I just bought one but it came with plastic head. Is there a way to swap the hose/head?

That’s the standard unit. But I’m sure you can get the metal head.

You can use either. I just cleaned it, and used both.

Some people do. But I never do and only have to clean it once every 5 years. Really.

Same here, I've never empty my Jiffey out, and have never had to clean it. Now if you have extremely hard well water that might be different. I live in Milwaukee, where we get all from Lake Michigan which is considered hard but not extremely hard, and like I say, I've never had to clean and I've been using my for over 10 years.
