Julián Tavárez's MELTDOWN in game 4 of the 2004 NLCS 

October 17, 2004 (NLCS Game 4) - St. Louis Cardinals' pitcher Julián Tavárez comes in for relief. He gives up the go-ahead run on a Carlos Beltrán home run and then nearly takes Jeff Bagwell's head off, a pitch that would cost Tavárez $10,000 after being fined by the commissioner's office. After escaping a bases-loaded jam, Tavárez breaks his hand when he punches the bullpen phone.
