The sad part about this issue is that many medical professionals and doctor don’t even want to talk about the burnout for the fear of job stability and possible license issues if they speak out on top Of that lack of time to recuperate from hard week and lack of time with family and friends.
I totally agree, there is no shame to talk to a therapist if you are a depressed doctor I think more doctors should talk to therapists could prevent more suicides.
Yeah I don’t like how it’s not accepted to speak about anxiety and depression as a doc. You’re supposed to suck it up and keep working. Why is the culture like that? Any other profession welcomes breaks and wellness, but when a doc talks about it, their job is on the line and colleagues question their “resilience”. It’s called being human!! You just physically can’t work this many hours, see this many people, and train for this long without feeling tired and burned out. We’re just not built for that much work.
I have to say a word from the pharmacists. The stress and burn out is terrible, as are the working conditions in retail pharmacy. Pharmacy is the “quiet” profession. They take a tremendous amount of abuse from the public, while we do everything that we can to get your medications ready. A heroic effort to get prescriptions renewed by your doctor, insurance overrides, check for drug interactions, appropriate dosing, etc etc. Long hours with no breaks or lunch, it’s crazy.
I have followed you for years and am concerned for my son's future. he is currently jumping through hoops filling out his med school applications and studying for MCAT while in his senior year of college. I can see the demands on his time and energy compared to his peers going into business. I still need help understanding the need to work 89 hours a week or the demands on the body clock for lack of sleep; the justification for the wages paid during residency is equal to that of a fast food service worker. As his father, I almost want to tell him to find another field with his talents! ( But this is what he wants to do.) There are regulations limiting the total hours a truck driver can operate on the highways for public safety, yet doctors are pushed to the physical and mental limits. The military pays the recruits a salary, hazard pay, and sign-up bonuses for those wishing to be soldiers. The soldiers go through boot camp and special schools for desired positions within the service! Doctors go through the same grueling training, yet they pay $60,000.00 a year for the privilege to attend school and start practicing with a $250,000.00 debt. It doesn't make any sense to me why more political pressure isn't applied, making schools more affordable and fewer work hours that seem based on the out modeled tradition of suffering for art. Please start raising the rate of pay for residents. For God's sake, even college athletes are being compensated these days by attending schools on scholarships. Not to get political, but if this country can find 100 billion dollars overnight to send overseas for war, it can surely fund healthcare and med students for the public good.
Go with your gut and tell your son to switch to engineering. He can work in the medical device industy instead. My daughter is working in med devices and has a GREAT work life balance! I would NOT allow my own children to enter a health care career.
@@acd1168 Do you work in health care? I do. I have seen the immense strain that results: financial ruin, relationships that crumble, PTSD and depression.
That would be great if there weren't any crooks...there are medical professionals that exploit the system. I feel bad that the bad ruin it for all the good.
Had to take a few months' FMLA this past year for intensive tx for severe depression, anxiety and burnout. I was fortunate to have a very supportive team and medical director, who jumped in and covered for me on virtually no notice. Time away from work literally saved my life, as did a great behavioral health team. Trying to make the switch from clinical med to administrative/clinician support and writing.
Dang this is rough...I work in the ER and I can tell that it's not just us nurses that are burned out. I see it from my physician colleagues too. And the worst part is that it doesn't seem to be getting any better.
If there's a shortage of nurses, imagine the shortage of DOCTORS. That must be a lot worse. There's not enough doctors and not enough nurses in the medical field.

As a superspeciality resident .. I am working 14 hours per day at hospital minimum and it's mentally n physically exhausting but we are taught to get thru it as it is how we learn. I don't learn anything.. I'm more lost and confused. Patients are understanding but worst is the group of senior colleagues who just berate us for every error. Not required
I'm glad I went into academic research before med school. My PI was terribly abusive. I learned all jobs suck, and now I'm able to tolerate medicine haha

I agree with all the points you are making, and I would like to argue that Physicians have an ethical obligation to prioritize their own mental health and well-being to provide the best possible care for their patients. With physician shortages on the rise and physician burn-out also soaring, I think patient care is being compromised by the lack of value physician’s place on their own wellbeing. Can a burnt out and depressed physician be competent? Yes definitely. But can a burnt out and depressed physician deliver great patient care? I honestly don’t believe so, and here are the reasons why. Physicians who are experiencing burnout or other mental health challenges may be more prone to making errors in their work, which can compromise patient safety. In this way, neglecting physical and mental health goes against the principle of non-maleficence. Physicians also have a duty to maintain a high level of professionalism in their work. This includes demonstrating competence, integrity, and respect for their patients. The goal of healthcare is to improve the health and well-being of patients, and physicians who are depressed or burnt-out may be less able to listen to their patients, empathize with their needs, or communicate effectively with them. Overall, taking care of their mental health is an ethical imperative for physicians. By prioritizing their own well-being, they can improve patient safety, maintain professionalism, provide patient-centered care, and fulfill their moral duty to care for themselves in order to care for others.
I'd be interested to see the differences in physician burnout between countries; especially between the U.S. and countries with socialized healthcare that doesn't require such extensive contact with insurance companies.
For context, I work in the ER. Most of my patients are not critical, but some are sick enough that waiting days/weeks for a surgery or exam could lead to a bad outcome. In Brazil, we have that in both. If you're working in the private sector, burocracy is an issue, and we have to fight sometimes (but not too often) to get an exam/test. If you're working in the public (think NHS) sector, we have to deal with less bucrocracy BUT we have to fight a lot more to get patients to see a specialist, get CT/MRI scan and so on. We end up doing what we can, and that means cutting corners (but not being reckless) and/or giving subpar care, because the alternative is doing nothing. That being said, I'd be lying if I told you that it doesnt affect me. 'I did what I could, I did my best' only goes so far.

Compensation with time off too. Money no good if you can't use it in a positive way. And I not talking a fancy car or home. Time with family and friends

Hi Dr. Cellini, I just watched your previous video of when you were talking about your job in the corporate world with your economics background degree. I went to med school and couldn't finish residency, I have no business background, but I always envied the people working in business because of the wonderful life they get. In that other video you mentioned CEOs of companies are working and they are working more than doctors. How do people in the corporate world cope with job stress and depression/anxiety and burn out overall? I am wondering how people in other job sectors deal with this. I can only speak to medicine and I admit I was always the person looking out the window and wondering what other peoples' lives were.
Dear Dr. Cellini, I also love that you say a "work like balance" is so important to well being and job wellness. Some people say there is no such thing as work life balance, but I think those are the people who didn't carve the time to create a "work life balance." So I think I agree with you and your emphasis on it.
I see the burnout daily amongst my hospital nursing colleagues. Doctors situation is even worse!! Depending upon the specialty there is NO work/life balance! Compensation would be helpful but the next 5-6 survey answers on that list would make more of an impact. Healthcare is broken. I applauded my daughter when she changed her major from premed to computer science!!🥰
I am not in the medical field. I am not sure getting compensated better is the answer. I think fixing the causes of overworking which causes depression is more the issue. Tell the hospital 40 hours is it or tell them to hire more physicians or get some help for the work that takes time away from patients. Would a union work? Getting paid more isn’t really solving the issues people are listing as causes!
My wife has a huge saddle and it was missed at one large hospital and found at another Feb 2023. We have been in and out of the hospital all year for various other reasons. Lack of communication is the most consistent thing in hospitals. I watched my mom die and in the er or admitted for weeks since i was 7. She passed, but it was a 20+ year ride. Now my wife is really sick. If communication increased, hours would decrease and rill down hill. Our entire healthcare system has gone downhill thanks to insurance companies.
Sadly, so many PCPs are moving to the urgent care setting. "Greet em, treat em, street em" and your work is done (most of our MDs sign their charts before the end of the shift). With Amazon in the market, I'm anticipating an evolution to walk in primary care. Craziness!
The issue is nobody is actually dealing with the lack of respect that sometimes comes from health professionals to patients themselves in any speciality of the medicine but health professionals forget about that Don’t they??
I am a bit surprised that pathologists are at the bottom of the list, but I suppose it highly depends on where you work. I am pathologist who works at one of the busiest academic hospitals in the country. I work 11 hours a day (8 am to 7 pm) 6 days a week. The volume of cases and biomarkers for each case has gone up exponentially. I can see how community pathologists who just send out a large chunk of their work to the academic institutions have no stress, but believe me when I tell you that the pathologists who work at the large academic hospitals are some the busiest doctors out there. And yes I am totally burnt out! and seriously, pathologists have to be 100% correct on every report. We are also on call for intra-operative frozen sections, which essentially dictates the management of the patient in real time..
I believe they don't get paid what they deserve; the hospital, insurance company, and big corp. are getting all the money, not the ones that are really doing the work! Everyone that works in the healthcare system constantly gets screwed over.
This is extremely sad. I am sure much of it is involved with Covid, and sorry they don't get help for their depression. I have experienced neglecting care terrible care in the ER and inpatient hospital care. I have always been kind to physicians and nursing staff. But never matter even people that would draw blood, what happened to kindness. I am sad that they burning out; why are they not getting help!
Hi Dr. Cellini! I would like to know, is it possible to become a radiologist and just sit somewhere are read CT, MRI, etc. and give back results? I'm trying to figure out, is it possible to be a doctor but not to see patients and not to be present "at work"? Thanks a lot!
When you are ready to buy the car, the salesman will keep you in the office for seven HOURS for him to finally agree to the price you had in your head. It looks like business people ( the "insurance" company) practice the same techniques with doctors. The insurance company is trying to save money by "playing out the clock" hoping the doctors will not stay online for two hours to get the insurance to approve the drugs. Doctors should unite and stop playing with businesses/insurance companies. Just say right off the bat: " you do not approve? what is your name? i am recording this call and i will make a note in the medical records that you refuse the treatment that the doctor that is treating the patient is recommending because you, a business person that never took a science class, is saying you will not pay for it. And make those calls public if the patient agrees. If all doctors united... or doctors could have their own business person, that never took science, to fight with the insurance business person. I bet you could find lots of people that had their loved one mistreated because of an insurance denial to fill this position. Waiting two hours a couple of times a day every day would drive anybody crazy!!!!
Med students often experience the nervous breakdown from depression & burnout. Not to mention with all the studying and reading....it gets like Dr.! Stat! I can't take this freaking job anymore!! Especially with all the nurses. Surgical techs are hot but often experience Anorexia Nervosa. ❤
Dr. A. S. Cellini I am in doubt between anesthesiology, gastroenterology or oncology/hematology, as they have interesting and not very massive routines. In your opinion, what is the best long-term specialty? I'm afraid to invest in anesthesiology and regret the most insane routine in the future, however I appreciate it more.

Medicine is so political! The hospitals are all for money and CEO and others that have no pt contact and work from an office or home- get paid big bucks! During COVID I never stopped working only when I got it and of course only nursing and allied health peepz working so hard 24/7 BUT NO ONE IN THE BOSSES OFFICES OPEN DURING THAT TIME. 😢😢

As someone who works in a major healthcare system with students I am absolutely cringing at this request. Please be aware that whatever hospital or system Dr. Cellini works for definitely has a set of legal and compliance requirements around shadowing experiences and you need to go to them to make your request.

20 years ago nobody got burnout. You had a few bad days in a row. It's human. Now it's fashionable to complain about burnout. "Lack of respect." Give me a break. How bout burnout among janitors and plumbers? Speak about lack of respect. At least plumbers make up for it by charging more than doctors.
I'm so tired of physicians complaining about their lifestyle...you realize there's a reason physicians get paid such high salaries, it's not supposed to be an easy profession. You knew this was part of the deal when you entered this life. I promise life could be so much worse. Stop and be appreciative of the life you have built. Look at the big picture to get some scope and stop fixating on the menial inconveniences you experience as you go home to your abode with running water and electricity. If you can't handle it, that's okay... do something else.
