You have to just define the trigger condition on creation of RITM

You have the option to change it in administration --> properties

In the Survey designer you can find an only text cell, maybe this will do.

@@toniferra418 nope .. service now don't allow to make question ready only on survey ...there is no function defined in assessment_take2. And also there are so many scripts that disabled the read-only..One thing I also got to know that we need some DOM manipulation.but I don't think we need to do change system properties
