Theo shows you during his mission, he can hack ISAC into thinking he was dead or that someone else was him. I assume he did the same for Keener

It's explained in the comms from the Kelso manhunt: Agents have gotten sloppy and rely too heavily on ISAC: they trust anything that ISAC feeds to them on their heads up display through their special contact lenses. Theo warns the agents in The Tombs that ISAC can be fooled and that they need to be more careful. Theo demonstrates this by changing his facial recognition data in the SHD database so that ISAC thinks a dead JTF member is actually him. However, Theo doesn't tell the agents that he also figured out that he can spoof the vital signs coming from an agent's watch that ISAC reads to determine if a rogue agent has been killed. The next biggest factor is that the holographic imaging technology that Theo uses (aka the Decoy) which can project a 3D image that was good enough to fool the agents (at least until ISAC registered their presence) and the Black Tusk. Theo used that tech to project an image of his and Keener's dead bodies over themselves to make it look like they had been killed when they really hadn't. The final thing tying the ruse together was Theo's discovery of the backdoor into ANNA and his success at porting her onto Keener's rogues' local systems. With ANNA's help, Theo could spoof his and Keener's vital signs to make it look like they had died and project an image of their corpse over themselves seamlessly. All that was left was to give the agents from DC a good enough "fight" to be convincing and then drop to the floor, play dead, and hope that the agents would rely on ISAC to confirm the kill. Once Theo confirmed it would work, all Keener had to do was, as he said in his own words, "put on the big show."
