Integrated Patient Information Form 

Is there a way to customize the documents we send, including which information we ask for on the patient information form?

I would like to see a RU-vid video that shows my client's how to create a Therapy Portal account. My clients are having difficulties creating a Therapy Portal account. They tell me that the system would not allow them to enter a password or create the account. They enter their e-mail address, and their date of birth is correct. But when they get to the password section, they described having difficulties. Please help! Thank you!
