Thanks I really enjoyed that review! Update- I just graduated from DNP school. I'm officially an NNP! Next step, boards!

YAY!!!! Congratulations Vicki!!! That's so exciting! I bet you're a fantastic NNP!!!! Good luck on your boards (although as you've obviously realised by now- it's not luck, it's hard work and caring!) Please keep us posted!!!

What a great hands-on lesson, absolutely useful. Love myself some case studies and lessons „straight from the bedside“, keep them coming :)

Thanks so much Christoph! So glad it helped! We'll have a lot more of these- and finally- when we get through the back-log of requests- we're planning on a series of videos on bedside issues- e.g. green emesis, or an SVT etc etc. Thanks for being a subscriber!

Hello Jee! Thanks for the compliment. We are trying to finish our core group of lectures (HFOV, NEC, BPD etc) and then we're planning on a series of common neonatal scenarios (e.g. SVT, bilious emesis etc). Thanks for your suggestions!

Great video as usual! Would you mind expanding on bicarbonate and serum CO2? I know you said the bicarbonate on a blood gas is a calculated number. Would a serum CO2 and bicarbonate on a blood gas be similar if drawn at the same time?
@TalaTalksNICU
@TalaTalksNICU 2 года назад
This is a great question! Even though as you said the bicarb is calculated on the gas- it's generally pretty close to the bicarb measured as part of the serum chemistry. May be off by 1-2 but they're definitely close enough to use. (Like everything in medicine- sometimes the calculated bicarb can be wildly off in blood gases- this can happen if the C02 is super high or low- and a few other rare reasons).

Hello Bea! We love your allegiance! Wish we could post more often than once every two weeks- but with jobs/ kids etc it's hard! Maybe in the future, as we become more efficient!!! Thanks again!

Thank you Nat_tasha! That makes us so happy! I am an NRP instructor, but we are concerned about copyright issues with posting stuff on NRP. We are figuring out what we're allowed to do! If that remains sticky- we'll go through code scenarios in the NICU. Thanks so much!!

May I ask if your hospital will use ISTAT to analysis Blood gas in NICU? Thanks
@TalaTalksNICU

Yes- most places I've worked have used ISTAT machines- love them because the info is delivered quickly!

Hello Havva! We're so glad you're still watching! That is a great idea- we've been wondering how to tackle the huge topic of fluids and electrolytes. That's probably how we should start! Thanks again!!!

Hi... What it's means when there is respiratory acidosis and the bicarb is 23.6..... PH 7.26, PaC02 52.2, po2 59, HC03 23.6, BE - 3. Thx u
@TalaTalksNICU
@TalaTalksNICU 3 года назад
Hello Bea! Sorry this is a couple of days late! This gas shows that there is mild respiratory acidosis (in a preemie we would happily allow this- and probably wean on it). The bicarb and the base are effectively normal. Remember that the base is a calculated number so does not always accurately reflect exactly what is happening. Does this answer your question?

Hello! It's very difficult for us to predict anything else about your baby from just these facts- we are glad he is slowly recovering though. Good luck!
