Hey doc! I’m a licensed esthetician turned optometry technician, in NYS at least you must be a licensed cosmetologist/esthetician to apply lash extensions- generally speaking looking for licensure and further continued education is one way to cautiously look for lash extension technicians

As an anesthesia resident, I diagnosed an OB patient for a C-section with total situs inversus. Listening to her heart sounds, they did seem a little more on the right than usual. I ordered an EKG, which said "Consider limb lead reversal". Then I ordered an echo which showed total situs inverus. Interestingly, she had her previous C-section at the same teaching hospital, the University of Washington. The previous anesthesiologist and obstetricians totally missed it.

We had this in our ED once that I can recall and it was wild. The ER doc did all the initial treatment for the patient and then the ophthalmologist showed up happy as a clam to finish up the job. From what I was told when I discharged the patient, they were able to see out that eye again. So that's good news!

Tip for Canadian audience members- ophtho practices don't always have someone on call for their specific patients, but there will always be someone on call for your region. So if you have an eye problem and can't get in touch with your ophthalmologist, or if you don't have one, try seeing an optometrist! They can do the testing, they are informed on specific conditions, they can refer to the ophthalmologist on call. Much better than sitting in an ER for 12 hours if you don't have something like an ocular trauma or major emergency.

I'm not even in the medical field, and I LOVE Match Week. I love watching their reactions on Match Day, and on the following Friday to see where they matched and in which field. I love it.

Walking out of general surgery rotation for the final time, I felt that so deeply. Second only to finishing trauma surgery floor rotation as an EM intern. By the way, thx for the shoutout to emergency lateral canthotomies, I’ve done it once and it was definitely a scary moment. High impact, low frequency procedure in the ER for sure.

The greatest moment of my life was when I got a phone call asking if I could work (as a paramedic) the last day of my ob/gyn rotation so I just up and left and blasted Lily Allen’s “f- you” as I drove away 😂 OB was the worst month of my life, hands down. It was the most incredibly liberating thing knowing I would never have to be anywhere near an L+D floor ever again.

Making a diagnosis…First, patient is great. I was lucky enough to diagnose cancer twice in a child. First was ALL when he was 6 or 7 and then testicular cancer when he was 15ish. I felt so fortunate to catch both of these by just being more curious than someone else, and finally, diagnosing biliary atresia in a 2.5m old. Also doing very well now. So blessed to be in healthcare as an FNP in Pediatrics.

Jesus, testicular cancer as a teenager. Is he sterile now? Did he make it? My nephew had testicular cancer and he’s sterile now but he froze his sperm before treatment.

Same, froze some as well. Had stem cells from sibling, healthy and strong as a horse :)

Episode Suggestion: Pediatric Opthalmology, the NICU, and Retinopathy of Prematurity. Include rarity of Peds opth, the shortage, and a call to action for recruitment! My son was in the NICU for 65 days after being born at 28 weeks. Had stage 1 RoP that recovered spontaneously by 32 week exam. Desaturated badly due to stress during week 35 eye exam and had bowel injury which was suspected to have been ischemic caused by the desats, but thankfully recovered with a week of NPO, TPN and SMOF, though he did get a PIC line complication from being put back on TPN, which was also minor. Long story short, I am both terrified and very thankful for pediatric opthalmology and would love to hear your insights.

Another hint. If you wear rigid contacts, don't store your decalcification drops near your lens drops. The morning I was taking off for Europe I did. I put the drop in and my eye went crazy! BURN! No time to go to the ER. I flushed it a LOT, and got on the plane. My eye looked like cherry jello for 24 hrs. or so. It recovered. But...be careful.

I had a professor in NP school who worked in primary care-he diagnosed a pheo and came into class the next day soooo excited to tell us about it 😂

That feeling ypu had diagnotisking that rare sickness, my ORL had it when he diagnostic me with cluster headhaches. He was exited like a kid because it was exactly like the books' descriptions. It made his day (and mine). He had a 25 years career before that day.

My husband matched.. he isn’t thrilled. Rural Alabama… he said he will probably be working with a version of texaco mike… so small that there isnt even a walmart or restaurants. Upside… nicer houses are like $1.50. So much to do… selling my house. 😭

On the other hand, rural patients are usually fairly solid folks. When we were military, I always hated finding out our next duty station was going to be in a prohibitively expensive heavily urban area. Plus seeing crime scene tape in your neighborhood really sucks. Personally, I was much happier finding out we had a more rural duty station. Hope your house sells quickly, you find a really nice new home, and you are pleasantly surprised with your peaceful new surroundings.

If you know how to cook, rural medicine sounds interesting. You get to do everything. However, the lack of healthcare for yourself will suck. Probably want to start an emergency travel fund for a plane ticket to Virginia for necessary healthcare, if you know what I mean.

I think for EM, a “pant patient” is a patient that comes in still wearing pants.

I didn't know there was a difference in lab-coat length. In nursing school, We were not told what length to get, so I just got one that looked long enough on me. I think I had a doctor's length. Got it at a Goodwill :)

Another bad point for eyelash extenstions is that they tend to be pricey. I find that is why people compromise and go to some places that offer them cheap. But they are cheap because they have no training and might be using worse products during application.

I'm really curious about the outcome for the person with the orbit compartment event. Was their sight restored? Do you repair the lateral canthotomies? Then what? Thanks for sharing all of these stories.

As a total lay-woman, I would have thought a pants patient for an Emergency Physician was a "skip the ultrasound."

A cotton ball or similar soaked with olive oil or any veg oil and placed on superglued eye will free the eyelashes in 10-20 minutes.

I think the pants patient in the emergency setting is any where we would payage a trauma surgeon. They're the one would have to put on their pants. And stemi. That's the one that makes cardiology and the cath lab crew put on their pants. And somewhere a nephrologist wakes up and puts on these pants for no particular reason.

Ocular pressure is apparently the replacement for horror movies. Can't dr's make the eyelid stay open with stitches, and thereby relieve the pressure on the eye? it sounds as if the actually eyelid has to be removed, and if they are, can they be replaced---like fingers, etc.

My husband needs a surgical rotation for his final rotation… anyone can help? We have two autistic kids… I wanted him here in Jacksonville, Florida, but Im having so many issues nailing down a rotation. His final rotation had to cancel for family reasons. He feels the pull to FMOB, so a rotation with a OB/GYN would be great. We have been apart his entire time at med school and he has to report in June, so just a few weeks together. Anyone have any strings to pull?

13:49 What, are ya worried some freak will figure out your call schedule, drive to Portland, poke themselves in the eye, and feign no light perception for a meet and greet? Ok yeah the internet is a weird place. That would be an oddly specific but valid concern.

That is not the worry as much as if you know someone in the area that had an eye emergency. Then you realize this is a friend_coworker/contact that you know and you now have medical information on that you can connect to a specific person.

Define on call. Is a return phone call three hours later on call I don’t think so. I had plenty of time to get extremely distressed call the emergency room call strange I doctors because my doctor of the last 20 years didn’t call me back for three hours. Let me know.

Curious how much extra you get paid for that week on call? As a low born person, I here complaints from doctors all the time about how hard their life is and burnout. I hear about how hard they work constantly. Do people like this seriously think that the average person wouldn’t work just as hard if they made a comparable amount of money? I’ve heard stories of how the surgeon’s oldest son was sooo pissed because the father (also a surgeon) told him that he cant pay for his final year of med school because he has to pay for the two younger brothers’ college tuitions. I guess its a matter of perspective. Life is tough. But not for people with rich parents. Sorry we dont feel sorry for you

Chip on your shoulder much? Many doctors are saddled with massive debt from their schooling and not everyone makes loads of money. Certainly you probably have a higher number of people with rich parents than some other careers, but it's by no means the default.

I’m an ophthalmologist and have never been paid extra for taking eye call - it’s always just been one of my job responsibilities. I have been paid occasionally for performing emergency ruptured globe repairs- usually on New Year’s Eve/Day or July 4th - trust me, I’d rather have spent that time with my family instead of earning whatever $ I may have been paid.

You’re unhappy about it and I get that but I don’t think it’s fair. It’s not about money and being rich or whatever. A lot of people work really hard no matter whether they get paid well or not, and we all have to appreciate everyone. All of us rely on each other to do all the great jobs that people do for life to carry on as normal, and many of those people work very hard for all of us so I for one thank them all for pushing through even when it’s really tough. We’re all human, regardless of pay a tough job is still a tough job.
