INTRAVENOUS CATHETER INSERTION IN CHILDREN WITH CEREBRAL PALSY

This video describes a technique that substantially facilitates IV access in children with cerebral palsy. Dr Hadzic guides the viewers through a technique he has used for years in near-impossible to find veins in children with contractures due to the cerebral palsy. Insertion of an IV catheter or drawing blood in these children, who are often traumatized by multiple needle sticks, is stressful to children, parents, and health care providers. However, using this simple, reverse Esmarch technique can turn a near-impossible mission into a successful IV placement.

The god of cannulation smites another difficult cannula 😍😍😍and also validates not wearing gloves for the toughest cases! Sometimes I get weird looks from the next of kin or other staff when I do that but you gotta do whatever's necessary to save that patient!

I can recommend using a heat source, like a simpel glove filled with warm water, on the cannulation site. Works wonders aswell.

In addition to this undoubtedly valuable tools, l would recall here simple heating of patient's palm under hot tap water. I personally very happy to get acquainted to this simple and useful trick

As a trainee in pediatrics I absolutely love your cannulation videos. keep them coming :) Maybe you can make one about spots for "blind" cannulation? As you know often times kids under 2 years of age are sometimes just a pile of subcutaneous tissue without any visible or palpable veins. Keep up your great work :)

This is such an overlooked population when practitioners discuss IV access, many of these tips can also be applied to stroke survivors who don't necessarily need a central access.

This is quite helpful to me as a registered nurse with cerebral palsy who is surprisingly normal looking, but a hard stick. Also having hypogammaglobulinemia, hypoglycemic seizures, type 1 diabetes, and frequent infections means I need iv infusions regularly, making me very appreciative of this review. I almost always go for long, straight forearm veins away from flexible joints.

Does using EMLA cream help? Of course if it is available, for the purpose of a painless IV access procedure.
@DRBLUESNYC

Totally Goran. Unfortunately, sometimes we get involved too late in the game when the OR is ready and waiting and the health care providers before you already exhausted their efforts - then there is simply no time for EMLA. Greetings and thanks for watching!

Thank you very much indeed for great video. Just one question: isn't it a better to cannulate a deep arm vein under ultrasound to establish a more reliable vein access in such kind of difficult patients? I'm afraid that 24 g catheter could be easily displaced or simple not enough in some cases for toddlers.
@DRBLUESNYC
@DRBLUESNYC Год назад
Hi there. In some situations yes. In this one - now because of the contractures that would make it difficult/impossible to position the arm for vascular access as you describe. Greetings.
