Thats the point to show that even teens are involved in drugs and mafias. Plus only half of the main cast is a teenager/young adult, Giorno is 15, Fugo 16 and Narancia 17, the rest are all adults

I think they both just misunderstood each other. I think the narancia boy think that: you take away one, so you get one. It's like you order one take away meal, and that's what you get.
It's true Fugo is younger. Narancia just looks like he's younger than everyone because he's smol, with a slender body and a baby face and all. If he lived to 27 he would have probably looked the same, like a half-grown boy.
