I've heard it's not permitted to speak first to the Queen, you must wait until you have been asked something... So even Katherine saying "It's a pleasure to meet you, your Majesty," would be a breach of royal protocol. I remember the Australian Prime Minister many years ago accidentally touching the Queen on her back, guiding her through a door or something. It was completely innocent, as one might do to a friend. He'd simply forgotten who was with & slipped up, I guess... But that small thing made the international news as a "scandal" because nobody touches the Queen. Strange customs indeed.
