor skills are affected
i just went to the optometrist and found out that my already elevated optic nerve has elevated even more since my last visit and he thinks maybe a tumor or spinal fluid (most likely spinal fluid) might be pushing against it, is this what that would look like ?
As one who has suffered from this rotten malady I can attest that yes, it feels like the Chinese government could very well be taking up residence inside your skull and banging on gongs in rhythm with your heartbeat.
Just a suggestion : the explanation is clear but to make it more interesting 1) remove the background grainy noise 2) add a very light delightful music is background.
