In Israel you pay national insurance like the UK, you then have a choice of 4 health authorities, and if you don't get good service you can move to another one; you can see your GP the same day you requuest it, the maximum wait to see a constant is about 4 days, and an operation about 7 days, both a lot quicker in an emergency - and their very high tec hospitals are far superior to any in the western world.
