@@AH20XX where do you buy the cream from and how much is it? What were your T levels before and after the cream? Can someone get T levels over 1000 with the cream or is that only possible with injections? I tried the injections but I keep hitting a nerve or have some pain afterwards along with severe muscle twitches. I do not look forward to my next injection because of it. And the thought of dealing with this pain for the rest of my life is just dreadful.

@@CJ-yf5cr I don’t know where are you injecting but try searching a little more. I have learned that that leg and gluts is where you hit nerves and it’s not good at all. Pecs and lats is better. Just inject less amount of what you will do on glut or leg. You can inject twice a week instead of once. Shoulder or chest is fine.... just do it with a thinner needle 29g 1/2” works.

I have used both cream and injections. He is right about injecting every 2 weeks. So….you inject every 3 days. Like I do. 60mgs every 3.5 days. And that keeps me in the sweet spot. Insurance covers my testosterone cypionate. It lasts me roughly 10 weeks. And if my insurance didn’t cover it. It was 103.00 for the 10 weeks worth. Cheaper then the cream. Needles are subcutaneous. 1/2 inch. 27 gauge. Rather painless.

That's all correct. And if you inject in the upper, outer quadrant of the buttocks, it really is painless. Very few nerves there. I recommend at least a 5/8" needle to get into the muscle. 1/2" will work if you are lean enough. Cheers!

Thanks for the easy to understand, informative video. What is the strongest compounded testosterone one can get? I have just started using TT 200MG/ML T cream 2 clicks (0.5ml) a day. Is it best to apply .25ml twice a day rather than .5ml once daily. Also do I require a script to get thru you and could you ship to Australia? Many thanks.
@hormonespecialist
@hormonespecialist 3 года назад
Hi. My pleasure! Sorry for the delayed response. The 200mg/ml is about the strongest that can be compounded. It starts getting too pasty and thick at higher concentrations than that. Once a day application is fine for maintaining testosterone and it also allows you to space the application apart from when you have contact with another person, preventing transfer. We do require prescriptions, but cannot ship to Australia. Compounding pharmacies are abundant over in Oz though. I suggest you do some phone shopping for the best price.

@hormonespecialist
@hormonespecialist 2 года назад
Thanks for the comment. In my 20 years doing this, I find most guys settle on twice a week. Once a week allows it to drop into too much of a trough and can be a little too high of a peak precipitating acne. All you have to do is break through that threshold once or twice to trigger acne. I find that women can get away with once a week--probably because it's a much smaller dose.

What do you mean no effects? If you watch the video, injections are not recommended due to spiking. How do you know it's not working? 2 weeks is nowhere near enough time on it. You need to go 2-3 months and then get blood work done to check your levels.

Depending on your cream, the strength, and your starting testosterone level your dose may be too low. I just started and my doctor has me on 150mg/ml cream and I apply .5 ml twice a day for a total of 150 mg. You started at a very small dose and still only advanced to half of what most people are starting at. Initial improvement usually doesn't start showing for three weeks with improved erections, better sleep, and more energy but optimal effects take months.
5 лет назад
Injections twice a week work well also. I don’t know of too many doctors (who know what they’re doing) who would give a guy one shot every 2 weeks... that’s ridiculous. But daily cream is the way to go if you can afford it.
@hormonespecialist
@hormonespecialist 5 лет назад
You're right. More frequent, smaller dose injections better reach the goal of mimicking physiologic release if you're up for it. Anything to flatten that dose vs time curve. But it is an intramuscular injection with an inch or longer needle, so... not pain free. A T shot every two weeks IS ridiculous, but you'd be surprised what I see. If you choose injections, I recommend twice a week at a minimum. You can even inject daily, in which case you can do a more shallow sub-q injection. Even closer to ideal! Thanks JH. -Pharmacist Layne

@hormonespecialist
@hormonespecialist 2 года назад
Androgel is a commercially made transdermal testosterone preparation in what we call a carbomer gel, which uses alcohol (about 67%) as the delivery vehicle. Alcohol generally does not sit well on tender thin skin like that of the scrotum. It can burn, especially if it gets near the anus. This is why they have you avoid those areas. Additionally, because of it's relatively weak concentration of T, you have to apply 5-10 or more grams of gel to get therapeutic blood levels. That's kind of a lot to smear all over your boys, so they direct you to larger surface areas (no judgment whatsoever there with the adjective). Compounding pharmacies generally concentrate the testosterone in their formulations requiring less volume to deliver the same payload, and use bases compatible with sensitive skin. The scrotum is ideal because it is out of the way (avoids contact transference to others) and its very absorptive. We get good levels when applying there. And... as you mentioned. Compounded testosterone is MUCH cheaper. Compounded testosterone is often covered by insurance. Ask your compounding pharmacy. Thanks for your question, Mr. Chip.

Hi, I have heard topical testosterone raises DHT levels significantly. Is this true, and should I be concerned of balding? Also thank you for the video
@hormonespecialist
@hormonespecialist 2 года назад
Hi! You're welcome. It is true that the topical testosterone route raises DHT levels higher than injectable, especially when applied in the genital area. This is because there is a higher 5 alpha reductase enzyme level in the skin and genitals, which is what converts testosterone to DHT, but here is the hard truth. The likelihood of male baldness is most influenced by genetics. And those predisposed to it genetically can experience hair loss from even normal circulating levels of testosterone, so I think DHT concentrations are a bit of a wild goose chase. Yes, they bind with a higher affinity to androgen receptors than testosterone, but your receptors are likely already saturated, so we're back to genes. Thank your parents. I really don't like blocking DHT (with finasteride or dutasteride) because it's a party killer. DHT is a much more potent stimulator of libido that testosterone and you don't want to be without it. Besides, haven't you heard? Bald is sexy, man! I know, it's mostly the bald guys saying that.

as a RPh, is it better to apply the cream in the morning or the evening to simulate the body's natural release?
@hormonespecialist
@hormonespecialist 2 года назад
Hi Fred. The body naturally pulses the release of sex hormones so that receptors do not develop resistance. So morning OR evening is okay. It's best to make that decision based on when you are less likely to transfer your topically applied testosterone to your partner. The decision to apply hormones to the skin should be made by the owner of the skin! Now, having said that, testosterone drives libido and more so at higher levels, so if you want to apply it closer to the time of sexual activity, you'll probably like that. But give yourself 2-3 hours after applying it before rubbing that skin on somebody else's.

How do I get cream in Germany? There are only Gels and Injections, would really try the cream

You would more than likely need to find a pharmacy that could compound the cream. Compounding pharmacies can make different dosage forms that are not usually found at your typical pharmacy.

@@hormonespecialist Excellent video!! Been reading through all the guys comments, some great knowledge and experience’s here. I was going to ask a similar question about the UK…from reading all of the above I take it you don’t ship there? I have seen one or two guys above talking about the gel…if I am after the cream which many of the guys above are…what are we looking for…as I’m just assuming it’s not going to be that easy to find even in the uk…but will be starting some research straight after this!!!

What about eod or ed injections
@hormonespecialist

I presume by "eod" and "ed" you mean every other day and every day? I like that dosing schedule, AND it can be dosed sub-q with a shorter needle, so less painful. You use the same weekly dose (commonly around 1ml of the 200mg/ml testosterone cypionate) divided by the number of doses you give in a week. Be sure and get 3/10cc insulin syringes with 29 guage 1/2" needles. If you try and use the short needles, they are smaller around (31 guage, which will be like sucking honey through a straw) and won't work well. I have patients who do daily SQ injections. It just requires some patience pulling the oil up into the smaller syringes. You should check with your doctor if you want to administer your testosterone differently than the label directs. I'm just giving you options to discuss. -Pharmacist Layne

Cream?? Not a chance. If the levels are to high and to low in 2 weeks administration do it every week cipionate. Cream you have to do it every day, you cant sweat after, you have to wait for the absorption. Not for me.

I don't mind your take on the subject. You just simply state the cream is "not for me". I see so many people saying they won't use cream because it's not as effective as the injections. Injections have been the gold standard for so long that people are afraid to try anything new. Yes, you do have to administer every day but in reality, it's not too bad, apply, let it dry for 30 seconds, go on your way. You just can't hit the gym or have sex for 5 hours to avoid transference. The reality of it is that all recent studies have found the creams to be just as effective as the injections so it's a matter of personal preference. It's a personal choice and you like what you like.

1:13 this is kind of comparing apples and oranges, if you were compare this with microdosing testosterone injecting every day in the morning, you would have the same effect. No endocrinologist in 2023 is doing this old brutal injecting once every week or 2 anymore.

Once a day or twice a day? For the cream.

No, its just that people vary and some skin is thicker/thinner than others. For X percentage of men, topical just doesn't work. In this case, its now a question of convenience, rather, one of necessity. After all, who would prefer something you rub on? Not to mention is a whole lot easier to travel with.
